Assessment of the multiple dimensions of nausea: the Nausea Profile (NP).

Nausea, unlike emesis, is a subjective experience that is difficult to describe to others, be they clinicians or researchers. Previous research has been limited to examining the frequency, severity, and duration of nausea. The goal of this study was to design a questionnaire that would allow for the evaluation of the characteristics of nausea across individuals and situations. This study consisted of 4 stages: descriptors were generated, categorized into 3 dimensions, and reevaluated to verify their reliability; in the final stage, the developed checklist was compared to a visual-analogue-scal

Attributions and adjustment to life-threatening illness.

An analysis of the role of attribution in major illness and serious injury is presented. Evidence reviewed includes the impact of illness variables on attributions, the association between attributions and adjustment to illness, and the proposed mechanisms of this association. Illness and injury characteristics such as severity and time since diagnosis appear to relate to attributional activity and content, but the association between attributions and psychological or physical adjustment is weak. Overall, it would appear that the attribution construct can describe individuals' reactions to lif

Psychiatric aspects of bone marrow transplantation: a review and current issues.

BMT represents a major medical advance and offers to most individuals who undergo this procedure the only hope for long term survival. The current psychiatric literature is anecdotal and mostly restricted to discussion of psychiatric issues related to the BMT hospitalization itself. There is substantial agreement about the important psychiatric issues concerning BMT recipients, their families, and the BMT team. However, many post-BMT hospitalization psychiatric and psychosocial issues need attention: the psychological impact of being a BM donor, the consequences of BMT for donor-recipient rela

Drug treatment of depression in medically ill patients.

Depressive symptoms are common in medically ill patients although depressive disorders are considerably underdiagnosed and undertreated. Drug treatments for depression are reviewed in terms of a risk/benefit analysis. The main benefit is approximately to double the chance of recovery (from about 30 to 65%), with possible associated improvements in physical condition. The risks of treatment are considerable and include overdose, unwanted effects at therapeutic dose and interaction with other drugs. Among the risks associated with specific medical conditions are orthostatic hypotension, cardioto

Head lice in drawings of kindergarten children.

Drawings by kindergarten children were analyzed in order to study the emotional reaction of these children to head lice. The choice of color was significant (black was used by 43% of the children, indicating that the subject of lice is associated with anxiety and fear) as were the unhappy faces and omissions of mouths in the drawings. While we know today that lice do not discriminate between clean and dirty hair, parents and professionals react very negatively to the condition, and this feeling is transferred to children as indicated by their drawings.

Neurasthenia and chronic fatigue syndrome: the role of culture in the making of a diagnosis.

Chronic fatigue syndrome is an increasingly popular diagnosis consisting of multiple psychiatric and somatic symptoms. It bears a striking resemblance to the nineteenth-century diagnosis of neurasthenia. Both disorders arose during periods characterized by a preoccupation with commerce and material success and major changes in the role of women. They illustrate the role of culture in the development of a new diagnosis that emphasizes a "medical" rather than "psychiatric" etiology. The authors argue that chronic fatigue syndrome will meet the same fate as neurasthenia--a decline in social value

Screening indexes in DSM-III-R somatization disorder.

A sample of 196 general medical patients with multiple unexplained somatic complaints was used to compare the performance (sensitivity and specificity) and positive and negative predictive values for the published screening indexes for somatization disorder. This study shows similar performances for both the DSM-III-R and the Swartz et al. screening indexes for somatization disorder with use of DSM-III-R diagnostic criteria. By use of Receiver Operating Characteristic (ROC) curve analysis, graphs of achievable sensitivity and specificity through the range of possible symptom thresholds allow f

Self-care within a model for demand for medical care.

Self-care is interpreted from a health economic point of view. Various approaches are presented. It is stressed that the decision-oriented approach used by other health service researchers is an integral part of the economic approach to the topic as is the idea of a continuum of care, from self-care to professional care. A new approach is taken to the modeling of self-care, in that self-care becomes part of a four-part demand for care model. This makes it possible to model the demand for care for three different groups separately: 1--persons with zero episodes; 2--persons with pure illness epi

Disclosure of traumas and psychosomatic processes.

Results from a series of studies are summarized in support of a general theory of inhibition and psychosomatics. According to this view, to inhibit thoughts, feelings, or behaviors is associated with physiological work. In the short term, inhibition results in increased autonomic nervous system activity. Over time, inhibition serves as a cumulative stressor that increases the probability of psychosomatic disease. Actively avoiding thoughts and feelings surrounding a trauma and/or not discussing a trauma is a particularly insidious form of inhibition. The results from recent surveys and experim

Tourette's syndrome symptom onset at age thirty-five.

It is apparent to the authors that the primary factors influencing this particular case of Tourette's Syndrome are psychological. The condition appeared to result from threatening environmental conditions causing repression. The repressed material manifested itself in multiple tics, echolalia and coprolalia. Because of this patients's extreme resistance to accept the true conditions of his situation, individual and group psychotherapy have been difficult modes of treatment.

Catecholaminergic influences on motion sickness.

The evidence for possible roles of catecholamines in motion sickness is reviewed. l-Dopa is implicated in that it is the precursor to catecholaminergic neurotransmitters. Dopamine may have a role in stimulating motion sickness, though much basic information is lacking. Epinephrine may have a stimulatory role, since anatomical, pharmacological and physiological evidence is consistent with such a role. Norepinephrine probably inhibits motion sickness, as indicated by anatomical, physiological and pharmacological evidence. Information is missing that is critical to the understanding of the roles

Job stressors and long-term sick leave due to depressive disorders among Japanese male employees: findings from the Japan Work Stress and Health Cohort study.

BACKGROUND: Research on the association between job strain or other job stressors and depressive disorders is still limited. The purpose of the present study was to investigate the prospective association of job strain, role stressors and job insecurity with long-term sick leave due to depressive disorders. METHODS: A prospective study was conducted of a total of 15 256 men aged 18-67 years with no previous history of mental disorders employed in six manufacturing factories located in several regions of Japan. At baseline, they were surveyed using a self-administered questionnaire, including
