The Evolution of Shiga Toxin-Producing Escherichia coli Characterization and Risk Categorization Strategies.

Shiga toxin-producing Escherichia coli (STEC) are a group of E. coli that cause food and waterborne enteric illness. STEC are genotypically and phenotypically diverse as they do not have a single evolutionary origin, instead multiple STEC lineages have appeared independently following horizontal gene transfer events. The diversity of STEC is also reflected in an apparent variability in the potential of individual STEC strains to cause severe illness (bloody diarrhea or hemolytic uremic syndrome) or be involved in large foodborne outbreaks. In the context of food safety decision making, the var…

Shiga toxin-centered pathophysiology defines the therapeutic limits of enterohemorrhagic Escherichia coli infection.

EHEC infection is a toxin-driven systemic disease primarily mediated by the production of Shiga toxin (Stx). Stx is encoded by lysogenic bacteriophages, and SOS-linked prophage induction can increase toxin production in response to environmental or therapeutic stimuli. Once expressed, the toxin rapidly disseminates systemically and binds to cells expressing the globotriaosylceramide (Gb3) receptor. The toxin is then internalized via retrograde intracellular trafficking pathways, ultimately causing inhibition of protein synthesis. These characteristics impose a limited therapeutic window for ef…

Colistin- and cefotaxime-resistant Shiga toxin-producing Escherichia coli (STEC) in buffalo meat.

In this study, Shiga toxin-producing E. coli (STEC) were detected in 64% of all tested buffalo meat samples, distributed as 62% of buffalo meat chops and 66% of ground buffalo meat. Six serotypes (O55:H7, O111:H8, O145, O157:H7, O26:H11, and O103:H2) were identified, of which, O55:H7 (37.26%) and O111:H8 (21.29%) were the most predominant. The eaeA, stx1, stx2, and EHEC-hlyA were detected by multiplex PCR in all E. coli isolates at different percentages, and multiple virulence patterns with stx2/stx1/eae were the most prevalent pattern indicating that the serotypes will be more virulent in pat…

Monocytes serve as Shiga toxin carriers during the development of hemolytic uremic syndrome.

Shiga toxin (Stx)-induced hemolytic uremic syndrome (HUS) poses a life-threatening complication for which a definitive treatment remains elusive. To exert its cytotoxic effect on renal cells, Stx must be delivered from the infected intestines to the kidney. However, the mechanism underlying Stx delivery remains unclear. Here we pinpoint monocytes as the primary carriers responsible for transporting Stx2 to the renal region. Through single-cell sequencing analysis of Stx2-B-bound peripheral white blood cells sorted by flow cytometry, we observe that nearly all monocytes exhibit strong Stx2-B bi…

A Successful Approach to Diagnosing Shiga-like Toxin-Producing Escherichia coli-Induced Colitis.

Shiga-like toxin-producing Escherichia coli (STEC) is a well-known cause of foodborne acute diarrheic diseases, especially in children and the elderly. The potentially fatal complications associated with toxin production range from bloody diarrhea and ischemic colitis to kidney failure, hemolytic-uremic syndrome (HUS), and colon perforation. Here, we describe a case and literature review of STEC-induced colitis, highlighting the clinical features and the necessary tools for the best diagnostic approach and management. Facing challenging differential diagnosis, ranging from ischemic colitis and

Diagnosis and Treatment for Shiga Toxin-Producing Escherichia coli Associated Hemolytic Uremic Syndrome.

Shiga toxin-producing Escherichia coli (STEC)-associated hemolytic uremic syndrome (STEC-HUS) is a clinical syndrome involving hemolytic anemia (with fragmented red blood cells), low levels of platelets in the blood (thrombocytopenia), and acute kidney injury (AKI). It is the major infectious cause of AKI in children. In severe cases, neurological complications and even death may occur. Treating STEC-HUS is challenging, as patients often already have organ injuries when they seek medical treatment. Early diagnosis is of great significance for improving prognosis and reducing mortality and sequ…

Phylogenetic context of Shiga toxin-producing Escherichia coli serotype O26:H11 in England.

