[Reconstruction following shoulder resection for bone tumor].

Scapula and proximal humerus are the most frequent site of primary bone tumors of upper limb. Surgical reconstruction after resection, aimed to obtain a stable painful limb and active motion of the shoulder. Three major key-points can affect functional results: intra- or extra-articular resection, ability to offer a strength fixation of rotator cuff tendons, and remaining function of the deltoid muscle after resection. When most of the deltoid muscle is active associated with an intra-articular resection, recent results of reversed prosthesis are very promising in terms of active motion. For o

Comparison of Survivorship and Failure Modes Between Anatomic and Reverse Total Shoulder Arthroplasty Across Multiple Government Joint Registries for a Single Platform Shoulder System.

INTRODUCTION: We analyzed two different government joint registries for survivorship associated with one platform shoulder system and compared reasons for revision and trends in usage of anatomic total shoulder arthroplasty (aTSA) and reverse total shoulder arthroplasty (rTSA) over a period of more than 10 years to elucidate reasons for any changes in market trends. METHODS: A review of the United Kingdom (UK) and Aus- tralian national joint registries was performed for a single platform shoulder prosthesis (Equinoxe; Exactech, Inc, Gainesville, Florida, USA) from 2011 to 2022 to investigate …

Chronic unreduced dislocations of the shoulder.

We evaluated the results of treatment in twenty-three patients with twenty-four shoulder dislocations that had gone unreduced for at least three weeks. Fourteen dislocations were posterior, eight were anterior, and one each was superior and inferior. Seventy-nine per cent of the posterior dislocations had not been recognized by the initial treating physician. Fourteen shoulders (58 per cent) were operated on. Of seven that were treated by open reduction with preservation of the humeral head, the results in two were graded as excellent; in three, as good; and in two, as fair. A Neer total shoul

"Rhin sling," A Novel Type of Shoulder Sling in Post-Stroke Shoulder Subluxation: A Case Report.

This case report introduces a novel type of shoulder prosthesis in 2 patients with hemiplegic shoulder subluxation. A unique reel traction device was incorporated to allow easy traction and accurate correction of joint subluxation. X-ray images taken before and after application showed immediate correction effects that were maintained up to 2 hours after application with no change of sling position. These 2 cases support the idea that this new type of shoulder sling could be applied for therapeutic and corrective purposes in hemiplegic stroke patients with shoulder subluxation.

Optimizing baseplate position in reverse total shoulder arthroplasty in small-sized Japanese females: technical notes and literature review.

The management of cuff tear arthropathy (CTA) has always been a challenge for shoulder surgeons. Introduction of reverse total shoulder arthroplasty (RTSA) helped in providing pain relief and improved shoulder function in patients with CTA. In this study, we aimed to evaluate the short-term clinical results and some clinical details regarding the types of available prosthesis, positioning, and size of the components for RTSA in a population of short-stature female Japanese. In our seven cases, the average glenoid size was 23.9 mm in width and 34.2 mm in height. The average width was smaller th

Reverse shoulder arthroplasty for proximal humeral fractures: update on indications, technique, and results.

The introduction of the reverse shoulder arthroplasty has provided shoulder surgeons with more options for the treatment of complex proximal humeral fractures in the elderly. Early reported results suggest that the average functional outcome may be better than hemiarthroplasty in certain patients and specific clinical scenarios. In addition, these results seem to be reached more quickly with less dependence on rehabilitation. The reverse prosthesis may be particularly useful in patients aged older than 70 years, especially those with severely comminuted fractures in osteopenic bone. These fact

Resurfacing shoulder arthroplasty for the treatment of severe rheumatoid arthritis: outcome in 167 patients from the Danish Shoulder Registry.

BACKGROUND AND PURPOSE: There is no consensus on which type of shoulder prosthesis should be used in patients with rheumatoid arthritis (RA). We describe patients with RA who were treated with shoulder replacement, regarding patient-reported outcome, prosthesis survival, and causes of revision, and we compare outcome after resurfacing hemi-arthroplasty (RHA) and stemmed hemi-arthroplasty (SHA). PATIENTS AND METHODS: We used data from the national Danish Shoulder Arthroplasty Registry and included patients with RA who underwent shoulder arthroplasty in Denmark between 2006 and 2010. Patient-re…

Stabilisation of a posteriorly unstable glenohumeral joint during total shoulder arthroplasty: A novel capsulorrhaphy technique.

