OBJECT: A growing skull fracture (GSF) is a rare but significant late complication of skull fractures, usually occurring during infancy and early childhood. Delayed diagnosis and improper treatment could exacerbate this disease. The aim of this study was to introduce a new hypothesis about, describe the stages of, and discuss the treatment strategy for GSF.
METHODS: The authors performed a retrospective review of 27 patients with GSF, who were grouped according to 3 different GSF stages.
RESULTS: Over a period of 20 years, 27 patients with GSF (16 males and 11 females) were treated in the au…

We describe a full term male infant born by cesarean section, who presented a deep round depression of the left parietal bone at birth. MRI scan showed no signal alteration on cortical and sub-cortical level in correspondence of the depressed skull fracture. At four months of age, a skull X-ray was normal. At 18 months of age growth and neurologic follow-up is normal.

We report a case of pontine infarct that occurred after facial trauma with a fracture of the skull base. Reports of trauma-induced pontine infarcts have generally involved cervical trauma. This is the first report to our knowledge after facial trauma with skull base fracture. We suggest that intracranial vertebro-basilar dissection could be a possible mechanism.

INTRODUCTION: Elevation and repair of an open depressed skull fracture is often thought of as an emergency procedure. Common indications for emergent elevation of a depressed skull fracture have been dural tear, seizure, gross contamination or mass effect from bone or a sizable underlying intracerebral hematoma. As treatment of head injury moves towards management of cerebral perfusion pressure (CPP) rather than intracranial pressure (ICP), we sought a way to maximize CPP in the initial treatment of head-injured patients with depressed skull fractures that would eventually require surgery by d…

We report a case of bitemporal hemianopsia after skullbase fracture. An 18-year-old male presented with frontal head hit due to a traffic accident. Consciousness level on admission was JCS 10. Initial CT scan revealed traumatic SAH and pneumocephalus. We treated him conservatively. Ten hours after the accident, consciousness level went down, and CT imaging disclosed bilateral frontal hemorrhage. The three dimensional CT (3D-CT) imaging showed two fracture lines from the roof of the ethmoid sinus to the planum sphenoidale. Although his consciousness improved gradually, he complained of rhinorrh

A case of fracture of the skull with progressive separation of the fracture line in a seven-month-old is reported. The patient presented with a swelling in the right parieto-occipital area and paresis of the upper left limb; there was no clear history of trauma. The roentgenogram of the skull disclosed a large bone defect and the cerebral CT scan showed herniation of intracranial contents through this defect, confirming the diagnosis of fracture of the skull with progressive separation. In this form of skull fracture, the fracture line separates gradually and herniation of intracranial content

In recent literature the question of the convenience of neurosurgical treatment of cranial depressed fracture of the newborn has been a subject of discussion. The authors review their experience with 4 cases of this condition, that stress the necessity of the operation. Most reported series have dealt with the short-term results of the different ways of treatment. In this article the outcome is evaluated at least 10 years after surgery.

A patient in whom cryptococcal meningitis complicated a nine year old depressed frontal skull fracture, an association which has not been reported previously, is recorded. This is also the first case of cryptococcal meningitis recognised in Ethiopia.

AIM: To investigate the contribution of routine review of submillimetric multiplanar reformats to the diagnosis of skull-base fractures.
METHODS: A prospective analysis was performed of 407 cases referred over a 6-month period for CT of the skull following cranial trauma. The reformatted 5-mm axial sections and subsequently the high-resolution multiplanar reformats (HRMPRs) were viewed on an ADW 4.1 workstation using bone windows and algorithm. All skull-base fractures and related features, recorded by the consensus of two radiologists, were classified as anatomically significant or non-signi

This is a case study describing the finding of a depressed skull fracture in a neonate who was delivered without instrumentation and with no history of trauma. Depressed skull fractures are described as being associated with forceps delivery both vaginally and with caesarean section but are much rarer without instrumentation. This obvious abnormality was very concerning for the parents as it was not picked up on antenatal scans and there was no clear cause. There were both cosmetic and neurological concerns and we found no clear consensus on appropriate treatment and prognosis in the literatur…

BackgroundSkull fractures are a common heterogenous finding in patients with traumatic brain injury (TBI). Their clinical significance, particularly of displaced and nondisplaced fractures, remains a topic of debate. This study aims to compare injury severity and clinical outcomes between patients with and without skull fractures.MethodsA retrospective study of prospectively triaged adult trauma patients with blunt TBI and radiographic evidence of intracranial hemorrhage (ICH) or skull fracture was performed. Patients were grouped based on the presence or absence of skull fracture. Multivariab…

When infants are admitted to the hospital with skull fractures, providers must distinguish between cases of accidental and abusive head trauma. Limited information about the incident is available in such cases, and witness statements are not always reliable. In this study, we introduce a novel, data-driven approach to predict fall parameters that lead to skull fractures in infants in order to aid in determinations of abusive head trauma. We utilize a state-of-the-art finite element fracture simulation framework to generate a unique dataset of skull fracture patterns from simulated falls. We th…

BACKGROUND: Growing skull fractures (GSF) represent a rare complication following linear traumatic skull fractures with an underlying dural tear, mainly occurring during infancy and early childhood. GSF can cause encephalocele, hydrocephalus, or encephalomalacia, potentially leading to long-term neurological sequelae. Therefore, prompt diagnosis and early treatment is paramount.
METHODS: We outline our surgical reconstruction technique for managing GSF.
CONCLUSION: GSF is effectively treated with a dural and bony reconstruction, using a fast-resorbable polymer mesh.

Growing skull fracture (GSF) is an uncommon form of head trauma among young children. In prior research, the majority of GSFs were typically classified based on pathophysiological mechanisms or the duration following injury. However, considering the varying severity of initial trauma and the disparities in the time elapsed between injury and hospital admission among patients, our objective was to devise a clinically useful classification system for GSFs among children, grounded in both clinical presentations and imaging findings, in order to guide clinical diagnosis and treatment decisions. PURPOSE: Depressed ("ping-pong") skull fractures can be treated by different means, including observation, non-surgical treatments, or surgical intervention. The authors describe their experience with vacuum-assisted elevation of ping-pong skull fractures and evaluate variables associated with surgical outcomes.
METHODS: The authors present a retrospective review of all ping-pong skull fractures treated with vacuum-assisted elevation at the Children's Hospital of Orange County in 2021-2022. Variables included patient age, mechanism of injury, fracture depth, bone thickness at the fracture sit…

OBJECTIVE. The purpose of this study was to assess the incidence of pediatric skull fractures contacting cranial sutures in abusive versus accidental trauma. MATERIALS AND METHODS. A retrospective review was conducted of head CT studies performed for pediatric head trauma at a free-standing tertiary care children's hospital from 2012 to 2019. Statistical odds ratios were evaluated to assess the significance of skull fracture extension to sutures in abusive versus accidental injury. A two-proportion Z-test was used to determine the statistical significance of suture type contacted by skull frac…