The increasing use of PCR for the detection of gastrointestinal pathogens in hospital laboratories in England has improved the detection of Shiga toxin-producing Escherichia coli (STEC), and the diagnosis of haemolytic uraemic syndrome (HUS). We aimed to analyse the microbiological characteristics and phylogenetic relationships of STEC O26:H11, clonal complex (CC) 29, in England to inform surveillance, and to assess the threat to public health. There were 502 STEC belonging to CC29 isolated between 2014 and 2019, of which 416 were from individual cases. The majority of isolates belonged to one…

Molecular epidemiology of Shiga toxin-producing O113:H21 isolates from cattle and meat.

The serotype O113:H21 is considered one of the relevant non-O157 STEC serotypes associated with severe human infections. Due to the increased detection of O113 strains and their relationship with clinical cases, which emphasizes the importance of this serogroup as an emerging pathogen, our aim was to determine the characteristics of STEC O113:H21 strains circulating in bovine cattle and retail meat from Argentina. For this purpose, we determined the presence and combinations of various virulence genes (and their variants) related to adhesion and toxicity in a collection of 34 isolates. Their g

Latrunculin B facilitates Shiga toxin 1 transcellular transcytosis across T84 intestinal epithelial cells.

Shiga toxins (Stx), released into the intestinal lumen by enterohemorrhagic Escherichia coli (EHEC), are major virulence factors responsible for gastrointestinal and systemic illnesses. These pathologies are believed to be due to the action of the toxins on endothelial cells, which express the Stx receptor, the glycosphingolipid Gb3. To reach the endothelial cells, Stx must translocate across the intestinal epithelial monolayer. This process is poorly understood. We investigated Stx1 movement across the intestinal epithelial T84 cell model and the role of actin turnover in this transcytosis. W…

Sorbitol-fermenting Shiga toxin-producing Escherichia coli O157: indications for an animal reservoir.

This study investigates a sorbitol-fermenting enterohaemorrhagic Escherichia coli (SF EHEC) O157 infection in a farmer's family in the Austrian province of Salzburg. The investigation commenced after a 10-month-old boy was admitted to hospital with the clinical diagnosis of a haemolytic-uraemic syndrome (HUS) and his stool specimen grew SF EHEC O157:H-. In a subsequent environmental survey, a stool specimen of the 2-year-old brother and faecal samples of two cattle from the family's farm were also found to be positive for SF EHEC O157:H-. All four isolates had indistinguishable phenotypic and

Intestinal bleeding and occlusion associated with Shiga toxin-producing Escherichia coli O127:H21.

We report a case of a nine-year old boy with vomiting, abdominal pain and fever, who underwent surgery with a diagnosis of appendicitis in Mendoza and from whom a Shiga toxin-producing Escherichia coli (STEC) O127:H21 strain was recovered. Forty-eight hours after surgery he presented bilious vomiting and two episodes of intestinal bleeding. Laboratory findings included: hematocrit, 35%; blood urea nitrogen, 0.22 g/L. The urinary output was normal. The following day physical examination showed an alert mildly hydrated child, without fever but with distended and painful abdomen. The patient was …

Cytotoxicity of enterohemorrhagic Escherichia coli correlates with Shiga toxin rRNA-N-glycosidase activity but not with toxin subunit ratios.

The expression of Shiga toxins (Stx) is the major pathogenicity factor of enterohemorrhagic Escherichia coli (EHEC). Stx structurally consist of one enzymatically active A-subunit (StxA) and a pentamer of non-covalently linked B-subunits (StxB). While the B-subunit mediates binding to the cell surface receptors globotriaosylceramide (Gb3) and globotetraosylceramide (Gb4), as well as the cellular uptake of the toxin, the A-subunit exhibits a rRNA-N-glycosidase activity leading to inhibition of protein synthesis and consequently apoptosis of the eukaryotic target cells. Recent work has demonstra…

Prevalence, characterisation and clinical profiles of Shiga toxin-producing Escherichia coli in The Netherlands.

