Safety of outpatient cerclage.

One hundred one women who underwent scheduled McDonald cerclage were studied. Fifty outpatients were reviewed prospectively, and 51 inpatients were studied retrospectively and concurrently. Forty-five outpatients and 48 inpatients completed pregnancy. The fetal outcomes were similar for both groups, with no significant differences in the number of complications for newborn infants. The average cost of the procedure (exclusive of physician fees) for outpatient cerclage was $546 and for inpatient, $1,174. The difference clearly shows the cost-effectiveness of outpatient cerclage.

Safety profile of the quinolones.

The most important finding from preclinical evaluation of fluoroquinolones has been their arthropathogenic potential in young animals. This toxic effect is found with all quinolones known so far and has led to the decision not to use them in children and adolescents, despite the fact that the significance of the effect for humans is still unclear. The mutagenic potential of the drugs seems to be low although bacterial DNA-metabolism is a major target of their action. Newer in-vitro methods to study topoisomerases from bacterial and mammalian cells are suitable to detect differences in the deri

The safety of beta-carotene.

Epidemiological studies have associated low dietary and/or plasma level of carotenoids with higher incidences of certain cancers. This evidence has led the National Cancer Institute to initiate more than a dozen prospective clinical trials in which supplements of beta-carotene alone, or in combination with other micronutrients, are being taken. In these trials, the beta-carotene supplements are given in the range of 15-50 mg/day. The safety of this level of intake is well documented. beta-Carotene has been successfully used to treat inherited photosensitivity diseases for more than 15 years at…

Safety profile of cefixime.

The clinical and laboratory safety of cefixime based on analysis of data from 1575 adults (1118 treated daily and 457 treated twice a day) and 615 children (299 treated daily and 316 treated twice a day) in studies of urinary tract and lower and upper respiratory tract infections (including otitis media) is reviewed. The incidence of adverse clinical experiences and occurrence of laboratory abnormalities were similar to those seen with other beta-lactam antimicrobial agents. Gastrointestinal side effects were the predominant adverse experiences seen in both daily and twice daily programs in ad…

The safety of blood transfusions.

Blood transfusion therapy carries a small risk of complications-usually minor-which include allergic reactions, hemolysis, and the transmission of infections. Fatal hemolytic transfusion reactions are rare and are usually due to human error resulting in administration of ABO incompatible blood. Viral hepatitis (usually non-A, non-B) remains the major infectious complication, with a risk of two to five percent. The transmission by transfusion of AIDS has also been reported, but the risk is much lower-about 0.001%. As yet, no blood substitute is available for clinical use. Routine hepatitis B te

The tolerance and safety of azlocillin.

The safety of azlocillin was evaluated in 631 patients treated for urinary tract or systemic infections in U.S.A. clinical trials. The mean azlocillin dose was 260 mg/kg/day and the mean duration of treatment was 11.1 days. Twenty patients (3.2%) experienced adverse local reactions and 92 patients (14.6%) experienced adverse systemic reactions. In thirty-one instances (4.9%) they led to premature termination of therapy, but only 14 of 135 reactions were classified as severe. All adverse reactions were reversible if adequate follow-up was done. Hypersensitivity reactions, manifest by rash, feve…

Safety aspects of midazolam.

The LD50 in the rat and the mouse is about 1600 mg/kg (oral administration) and 75 mg/kg (rat) and 50 mg/kg (mouse) on intravenous administration. Subchronic oral studies over 13 weeks in doses of 5, 15 and 45 mg/kg/day in the dog and 50, 100 and 200 mg/kg/day in the rat have demonstrated minimal toxicity for midazolam, as for other benzodiazepines. High doses produced increased liver weight in the rat and the expected increases in alkaline phosphatase in the dog (species-specific reaction). Detailed blood and urine analyses as well as histological examination of organs produced no indication

Radiation safety for children.

In recent years there has been much public and professional concern about reducing patient exposure to radiation. This concern is especially appropriate for children because of their greater sensitivity to it. Exposure risks to children may be reduced by using radiography only when there is a specific clinical indication. Bitewing radiographs are indicated for detection of approximal caries in the child when the molar contacts are closed. The frequency of this examination should be determined on an individual basis. Other radiographs should be obtained only when there are clinical signs or sym

Safety of Compounded Medications.

Compounded medications are medications that are prepared by compounding pharmacies to meet individual patients' needs that cannot be met by FDA-approved brand name or generic medications. This may be the case when a patient needs a different dose or route of administration or if a patient is allergic to a certain ingredient in an FDA-approved medication. While compounded medications help to fill a necessary gap, they can have significant safety issues. Due to lesser regulatory oversight, patient safety may be compromised due to contamination or inaccurate potency of compounded medications. Com…

An update on the safety of hydroquinone.

Hydroquinone has been used for years for multiple conditions, including melasma, post-inflammatory hyperpigmentation, dyschromia from photoaging, and solar lentigines. It is known to be a very effective lightening agent, but several concerns have been raised about this widely used agent. The recent U.S. ban on over-the-counter skin lightening products containing hydroquinone has prompted further questioning of the safety of this widely used agent. While there have been prior informative, large-scale reviews on the safety of hydroquinone, new findings have since been reported. Here, we provide …

Safety of Endoscopy in Cirrhosis.

BACKGROUND: Endoscopic procedures are among the most commonly performed medical procedures and the serious adverse event rate is reported to be 1-3 adverse events per 1000 procedures. AIMS: Here, we have examined the safety of endoscopy specifically in cirrhotic populations. METHODS: We conducted a retrospective case (cirrhosis)-control (non-cirrhosis) study of the outcomes of patients undergoing endoscopy in a large academic medical center. The primary outcome was a procedural or post-procedural complication. Complete clinical data were collected for all patients undergoing endoscopic proce…
