Gender-Based Differences in the Biomechanical Behavior of the Thorax During CPR Maneuvers.

In this study, 18 rib cages (8 males and 10 females) were segmented from computer tomography (CT) images. In order to analyze the potential differences in thoracic biomechanics during cardiopulmonary resuscitation (CPR), a set of numerical experiments was conducted using finite elements (FE). Compression forces were applied at different points on the rib cage. Results indicated that the optimal compression area for both sexes is the sternum at the 5th rib level, requiring the least force to achieve the desired compression depth. Males required greater force than females. Among females, those w…

Assessment of a fully-parametric thoraco-lumbar spine model generator with articulated ribcage.

The present paper describes a novel user-friendly fully-parametric thoraco-lumbar spine CAD model generator including the ribcage, based on 22 independent parameters (1 posterior vertebral body height per vertebra + 4 sagittal alignment parameters, namely pelvic incidence, sacral slope, L1-L5 lumbar lordosis, and T1-T12 thoracic kyphosis). Reliable third-order polynomial regression equations were implemented in Solidworks to analytically calculate 56 morphological dependent parameters and to automatically generate the spine CAD model based on primitive geometrical features. A standard spine CA

[The morphological and morphometric study of amianthoid transformation of the costal cartilage in health and in keeled chest deformity in children].

UNLABELLED: Amianthoid transformation (AT) is the accumulations of abnormal collagen structures (amianthoid fibers) in the hyaline cartilages, tumors, and tendons. Neither functional value of costal cartilage matrix AT, nor its role in the pathogenesis of congenital chest deformities is known now. AIM: to examine the morphological features of AT in the costal cartilage of children with the normal and keeled chest. SUBJECTS AND METHODS: Costal cartilages were studied in 6 children with the normal chest (autopsy material) and in 5 ones with keeled chest (surgical material). Tissue fragments we

The injury pattern in fatal suicidal falls from a height: an examination of 307 cases.

Falls from a height are a common cause of suicidal death in urban settings. The aim of the present work is to describe and discuss a specific set of 307 autopsies performed in a metropolitan city such as Milan, Italy from 2006 to 2011, with a special focus on the relationship between the injury pattern and height of the fall, age at death and weight at death. The rib cage (92%), the lungs (76%), the heart (53%) and the liver (58%) turned out to be the most injured structures in the whole population. Age resulted statistically significant for heart, thoracic aorta, mesentery, kidney hilus, hip …

[Rib stress fractures in rowers: three case reports and review of literature].

Rib stress fractures with an occurrence rate of between 2 and 12% of all other overuse injuries of rowers are the cause of the most time lost from on-water training and competition. As the rib stress fractures are very infrequent in the general population, of otmost importance is to think about such possibility in case of rib cage pain in athlete without a history of trauma. That is why we present our three cases of rowers in whom we suspected the existence of rib stress fractures. The review of the literature related to rib stress fractures in rowers with the newest knowledge of etiology, cli…

Effect of asbestos-related pleural fibrosis on excursion of the lower chest wall and diaphragm.

To examine mechanisms responsible for reduced lung volumes (restriction) in asbestos-related pleural fibrosis (APF), we studied diaphragm function and lower rib-cage excursion in 26 subjects with previous asbestos exposure and no evidence of asbestosis. Using posteroanterior (PA) and lateral chest radiographs taken at residual volume and at 25%, 70%, and 100% vital capacity (VC) during a slow inspiratory maneuver, we measured fractional expansion of the lower rib cage (FErc), fractional shortening of the diaphragm (FSdi), and changes (Delta) in diaphragm dome height (Hdo) and subphrenic volume…

Exercise improvement after pectus excavatum repair is not related to chest wall function.

OBJECTIVES: In patients undergoing corrective surgery for pectus excavatum, there is evidence of improvement in cardiopulmonary function. It is unclear how much of this improvement is attributable to improved chest wall function. Thus, we observed changes in chest wall function in response to an incremental load exercise pre- and postoperatively. METHODS: Using optoelectronic plethysmography, total and regional chest wall volumes were measured in 7 male patients with severe pectus excavatum who underwent a Nuss correction. Rib cage and abdominal volumes were recorded at rest and during exerci…

Traumatic rupture of thoracic aorta in real-world motor vehicle crashes.

OBJECTIVE: Traumatic rupture of thoracic aorta (TRA) was reported in the literature to be a major cause of death in motor vehicle crashes. This study aims at evaluating the most relevant risk factors of TRA. It also aims at analyzing the types of TRA as a function of car crash conditions and rib cage fractures. METHODS: In-depth crash data collected from 1998 to 2006 as part of the Co-operative Crash Injury Study (CCIS) were retrospectively investigated to assess frontal, near-side, and far-side injury risks. This database includes 15,074 occupants with individual detailed autopsy reports. Mu…

Flail chest injury in a patient with surgical absence of the sternum.

Although pulmonary disability is usually minimal following total sternectomy, the potential for remote complications exists. We present a case of severe respiratory failure resulting from simple rib fractures occurring two years after sternectomy for sternal osteomyelitis. Respiratory failure resulted from flail chest due to the combination of acute rib fracture and the lack of anterior fixation of the thoracic cage.