Detection of Shiga toxin-producing Escherichia coli (STEC) in The Netherlands is traditionally limited to serogroup O157. To assess the relative importance of STEC, including non-O157 serogroups, stool samples submitted nationwide for investigation of enteric pathogens or diarrhoea were screened with real-time PCR for the presence of the Shiga toxin genes. Patients were selected if their stool contained blood upon macroscopic examination, if they had a history of bloody diarrhoea, were diagnosed with haemolytic uraemic syndrome, or were aged <6 years (irrespective of the bloody aspect of the s…

Shiga toxins stimulate secretion of interleukin-8 from intestinal epithelial cells.

In the 1980s, Shiga toxin (Stx)-producing Escherichia coli O157:H7 (STEC) was identified as a cause of hemorrhagic colitis in the United States and was found to be associated with hemolytic uremic syndrome (HUS), a microangiopathic hemolytic anemia characterized by thrombocytopenia and renal failure. The precise way that Stxs cause hemorrhagic colitis and HUS is unclear. Stxs have been thought to cause disease by killing or irreversibly harming sensitive cells through a nonspecific blockade of mRNA translation, eventually resulting in cytotoxicity by preventing synthesis of critical molecules …

Antibodies to Shiga toxin in the serum of children with Shigella-associated haemolytic uraemic syndrome.

Antibodies to Shiga toxin (Stx) were measured in the sera of 49 children with Shigella dysenteriae serotype 1 infection, of whom 17 had haemolytic uraemic syndrome (HUS) and 32 had no complications (uncomplicated shigellosis, UCS). Children with HUS had lower levels of total IgG and IgM and lower IgM titres to Stx than those with UCS. The number of children with neutralising antibodies was similar in the two groups. Of the children with HUS, 11 had HUS on enrolment and six developed HUS subsequent to enrolment. Antibody titres in children who subsequently developed HUS were compared with those…

Epidemiology of Single-Etiology Non-Shiga Toxin-Producing Escherichia coli Outbreaks Reported to the National Outbreak Reporting System, with Comparison to Shiga-Toxin Producing E. coli and Shigella, United States, 2009-2023.

Reducing the occurrence of enteric illnesses and outbreaks requires a comprehensive understanding of their epidemiology. While the epidemiology of many diseases, such as Shiga toxin-producing Escherichia coli (STEC) infection, is well-characterized in the United States, the epidemiology of infections caused by E. coli other than STEC (non-STEC) is not. This report describes non-STEC outbreaks reported to the National Outbreak Reporting System (NORS) during 2009-2023. Historically, non-STEC infection was considered travel-associated with limited domestic transmission. During 2009-2023, 230 dome…

Prevalence and Implications of Shiga Toxin-Producing E. coli in Farm and Wild Ruminants.

Shiga-toxin-producing Escherichia coli (STEC) is a food-borne pathogen that causes human gastrointestinal infections across the globe, leading to kidney failure or even death in severe cases. E. coli are commensal members of humans and animals' (cattle, bison, and pigs) guts, however, may acquire Shiga-toxin-encoded phages. This acquisition or colonization by STEC may lead to dysbiosis in the intestinal microbial community of the host. Wildlife and livestock animals can be asymptomatically colonized by STEC, leading to pathogen shedding and transmission. Furthermore, there has been a steady up

Shiga toxin-induced haemolytic uraemic syndrome and the role of antibiotics: a global overview.

OBJECTIVES: The administration of antibiotics in infections caused by Shiga toxin producing E. coli (STEC) strains, such as O157:H7, was and remains controversial, as it has been associated with the development of haemolytic uraemic syndrome (HUS). We conducted a literature review to better examine this association. METHODS: We searched the PubMed and Google Scholar databases for relevant articles, using the key words: ``haemolytic uraemic syndrome'', ``Shiga toxin'', ``E. coli O157:H7'', ``E. coli O104:H4'', ``STEC colitis'', ``STEC antibiotics'', ``STEC fosfomycin'', ``STEC trimethoprim sul…
