Secondary Prevention in Older Adults: Putting It All Together.

Secondary prevention of cardiovascular disease (CVD) in older adults plays a pivotal role in reducing morbidity and mortality, improving quality of life, and minimizing health care costs. This aspect of care is essential in enhancing life expectancy as the aging population is increasingly affected by CVD. This review focuses on the strategies, challenges, and recent advancements in secondary prevention for elderly populations. It examines pharmacologic and nonpharmacological interventions, discusses the barriers to effective implementation, and highlights the need for individualized care strat

Aspirin Dosing in Secondary Prevention of Atherosclerotic Cardiovascular Disease.

PURPOSE OF REVIEW: To compile research evaluating low- versus high-dose aspirin efficacy and safety in patients with known cardiovascular disease. RECENT FINDINGS: The ADAPTABLE randomized controlled trial of ~ 1,500 participants with cardiovascular disease found similar ischemic efficacy and bleeding safety of low- versus high-dose aspirin at a median follow-up of 26 months. High-dose aspirin does not have improved efficacy and increases bleeding risk in secondary cardiovascular disease prevention. As such, patients with cardiovascular disease should be maintained on low-dose aspirin.

Secondary Prevention Strategies for Ischemic Stroke in Antiphospholipid Syndrome.

Introduction: Antiphospholipid syndrome (APS) is an autoimmune prothrombotic disorder associated with both venous and arterial thrombosis, most notably ischemic stroke. Patients face a high risk of recurrence, and yet optimal strategies for secondary prevention remain uncertain. Methods: We conducted a narrative review of the literature on secondary prevention of ischemic stroke in APS. We performed a comprehensive literature search of PubMed for English-language articles on secondary stroke prevention in APS. Studies were included if they were original human research (e.g., randomized trials,

Patients' preferences for secondary prevention following a coronary event.

OBJECTIVE: Despite clear evidence on the effectiveness of secondary prevention, patients with coronary artery disease frequently fail to reach guideline-based risk factor targets. Integrating patients' preferences into treatment decisions has been recommended to reduce this gap. However, this requires knowledge about patient treatment preferences. Therefore, through a survey study, we aimed to explore which risk factors patients self-perceived, prioritised for improvement, and needed support with after a recent hospitalisation for coronary heart disease. METHODS: A digital questionnaire was p

Secondary fracture prevention in primary care: a narrative review.

The global burden of osteoporosis continues to rise with an ageing population. Untreated osteoporotic fractures not only heighten the risk of subsequent fractures but are associated with excess mortality. Although primary care guidelines consistently stress the importance of secondary fracture prevention, fewer than 20% of patients are appropriately treated for osteoporosis following an initial osteoporotic fracture. This worldwide phenomenon is known as the osteoporosis care gap. This literature review examines the barriers to secondary fracture prevention in primary care and evaluates the ef

Primary, Secondary, and Tertiary Prevention of Congenital Cytomegalovirus Infection.

Cytomegalovirus infection is the most common congenital infection, affecting about 1% of births worldwide. Several primary, secondary, and tertiary prevention strategies are already available during the prenatal period to help mitigate the immediate and long-term consequences of this infection. In this review, we aim to present and assess the efficacy of these strategies, including educating pregnant women and women of childbearing age on their knowledge of hygiene measures, development of vaccines, screening for cytomegalovirus infection during pregnancy (systematic versus targeted), prenatal

Skin cancer: Primary, secondary, and tertiary prevention. Part I.

Skin cancer is the most commonly diagnosed cancer worldwide. Understanding the natural history of skin cancer provides the framework for the creation of prevention and control strategies that aim to reduce the skin cancer burden. Based on the target (individual vs population), disease stage, and risk factors (modifiable vs nonmodifiable), strategies can be categorized into 4 levels-health promotion (also known as primordial prevention), primary prevention, secondary prevention, and tertiary prevention. This is the first of a 2-part review, which will cover the epidemiology, risk factors, primo

Blood Pressure in Acute Stroke and Secondary Stroke Prevention.

PURPOSE OF REVIEW: Hypertension is common in patients presenting with stroke and is independently associated with unfavorable outcomes. This article reviews current guidelines for early management of blood pressure (BP) and highlights the findings of recent investigative works. RECENT FINDINGS: Intensive blood pressure reduction after receiving alteplase has not been shown to improve outcomes. Patients with large vessel occlusions may benefit from lower blood pressure targets post-intervention. Retrospective analyses of large intracerebral hemorrhage trials suggest that specific subgroups of

Acute Secondary Prevention of Ischemic Stroke: Overlooked No Longer.

Recent studies of interventions initiated acutely following onset of minor ischemic stroke or transient ischemic attack (TIA) have disclosed early stroke recurrence rates that are substantially higher than long-term recurrence rates and that can be reduced by acute antiplatelet treatment interventions. These observations, bolstered by analysis based on kinetic modeling of the time course of recurrence following initial events, suggest that acute stroke patients experience an underlying vulnerable state that quickly transitions to a more stable state. Some evidence also supports the benefits of

Post-myocardial infarction rehabilitation and secondary prevention in hospitals.