Rib Fractures With Concomitant Spinal Fractures May Benefit From Surgical Stabilization.

Surgical stabilization of rib fractures has demonstrated benefits in patients with complex thoracic injuries. Limited information exists regarding patients with thoracic injuries and concomitant spinal injuries. We hypothesized that patients who suffer both thoracic cage and spinal fractures and undergo surgical fixation (FIX) will have improved outcomes compared to non-fixation (NFIX) patients. In our retrospective review, adult patients with rib injuries from 2015 to 2019 were pooled from the National Trauma Data Bank. Mortality with FIX rib fractures with spinal fractures decreased by 6.1%

The anatomical growth of the thoracic cage in adolescents with specific reference to axial growth comparing the right and left hemithorax.

This study has demonstrated the changing volume of both the anterior and posterior thorax in normal adolescents (without spinal or thoracic deformity), differentiating for both sex and age, to further understand how the thorax grows, along with the differences in growth between the anterior and posterior thorax. The thorax was measured on axial CT slices at every vertebral level from T3 to T12 in a series of scans previous taken for routine clinical care. Measurements taken were the anteroposterior thoracic distance and the area of the anterior and posterior rib prominences on either side of t

Thoracic cage volume and dimension assessment by optoelectronic molding in normal children and adolescents during growth.

PURPOSE: The thoracic spine, the chondral and osseous ribs, and the sternum together make up the thoracic cage. These elements are strictly correlated, although their growth is not synchronous. The purpose of this study is to provide a comprehensive data set of thoracic dimensions and non-invasive volumetric assessment in a large cohort of males and females from early childhood to young adult age. METHODS: In all, 622 healthy individuals (406 girls, 216 boys) aged 6-18 years were consecutively enrolled between 2006 and 2016. All had to be healthy with no history of spinal deformity, or any lu

Comparison between the phase angle and phase shift parameters to assess thoracoabdominal asynchrony in COPD patients.

Determining the presence of thoracoabdominal asynchrony in chronic obstructive pulmonary disease (COPD) patients is clinically relevant, but there is no consensus on the optimal parameters for performing this analysis. We assessed 22 COPD patients (FEV1 40 ± 10% predicted) and 13 healthy controls during rest and exercise with optoelectronic plethysmography (70% maximum workload) on a cycle ergometer. Thoracoabdominal asynchrony was calculated by using phase angle and phase shift parameters following a three-compartment model involving the upper and lower rib cages and abdomen. Patients were cl…

Finite element analysis of pectus carinatum surgical correction via a minimally invasive approach.

Pectus carinatum (PC) is a chest deformity caused by a disproportionate growth of the costal cartilages compared to the bony thoracic skeleton, pulling the sternum towards, which leads to its protrusion. There has been a growing interest on using the 'reversed Nuss' technique as a minimally invasive procedure for PC surgical correction. A corrective bar is introduced between the skin and the thoracic cage and positioned on top of the sternum highest protrusion area for continuous pressure. Then, it is fixed to the ribs and kept implanted for about 2-3 years. The purpose of this work was to (a)

Association of thoracic cage fractures and pericardial effusion in blunt trauma.

BACKGROUND: Several case reports suggest that penetrating thoracic cage fractures are an important cause for hemopericardium and cardiac tamponade following blunt trauma. However, the prevalence of this mechanism of injury is not fully known, and considering this association may provide a better understanding of the utility of cardiac component of the FAST (Focused Assessment with Sonography for Trauma). OBJECTIVE: To determine the association of thoracic cage fractures and pericardial effusion in patients with blunt trauma. METHODS: We performed a retrospective, multicenter cohort study usi

Effect of lung volume on the respiratory action of the canine pectoral muscles.

Lung volume influences the mechanical action of the primary inspiratory and expiratory muscles by affecting their precontraction length, alignment with the rib cage, and mechanical coupling to agonistic and antagonistic muscles. We have previously shown that the canine pectoral muscles exert an expiratory action on the rib cage when the forelimbs are at the torso's side and an inspiratory action when the forelimbs are held elevated. To determine the effect of lung volume on intrathoracic pressure changes produced by the canine pectoral muscles, we performed isolated bilateral supramaximal elec…

Thoracic trauma in newborn foals.

In a report describing life ending fractures (255 horses) from the Livestock Disease Diagnostic Center, Kentucky (1993 and 1994), 32 foals had rib fractures. The purpose of our study was to examine the incidence of rib fractures in newborn foals on a Thoroughbred studfarm by physical and radiographic examination, to determine factors which may contribute to the problem and to document any clinical consequences. All foals (263) included were examined within 3 days of birth. The thoracic cage was palpated externally for abnormalities and all foals were placed in dorsal recumbency to evaluate tho

Unilateral chest wall paradoxical motion mimicking a flail chest in a patient with hemilateral C7 spinal injury.

After a lower hemi-cervical spinal cord injury, a patient presented with a left hemiplegia and on the same side a unilateral chest wall paradoxical motion mimicking a flail chest. X-rays demonstrated a left hemilateral C6 injury but no rib fractures. We demonstrated that the paradoxical motion was due to the action of the diaphragm acting on the rib cage with intercostal respiratory paralysis on the side of hemiplegia.
