Inhibition of inflammatory cells delays retinal degeneration in experimental retinal vein occlusion in mice.

The role of microglia in retinal inflammation is still ambiguous. Branch retinal vein occlusion initiates an inflammatory response whereby resident microglia cells are activated. They trigger infiltration of neutrophils that exacerbate blood-retina barrier damage, regulate postischemic inflammation and irreversible loss of neuroretina. Suppression of microglia-mediated inflammation might bear potential for mitigating functional impairment after retinal vein occlusion (RVO). To test this hypothesis, we depleted microglia by PLX5622 (a selective tyrosine kinase inhibitor that targets the colony-…

Chronic myeloid leukemia presenting with bilateral central retinal vein occlusion and massive retinal infiltrates.

Visual impairment as the initial symptom of leukemia, especially chronic myeloid leukemia, is rare, occurring in approximately 3% of all leukemias in children. We describe the case of a 14-year-old boy with vision loss attributable to bilateral central retinal vein occlusion and massive retinal infiltrates. Bone marrow biopsy demonstrated the BCR-ABL translocation, and the patient was diagnosed with chronic myeloid leukemia. A major cytogenetic remission was achieved 6 months after initiation of therapy.

Concurrent central retinal artery occlusion and branch retinal vein occlusion in giant cell arteritis.

Ophthalmic involvement in giant cell arteritis can manifest in a number of ways. Central retinal artery occlusion is one of the common causes of visual loss in giant cell arteritis. On the contrary, branch retinal vein occlusion is rarely associated with the latter. We report an 89-year-old lady with acute left central retinal artery occlusion on a background of progressive decline in vision over a 6-month period with a concurrent right branch retinal vein occlusion. Subsequent investigation confirmed giant cell arteritis on temporal artery biopsy. This is the first reported case of a concurre…

The treatment of branch retinal vein occlusion with bevacizumab.

PURPOSE OF REVIEW: New treatment modalities for branch retinal vein occlusion have recently been introduced. The role of intravitreal bevacizumab injections will be discussed and compared with laser photocoagulation and other novel intravitreal pharmacotherapies. RECENT FINDINGS: Argon laser photocoagulation is the single treatment for branch retinal vein occlusion that has been shown to reduce vision loss in a randomized controlled clinical trial. The effectiveness of this treatment is limited though. Currently, increasing data support the role of intravitreal bevacizumab as an effective tre…

Combined occlusion of the central retinal artery and vein.

A 56-year-old man, complaining of sudden visual loss in his left eye, demonstrated cream-colored retinal edema along a macular branch of the central retinal artery, overall delay of fluorescence in angiography and general depression of the central visual field, which were interpreted as the incomplete form of central retinal artery occlusion (CRAO) mimicking cilioretinal or branch retinal artery occlusion. After receiving paracentesis and fibrinolytic agents, the patient recovered his vision gradually, while the ophthalmoscopic findings progressed to show central retinal vein occlusion (CRVO) …

Development of a new mouse model of branch retinal vein occlusion and retinal neovascularization.

PURPOSE: Retinal neovascularization (NV) is associated with various disorders, such as retinal vein occlusion, diabetic retinopathy, and retinopathy of prematurity, and often causes severe loss of vision. To determine the mechanism of retinal NV and develop new therapy, we developed a mouse model using a photodynamic method. METHODS: C57BL/6 mice were injected with rose bengal via the tail vein, and then selected venous points were photocoagulated. RESULTS: All eyes demonstrated venous occlusion on day 1, and capillary nonperfusion areas were observed until day 3. Twenty of 33 eyes (60.6%) d…

Complement C3/C3aR Signaling Pathway Inhibition Ameliorates Retinal Damage in Experimental Retinal Vein Occlusion.

PURPOSE: Retinal vein occlusion (RVO) is a common retinal vascular disease that severely threatens visual function. This study aims to elucidate the role of the complement C3/C3aR signaling pathway in a laser-induced RVO mouse model and to explore its potential as a therapeutic target. METHODS: RVO was induced in C57BL/6J mice using laser photocoagulation combined with photosensitizer dye administration. Two days later, retinal tissues were collected for bulk RNA sequencing. The activation of the C3/C3aR signaling pathway was validated through RT-qPCR and Western blot. The C3aR antagonist SB2…

TGF-β1-induced apoptosis in retinal endothelial cells is implicated in retinal vein occlusion.