BACKGROUND: Norwegian studies have documented poor cardiovascular risk factor control and a high incidence of new cardiovascular events in myocardial infarction patients. There is little knowledge about the scope of secondary prevention treatment and cardiac rehabilitation in Norwegian hospitals. Therefore, we wanted to conduct a survey of discharge routines and outpatient follow-up after myocardial infarction. MATERIAL AND METHOD: In October 2018, the heads of cardiology departments and nurse managers/physiotherapists at cardiology outpatient clinics at all Norwegian hospitals (N = 51) were

Secondary stroke prevention in patients with patent foramen ovale.

PURPOSE OF REVIEW: Although a patent foramen ovale (PFO) is an established risk factor for cryptogenic ischemic stroke, strategies for secondary prevention remain controversial. Increasing evidence over the past decade from well designed clinical trials supports transcatheter PFO closure for selected patients whose stroke was likely attributable to the PFO. However, patient selection using imaging findings, clinical scoring systems, and in some cases, thrombophilia testing, is crucial for determining patients most likely to benefit from closure, anticoagulation, or antiplatelet therapy. Effect and efficacy of lifestyle interventions as secondary prevention.

INTRODUCTION: Improvements in health behaviour are often recommended as part of secondary prevention in patients with stroke and transient ischaemic attack. However, there is a lack of knowledge as to how this is applied in clinical practice. AIM: In this systematic review and meta-analysis, we examined the effect of counselling or educational intervention directed at individual or multiple behavioural risk factors on blood pressure and other reported outcomes. METHODS: PubMed, Embase, PsycInfo, CINAHL, Scopus and Web of Science were systematically searched. Meta-analyses were conducted on a

New Horizons in Pharmacologic Therapy for Secondary Stroke Prevention.

IMPORTANCE: Even with currently available therapies and lifestyle modifications following an ischemic stroke, there remains a substantial residual lifetime risk of stroke recurrence and cardiovascular morbidity. This review summarizes emerging novel therapeutic approaches that have demonstrated signals of efficacy for prevention of noncardioembolic stroke from phase II and phase III randomized clinical trials (RCTs) and provides an overview of drug regimens that have had promising results in primary stroke prevention and could be considered for further evaluation. OBSERVATIONS: After a minor

[Secondary prevention of chronic tonsillitis exacerbations by bacteriophage medications].

Chronic tonsillitis is one of the most common diseases to modern people, which treatment requires a preventative care course approach. The general purpose of the present research could be framed as follows: to estimate clinical efficiency of bacteriophage medications, in the case of 'Ostophag' mixture for preventative care. Study population consisted eighty people. Participants were divided into two groups (forty patients in each) both diagnosed compensated and decompensated forms of chronic tonsillitis with pathogenic microflora in the lacunae of the palatine tonsils. In the first group we bu

[Secondary prevention of ischemic stroke: overview of updated guidelines].

Secondary prevention of ischemic stroke: overview of updated guidelines Abstract. It is widely stated that stroke is one of the most common diseases and the leading cause of severe disability in adults. Secondary stroke prevention is based on drug treatment, risk factor control as well as interventional procedures such as carotid artery revascularization or percutaneous device closure of a patent foramen ovale in cryptogenic stroke patients younger than 60 years.

Controversies about the Secondary Prevention of Spontaneous Preterm Birth.

Preterm birth is one of the major global health problems and part of the Millennium Development goals because of the associated high number of perinatal or neonatal mortality and long-term risks of neurodevelopmental and metabolic diseases. Transvaginal sonography has meanwhile been established as a screening tool for spontaneous preterm birth despite its relatively low sensitivity when considering only the cervical length. Vaginal progesterone has been shown to reduce prematurity rates below 34 weeks in a screening population of singleton pregnancies. Up to now, no positive long-term effect c

HIV and coronary disease - When secondary prevention is insufficient.

Highly active antiretroviral therapy (HAART) has created a new paradigm for human immunodeficiency virus (HIV)-infected patients, but their increased risk for coronary disease is well documented. We present the case of a 57-year-old man, co-infected with HIV-2 and hepatitis B virus, adequately controlled and with insulin-treated type 2 diabetes and dyslipidemia, who was admitted with non-ST elevation acute myocardial infarction. Improving adherence to medication for secondary cardiovascular disease prevention.

Survivors of myocardial infarction are at risk of recurrent events and have an annual death rate of 5%. Advances in treatment and, in particular, the interventional management of myocardial infarction have seen important mortality gains over recent decades, yet cardiovascular diseases remain the biggest killer in many European countries. Change in lifestyle and adherence to medication to prevent further events are key to the recurrence of future events following myocardial infarction, but adherence to medication for the secondary prevention of cardiovascular disease is a profound problem.
