Massive soft tissue infections: necrotizing fasciitis and purpura fulminans.

Necrotizing fasciitis and purpura fulminans are two destructive infections that involve both skin and soft tissue. Necrotizing fasciitis is characterized by widespread necrosis of subcutaneous tissue and the fascia. Historically, group A beta-hemolytic streptococcus has been identified as a major cause of this infection. However, this monomicrobial infection is usually associated with some underlying cause, such as diabetes mellitus. During the last two decades, scientists have found that the pathogenesis of necrotizing fasciitis is polymicrobial. The diagnosis of necrotizing fasciitis must be

Skin and soft tissue infections in injection drug users.

Skin and soft tissue infections are the most common cause for hospital admission of injection drug users. Cutaneous and subcutaneous abscesses are the most frequent type of SSTI and occur most commonly when drug users are no longer able to inject intravenously and resort to injection directly into skin or muscle. Abscesses may be difficult to differentiate from uncomplicated cellulitis or may be confused with pseudoaneurysms, hematoma, phlegmon, or thrombosed vein. Special studies, including ultrasonography; CT scans, and MR imaging; or careful incision and inspection may be necessary to clari…

[Diabetic ketoacidosis in patients with soft-tissue necrotizing infections].

Necrotizing soft tissue infection is an uncommon and severe infection of the skin, subcutaneous tissue and superficial fascia which is usually accompanied by severe systemic toxicity. These infections occur more frequently in diabetics and are associated with higher morbidity and mortality rate. The prognosis of necrotizing fasciitis is known to be dependent on early recognition and treatment. Therefore, clinical awareness is important to avoid fatal outcome in patients with diabetes mellffus. We present three patients with undiagnosed type 2 diabetes in whom hyperglycaemia may have facilffate

Soft tissue and muscular infections (including group A streptococci).

Skin, soft tissue and muscular infections are very common and can progress to induce serious life-threatening local and systemic complications. They often occur in sites that have been compromised or injured by foreign bodies, trauma, ischaemia, malignancy or surgery. In addition to group A streptococci and Staphylococcus aureus, the indigenous local microflora is usually responsible for these infections. Anatomical sites that are subject to faecal or oral contamination are particularly at risk. The early recognition and proper medical and surgical management of these infections are of primary

Office laboratory diagnosis of skin and soft tissue infections.

Some skin and soft tissue infections can be treated empirically without culturing. Some infectious sites should be cultured before initiating antimicrobial therapy. When there is a choice, permissive swab specimens are never as good as nonpermissive aspirate specimens. Swab specimens should be cultured semiquantitatively on agar media only in an attempt to distinguish normal flora contaminants from the infecting pathogen. Needle aspirate specimens can be obtained at most infectious sites invoking Sutton's law ("Go where the money is.") to identify the point of maximal involvement. Office labor…

Candida albicans necrotizing soft tissue infection: a case report and literature review of fungal necrotizing soft tissue infections.

BACKGROUND: Necrotizing fasciitis (NF) most often is caused by bacterial infection while a fungal source has been reported in immunosuppressed patients. Early wide surgical resection remains the mainstay of treatment. Split thickness skin grafts are both versatile and easy to harvest, thus making them a long-established option for soft tissue reconstruction. CASE REPORT: A 55-year-old man was admitted for gunshot wounds. Purulence was noted at the central catheter site. Cultures grew Candida albicans. Antifungal therapy was initiated and debridement for NF of the neck and upper chest was perf

Complicated skin and soft tissue infection.

Skin and soft tissue infections (SSTIs) are common, and complicated SSTIs (cSSTIs) are the more extreme end of this clinical spectrum, encompassing a range of clinical presentations such as deep-seated infection, a requirement for surgical intervention, the presence of systemic signs of sepsis, the presence of complicating co-morbidities, accompanying neutropenia, accompanying ischaemia, tissue necrosis, burns and bites. Staphylococcus aureus is the commonest cause of SSTI across all continents, although its epidemiology in terms of causative strains and antibiotic susceptibility can no longer…

Actinomycosis: a rare soft tissue infection.

Actinomycosis is a chronic and suppurative infection caused by an endogenous Gram-positive bacterium. The usual sites of infection are the head and neck, thorax, and abdomen. Primary cutaneous actinomycosis is very rare and usually associated with external trauma and local ischemia. We report on the case of a primary cutaneous actinomycosis of the thigh in a 34-year-old man. The patient was treated successfully with surgical resection and combined antibiotic therapy, and eventually cutaneous reconstructive surgical procedure.

Imaging of musculoskeletal soft tissue infection.

The role of different imaging techniques in the assessment of musculoskeletal soft tissue infection will be reviewed in this article. As classification according to the causative agent is impossible with imaging, soft tissue infections will be classified in view of their location. Consecutively, musculoskeletal soft tissue infections of the joints, tendons and bursae, muscles, the skin and its related structures as well as infectious lymphadenitis and lymphangitis will be discussed.

Acute Chest Wall Infections: Surgical Site Infections, Necrotizing Soft Tissue Infections, and Sternoclavicular Joint Infection.

Acute chest wall infections are uncommon and share similar risk factors for infection at other surgical sites. Smoking cessation has been shown to decrease the risk of surgical site infection. Depending on the depth of infection and/or involvement of the organ space, adequate therapy involves antibiotics and drainage. Early diagnosis and debridement of necrotizing soft tissue infections is essential to reduce mortality. Sternoclavicular joint infections require surgical debridement, en bloc resection, and antibiotic therapy. A standard approach to wound closure after resection has yet to be es…

[Treatment of a necrotizing soft tissue infection].

The surgical procedure for the life-threatening course of a necrotizing soft tissue infection of the leg after minor trauma is described. The necessary consistent resection of extensive fascial and muscular necroses required the reconstruction of soft tissue defects of the knee, the ankle joint and peroneal tendons exposed over a long distance. The functional outcome is presented 1 year after use of MatriDerm® and a split-thickness skin graft for defect coverage.

Soft-tissue infection: necrotizing fasciitis.

Necrotizing fasciitis (NF) is a potentially fatal infection of the skin and soft tissue. The early presentation may first appear as cellulitis. The skin stays intact in the early stages of NF; this leads to a deceptive benign appearance. The skin typically appears red, hot and exquisitely tender. Blisters may or may not be present. Severe local pain, which is out of proportion to the size and type of wound, is a hallmark sign seen in NF. Recognition of the signs and symptoms, as well as timely diagnosis and treatment of this condition is imperative. This is necessary in order to assist in prev

MR imaging of soft-tissue infection.

MR imaging is the ideal technique to display the soft-tissue abnormalities produced by infectious processes. In general these soft-tissue alterations consist of signal intensity changes that reflect the increased water content of the soft tissues induced by the inflammatory reactions. In some cases these changes are nonspecific, but in others, MR imaging can be quite specific and helpful in detecting the presence and extent of the infection, which generally is suspected clinically on the basis of physical and laboratory findings. The MR manifestations of soft-tissue infection, including septic