Restoration of the soft tissue balance is of great importance during shoulder arthroplasty. We report a novel technique of posterior capsulorrhaphy through an anterior approach for the management of intraoperative posterior instability during implantation of the prosthesis. After the operation, the patients were able to follow a rehabilitation program with some minor restrictions, avoiding the necessity of a shoulder spica or other constrictive measures. This technique is simple, safe and sufficient to address instability due to abundance of the posterior shoulder capsule.

Tuberoplasty reduces resistance force in dynamic shoulder abduction for irreparable rotator cuff tears: a cadaveric biomechanical study.

BACKGROUND: Arthroscopic tuberoplasty is an optional technique for managing irreparable rotator cuff tears. However, there is a lack of studies investigating the resistance force during shoulder abduction in cases of irreparable rotator cuff tears and tuberoplasty. HYPOTHESES: In shoulders with irreparable rotator cuff tears, impingement between the greater tuberosity (GT) and acromion increases the resistance force during dynamic shoulder abduction. Tuberoplasty is hypothesized to reduce this resistance force by mitigating impingement. STUDY DESIGN: Controlled laboratory study. METHODS: Ei

Reverse shoulder arthroplasty: State-of-the-art.

The reverse shoulder arthroplasty conceived by Paul Grammont in 1985 has gradually gained popularity as a treatment for multiple shoulder diseases. Unlike previous reverse shoulder prosthesis characterized by unsatisfactory results and a high glenoid implant failure rate, the Grammont design has immediately shown good clinical outcomes. This semi constrained prosthesis solved the issues of the very first designs by medializing and distalizing the center of rotation with an increased stability of the replacement of the component. The indication was initially limited to cuff tear arthropathy (CT…

Development of novel 3D-printed robotic prosthetic for transradial amputees.

BACKGROUND AND AIM: Upper extremity myoelectric prostheses are expensive. The Robohand demonstrated that three-dimensional printing reduces the cost of a prosthetic extremity. The goal of this project was to develop a novel, inexpensive three-dimensional printed prosthesis to address limitations of the Robohand. TECHNIQUE: The prosthesis was designed for patients with transradial limb amputation. It is shoulder-controlled and externally powered with an anthropomorphic terminal device. The user can open and close all five fingers, and move the thumb independently. The estimated cost is US$300.

Shoulder surgery: from cuff repair to joint replacement. An update.

REPAIRING FULL THICKNESS CUFF TEARS.--Despite the sound rationale for repairing full-thickness rotator cuff tears, the procedure may fail to restore cuff integrity, which is indispensable to optimal cuff function. The functional role of each cuff muscle and the factors associated with anatomic failure (particularly those related to the muscles and tendons) provide a basis for rational patient selection and for determination of the best surgical strategy on a case-by-case basis. Bone remodelling of the scapula after a total shoulder arthroplasty.

According to Wolff's law, the changes in stress after a prosthesis implantation may modify the shape and internal structure of bone, thus compromising the long-term prosthesis fixation and, consequently, be a significant factor for glenoid loosening. The aim of the present study is to evaluate the changes in the bone adaptation process of the scapula after an anatomical and reverse total shoulder arthroplasty. Five finite element models of the implanted scapula are developed considering the implantation of three anatomical, cemented, all-polyethylene components; an anatomical, cementless, meta

A new technique for reconstruction of the proximal humerus after three- and four-part fractures.

The results of proximal humeral replacement following trauma are substantially worse than for osteoarthritis or rheumatoid arthritis. The stable reattachment of the lesser and greater tuberosity fragments to the rotator cuff and the restoration of shoulder biomechanics are difficult. In 1992 we developed a prosthesis designed to improve fixation of the tuberosity fragments in comminuted fractures of the proximal humerus. The implant enables fixation of the fragments to the shaft of the prosthesis and the diaphyseal fragment using screws, washers and a special toothed plate. Between 1992 and 20

Frequency, severity, and implications of shoulder pain in people with major upper limb amputation who use prostheses: Results of a National Study.

BACKGROUND: People with upper limb amputation are potentially at increased risk of shoulder pain because they often perform compensatory movements to operate their prostheses and rely more heavily on their nonamputated limb for everyday activities. OBJECTIVE: To describe the frequency, severity, associated factors, and implications of shoulder pain in people with unilateral major upper limb amputation who use prostheses. DESIGN: Cross-sectional, observational design. SETTING: National recruitment of people living in the community. PARTICIPANTS: U.S. veterans and civilians (N = 107) with un