Retinal vein occlusion (RVO) is a serious vascular condition that impairs vision due to retinal endothelial cell injury and apoptosis. This study aimed to identify key molecular pathways and therapeutic targets involved in RVO pathogenesis. Transcriptomic analysis of the retinal tissues from a mouse RVO model was performed to identify differentially expressed genes and co-expression modules associated with RVO. Protein-protein interaction network analysis pinpointed putative hub genes. In vitro experiments using human retinal microvascular endothelial cells (HRMECs) validated the involvement o…

Case report: indocyanine green dye leakage from retinal artery in branch retinal vein occlusion.

BACKGROUND: Little is known about retinal vascular lesions underlying hemorrhage in the acute phase of branch retinal vein occlusion (BRVO). CASE: A 64-year-old woman presented with a decrease in visual acuity of ten-day duration in her left eye. OBSERVATIONS: At the initial examination, her left fundus showed the typical appearance of BRVO, including retinal bleeding and soft exudates in the lower half of the fundus, after the arteriovenous crossing. Fluorescein angiography showed no detail in the retinal vessels, which were occluded by retinal bleeding. However, in the early phase of indoc…

Localized retinal detachment with combined central retinal artery and vein occlusion after retrobulbar anesthesia.

The authors report a patient who was observed to have a localized retinal detachment with combined central retinal artery and vein occlusion after cataract surgery performed with retrobulbar anesthesia. The authors propose that this condition resulted from injection of the anesthetic mixture into the optic nerve. No acute neurologic symptoms occurred, but visual loss was severe and permanent. This case adds to the previously reported spectrum of complications from retrobulbar anesthesia.

Branch Retinal Vein Occlusion Revealing Previously Undiagnosed Systemic Disease.

Branch retinal vein occlusion (BRVO) is a common cause of vision loss and frequently reflects an underlying systemic vascular disorder. Early systemic assessment is fundamental for preventing ocular and cardiovascular complications. This report describes the clinical case of a 61-year-old woman who presented with sudden, painless visual field loss in the left eye. Fundoscopic and angiographic findings confirmed superotemporal BRVO with macular edema. Intravitreal anti-vascular endothelial growth factor (VEGF) therapy was initiated, and referral to a primary care physician for systemic evaluati…

Central retinal vein occlusion : an uncommon complication in sarcoidosis.

Sarcoidosis is a multi-system granulomatosis of unknown etiology, defined by the presence of epithelioid and gigantocellular granulomas, without caseous necrosis. Ocular sarcoidosis manifests mainly as bilateral granulomatous anterior uveitis. Occlusion of the central retinal vein in sarcoidosis is a rare manifestation, which is the particularity of our observation. We report the case of a patient presenting with unilateral central retinal vein occlusion associated with granulomatous anterior uveitis on the same side. Systemic manifestations and further investigations led to the diagnosis of s…

Retinal arterial and vein occlusion: is surgery ever indicated?

PURPOSE OF REVIEW: To highlight the recent progression in surgical treatments for central retinal vein occlusion (CRVO) and central retinal artery occlusion (CRAO). RECENT FINDINGS: Anti-VEGF treatment, accepted as a primary treatment for CRVO, is unable to effectively treat all types of the diseases. Regarding CRAO, there are not any accepted therapies available. There have however been recent innovations in surgery, such as utilizing robotics-assisted tools in cannulation procedures for central retinal artery occlusion, or micro-cystotomy for refractory macular edema resulting from ischemic…

Chorioretinal venous anastomosis for non-ischemic retinal vein occlusion.

Retinal vein occlusion (RVO) refers to the occlusion of the central retinal vein or primary and secondary branches caused by multiple factors. Clinical treatments for it include intravitreal or systemic vasodilator application, local usage of steroids and NSAID (non-steroidal anti-inflammatory drugs), thrombolysis, hemodilution, retinal laser photocoagulation, vitrectomy with vascular sheath incision, chorioretinal venous anastomosis (CRVA), and so on. At present, most treatments are aimed at RVO complications, while chorioretinal vein anastomosis can fundamentally reflux retinal vein blood th…
