Rickets in osteopetrosis--a paradoxical association.

Osteopetrosis is a hereditary bone disease with intense positive balance of body calcium. Infantile variety is often associated with rickets--a paradoxical association. Two siblings with osteopetro rickets are reported in the article. The pathophysiologic mechanism of the paradoxical association has been explained and various management options have been discussed. Both cases were treated with high dose calcitriol and calcium supplements.

Rickets screening in the preterm infant.

Although rickets in the preterm infant has decreased with improvements in care and nutrition, there continue to be infants at high risk for this disease. Early diagnosis and treatment can prevent fractures and other complications, such as decreased linear growth. Nurses must know which infants are at risk, must be aware of the appropriate screening tests, and must know how to customize care for at-risk neonates.

Radiology of rickets, osteomalacia and hyperparathyroidism.

In osteoporosis, the amount of mineralized bone is diminished, but the remaining bone is of normal quality. In rickets and osteomalacia the trabeculae are irregularly mineralized, a feature reflected in the X-ray image. In hyperparathyroidism there is generalized and focal bone resorption leading to demineralization and poor definition of bone on the X-ray.

Etiology of rickets in Nigerian children.

We studied 26 Nigerian children with active rickets (13 boys, 13 girls), aged 1 to 5 years, and compared results of biochemical studies with those of healthy control subjects. The plasma 1,25-dihydroxyvitamin D level was elevated (568 +/- 317 pmol/L) and the 25-hydroxyvitamin D level was (36 +/- 28 mol/L) in the children with rickets compared with the control subjects (369 +/- 134 nmol/L and 69 +/- 22 nmol/L, respectively). The results suggest that rickets in Nigeria is largely the result of calcium deficiency and that vitamin D deficiency and possibly end organ resistance may be contributory …

Rickets and osteomalacia are still around.

The terms rickets and osteomalacia describe gross, histologic, and radiologic abnormalities common to more than 50 diseases that vary in cause and clinical presentation. Rachitic and osteomalacic syndromes seen in current clinical practice usually are resistant to vitamin D forms and are most often caused by phosphate loss or 1,25(OH)2D deficiency, or both. Radiographic identification of rachitic or osteomalacic syndromes and accurate diagnosis of a particular cause is essential, as many patients' conditions can be improved or cured with proper therapy.

Pycnodysostosis with visceral manifestation and rickets.

Pycnodysostosis is a rare bone disease. Visceral manifestations associated with anemia and/or rickets have been reported in pycnodysostosis. Five children with typical findings of pycnodysostosis with hepatosplenomegaly, anemia with rickets, one with visceromegaly and anemia, and another with rickets alone are reported here. These findings strongly suggest that extramedullary erythropoiesis does occur in pycnodysostosis.

Aetiological factors in rickets of prematurity.

Six very preterm (< 32 weeks' gestation) infants who developed late-onset respiratory distress were each matched for sex and gestation with 2 control preterm infants. Radiologically and biochemically the diagnosis of rickets and rachitic respiratory distress seemed clear and the pattern conformed with other reports of the syndrome. The control infants were of similar gastational ages but there was a significantly higher incidence of pre-eclampsia in the pregnancies of index cases. Also significant was a prolonged illness of several weeks' duration in the index cases; this illness was either he…

Rickets and osteomalacia of various origins.

Further causes of rickets or osteomalacia need to be added to the list previously published by the author. These include phosphate deficiency osteomalacia from prolonged consumption of aluminum hydroxide, rickets and osteomalacia from prolonged therapy by antiepileptic drugs, rickets associated with the severe form of osteopetrosis, further rare causes of the Fanconi syndrome, congenital rickets occurring when the mother has osteomalacia from dietary causes or has celiac disease and temporary neonatal rickets probably due to enzymic immaturity. The importance is stressed of defining as closely…

Serum MEPE-ASARM-peptides are elevated in X-linked rickets (HYP): implications for phosphaturia and rickets.

MEPE (Matrix Extracellular PhosphoglycoprotEin) expression is markedly elevated in X-linked-hypophosphatemic-rickets (HYP) and tumor-induced osteomalacia (TIO). In normal individuals, circulating serum-levels of MEPE are tightly correlated with serum-phosphorus, parathyroid hormone (PTH) and bone mineral density (BMD). Also, MEPE derived, C-terminal ASARM-peptides are candidate minhibins and/or phosphatonins. Our aims were to determine: 1. whether MEPE-ASARM-peptide(s) are abnormally elevated in HYP/hyp serum, and, 2. whether the ASARM-peptide(s) accumulate in hyp mice kidney renal-tubules. Us…

[Updates on rickets and osteomalacia: anti-FGF23 antibody, a new therapeutic approach for hypophosphatemic rickets/osteomalacia].

In the last decade, Fibroblast growth factor (FGF) 23 has been shown to be a causative factor of hypophosphatemic rickets/osteomalacia, and considered as potential therapeutic target for this disease. Now the most attention is paid to FGF23 blockades as a new category of therapy that may replace the current supplementation of phosphate and active vitamin D. In this chapter, pharmacological actions of the FGF23-neutralizing antibodies in murine hypophosphatemic rickets/osteomalacia model are reviewed. In addition, other types of FGF23 blockades are also briefly discussed.

[Diagnostic significance of calcitonin analysis in children with rickets and rickets-like diseases administered calcium test].

The secretory activity of C-cells of the thyroid was investigated in 25 children with rickets and rickets-like diseases. There were 13 children with grades II and III rickets, 6 with vitamin D-dependent rickets, and 6 with Debre-de Toni-Fanconi disease. Five normal children constituted a control group. The basal concentration of calcitonin did not differ from the control parameter in all patients' groups. An increased release of the hormone to the blood was noticed in response to calcium stimulation. The rise of blood calcitonin was determined by the disease pattern, which made it possible to …

Wilson's disease manifesting as refractory rickets.

Wilson's disease is a metabolic disorder affecting the liver, brain, skin and osteo-muscular organs. Refractory rickets is an unusual phenomenon in Wilson's disease. This 27-year-old man was primarily treated for rickets for 20 years without success and later developed features of tremulousness of limbs and dysphonia when he was evaluated for Wilson's disease. On treatment with a chelating agent, he showed clinical improvement neurologically. This case report emphasises on the consideration of Wilson's disease early in a case of rickets for better clinical outcome.

Nutritional rickets - Vitamin D and beyond.

Vitamin D deficiency has been considered to be the cause of nutritional rickets for most of the past century. During the past two decades, however, it has become clear that nutritional rickets may be caused by vitamin D deficiency or by dietary insufficiency of calcium. The combined deficiencies of calcium and vitamin D interact, and several other factors are also relevant in the pathogenesis of nutritional rickets.

Tumor-induced rickets-osteomalacia: an enigma.

Objective We report a case of pediatric thoracic tumor-induced osteomalacia (TIO) causing severe hypophosphatemic rickets with delayed diagnosis and emphasize on timely management of this rare entity. Case presentation A young boy presented with rickets since five years of age. Biochemical evaluation revealed hypophosphatemia, hyperphosphaturia, elevated alkaline phosphatase and normal calcium levels. Initially managed as hereditary hypophosphatemic rickets, he was given phosphorus supplements and calcitriol. Despite the therapy, skeletal deformities worsened requiring surgical corrections. Su…
