Management of developmental speech and language disorders: Part 1.

The identification of developmental problems in a child's acquisition of speech, language and/or communication is a core activity in child surveillance. These are common difficulties with up to 15% of toddlers being 'late talkers' and 7% of children entering school with persisting impairments of their language development. These delays can confer disadvantages in the long term, adversely affecting language, cognition, academic attainment, behaviour and mental health. All children presenting with significant speech and language delay should be investigated with a comprehensive hearing assessmen

Central timing deficits in subtypes of primary speech disorders.

Childhood apraxia of speech (CAS) is a proposed speech disorder subtype that interferes with motor planning and/or programming, affecting prosody in many cases. Pilot data (Peter & Stoel-Gammon, 2005) were consistent with the notion that deficits in timing accuracy in speech and music-related tasks may be associated with CAS. This study replicated and expanded earlier findings. Eleven children with speech disorders and age-and gender-matched controls participated in non-word imitation, clapped rhythm imitation, and paced repetitive tapping tasks. Results suggest a central timing deficit, expre…

Disorders of speech and language: aphasia, apraxia and dysarthria.

PURPOSE OF REVIEW: We review recent important papers pertaining to acquired aphasia, apraxia of speech and dysarthria with special attention to clinically significant work published in the last 12 months. RECENT FINDINGS: The role of the contralateral inferior frontal gyrus in language recovery after stroke is controversial, but is an area of active research, particularly in functional imaging studies. Recent treatment studies in poststroke aphasia have shown that intensity of language therapy may be more important than the method of therapy. Some studies have indicated that amphetamines, pir

The role of pharyngoplasty in congenital neurogenic speech disorders.

Children with dysarthria are often seriously handicapped by their inability to communicate. Velopharyngeal incompetence may contribute to their speech difficulties but the indications for pharyngoplasty are poorly defined. The speech of 41 children, thought to have a defect of neurological origin and referred for possible pharyngoplasty, was assessed by a speech therapist, by nasopharyngoscopy and by multiview videofluoroscopy. These investigations revealed that 16 had velopharyngeal incompetence, with a neurological aetiology, and might be expected to benefit from pharyngoplasty. Independent

[Agnosic attacks with speech disorders and asymmetric EEG anomalies].

The semantic and semiological limits of aphasia and agnosia are imprecise: in the present paper, aphasia is defined as a disorder of the verbal abstract representation of the world of signified, while agnosia is defined as a semantic disorder of sensorial analysis. Three electro-clinical observations of aphasic-agnosic paroxisms with intricated speech disorders and disorders of sensorial identification are reported here. These paroxisms are associated with bilateral and asymetric EEG discharges. The reported observations raise the problem of the function of speech as regards somatic representa…

Facilitating Parental Involvement in Speech Therapy for Children With Speech Sound Disorders: A Survey of Speech-Language Pathologists' Practices, Perspectives, and Strategies.

Purpose This study investigated the extent to which speech-language pathologists (SLPs) facilitate parents' completion of homework activities for children with speech sound disorder (SSD). In addition, this study explored factors related to more consistent communication about homework completion and strategies considered particularly effective for supporting this element of parental involvement. Method Licensed SLPs serving at least one child with SSD were invited to participate in an online survey. Questions relevant to this study gathered information regarding (a) frequency of communication

Procedural Learning, Grammar, and Motor Skills in Children With Childhood Apraxia of Speech, Speech Sound Disorder, and Typically Developing Speech.

Purpose This case-control study sought to determine if (a) children with childhood apraxia of speech (CAS), other speech sound disorders (SSDs), and typical development would perform differently on a procedural learning assessment and (b) whether grammatical ability would impact group differences. Method Communication, motor, and procedural learning abilities were assessed in 48 children with CAS (n = 13), SSD (n = 20), and typical development (n = 15), between 43 and 97 months of age (M = 66 months, SD = 12 months). Results On average, children with CAS demonstrated grammatical and motor impa

Progressive apraxia of speech presenting as isolated disorder of speech articulation and prosody: a case report.

Apraxia of speech (AOS) is a rare disorder of motor speech programming, and few case reports have included sufficient description of both clinical findings and lesion localization. We report a case with an isolated progressive speech articulation deficit and brain involvement restricted to the left superior frontal gyrus. This case suggests that slowly progressive AOS may be a clinical disorder distinct from primary progressive aphasia, and that it can occur without language disorders or bucco-facial apraxia. Our findings highlight the importance of the left hemisphere in speech articulatory d

Impaired speech input and output processing abilities in children with cleft palate speech disorder.

BACKGROUND: Cleft lip and palate is one of the most common oral and maxillofacial deformities associated with a variety of functional disorders. Cleft palate speech disorder (CPSD) occurs the most frequently and manifests a series of characteristic speech features, which are called cleft speech characteristics. Some scholars believe that children with CPSD and poor speech outcomes may also have weaknesses in speech input processing ability, but evidence is still lacking so far. AIMS: (1) To explore whether children with CPSD and speech output disorders also have defects in speech input proces

Differences Between School-Aged Children with Apraxia of Speech and Other Speech Sound Disorders on Multisyllable Repetition.

PURPOSE: To evaluate whether features of childhood apraxia of speech identified in previous literature could be replicated in a sample of school-age children. METHOD: A literature review was conducted to identify candidate speech features that have been previously considered when differentiating childhood apraxia of speech from other types of speech sound disorders. The candidate features recoverable from blinded transcriptions of multisyllable word repetitions (MSWR) were applied to a cohort of 61 children, aged 7-17, previously classified as having childhood apraxia of speech (n=21) or non-

[Tongue function in the rehabilitation of speech disorders, faulty breathing habits and deglutition disorders].

The tongue is as an organ of speech and articulation not as indispensable as the soft palate. The recovery of speech after extirpation of the tongue is usually satisfactory, and the intelligibility of patients with microglossia compares favourably with the speech of an untreated cleft palate patient. Hypernasality and nasal escape of air are typical also for the congenital short palate (in the absence of a cleft); syndromes with a congenital short palate are often misdiagnosed as suprabulbar paralysis. The function and involuntary usage of the tongue e.g. in swallowing, influence the growth an
