Total lateral sphincterotomy for anal fissure.

BACKGROUND AND AIMS: Initial experience with the posterior sphincterotomy for treating anal fissures was unsatisfactory, with a significant rate of recurrences and anal incontinence. This report describes the lateral approach to complete section of the internal sphincter. PATIENTS AND METHODS: Between 1997 and 2001 we surgically treated 164 patients for anal fissure. Preoperative and postoperative anal manometries were recorded. Postoperative course and early and long-term results were recorded. RESULTS: No fissure failed to heal. Early complications included bleeding, hematoma, and pain. A …

Endoscopic pancreatic sphincterotomy: techniques and complications.

Endoscopic pancreatic sphincterotomy of the major and minor papilla has expanded our approach to the management of a variety of pancreatic disorders. Analysis of the complication rates of this therapy is difficult, however, because a variety of techniques are often used in conjunction with the pancreatic sphincterotomy. This article reviews the techniques and complications of endoscopic pancreatic sphincterotomy. Based on the currently available data, it appears that the complication rates of pancreatic sphincterotomy are probably higher than those of biliary sphincterotomy. Should application…

Reappraisal of partial lateral internal sphincterotomy.

PURPOSE: The aim of this study was to delineate the results, mortality, and morbidity of partial lateral internal sphincterotomy for the treatment of chronic anal fissure. METHOD: A retrospective review of 500 patients undergoing partial lateral internal sphincterotomy for chronic anal fissure between 1980 and 1990 was performed. Patients were identified by a review of an office surgical ledger and included all patients whose diagnosis was anal fissure and for whom a partial lateral internal sphincterotomy was performed as treatment. RESULTS: Over an average follow-up of 5.6 years, only 1 pe…

[Delayed stenosis after endoscopic sphincterotomy (EST)?].

To answer the question of restenosis following endoscopic sphincterotomy (EST) different and sophisticated methods were performed. Using the Erlangen type papillotome we developed a practicable and standardised method for measurement of the size of the EST that is able to define a restenosis. Depending on the indication for EST we found different percentages of restenosis: common bile-duct stones 14%, papillary stenosis 23.1%, duodenal diverticula without bile duct stones as cholestatic entity 40%. On one hand reduction of EST size is not corresponding to clinical symptoms, on the other hand t…

Sphincterotomy failure in neurogenic bladder disease.

Among 60 spinal cord injury patients who underwent external urethral sphincterotomy 45 experienced success and 15 failed. Failure was established when symptomatic urinary tract infections and high vesical residuals persisted. Urodynamic findings demonstrated detrusor areflexia in 10 patients (66 per cent), detrusor-sphincter dyssynergia in 2 (13.2 per cent), detrusor hyperreflexia with unsustained bladder contractions in 1 (6.6 per cent), and detrusor hyperreflexia and bladder neck obstruction in 2 (13.2 per cent). Among these failures poor detrusor contractility predominated. Detrusor-sphinct…

[Long term results after endoscopic sphincterotomy].

A multicenter study of 25 centers covering 9041 endoscopic sphincterotomies showed that choledochal concrements still represent the main indication (83,9%). Circumscript papillary stenosis (10.64%) and papillary tumour(2.06%) are the next most frequent indications. Complications after endoscopic sphincterotomy may be expected in 7.55% of cases, somewhat more frequent after papillary stenoses than after choledochal concrements. The most frequent complication is haemorrhage, followed by pancreatitis, cholangitis in cases of choledochal concrements, and perforation. Mortality is around 1.12%. Lat…

[Endoscopic sphincterotomy in choledocholithiasis (author's transl)].

43 patients had 51 endoscopic sphincterotomies within 4 years; there were 2 complications, 7 patients had to have surgery in spite of endoscopic sphincterotomy. Stones, which had been left unnoticed in the choledochus during surgery could be removed in all cases endoscopically. These results speak in favour of endoscopic sphincterotomy, if compared to results of other non-operative treatments of retained bile duct stones such as percutaneous stone extraction. Endoscopic sphincterotomy has been widely used meanwhile and may be apt now to be introduced as a routine procedure also in smaller hosp…

Endoscopic pancreatic sphincterotomy: indications and complications.

BACKGROUND/AIMS: Although a few recent studies have reported the effectiveness of endoscopic pancreatic sphincterotomy (EPST), none has compared physicians' skills and complications resulting from the procedure. Thus, we examined the indications, complications, and safety of EPST performed by a single physician at a single center. METHODS: Among 2,313 patients who underwent endoscopic retrograde cholangiopancreatography between January 1996 and March 2008, 46 patients who underwent EPST were included in this retrospective study. We examined the indications, complications, safety, and effectiv…

Tailored lateral sphincterotomy for anal fissure.

PURPOSE: Most surgical texts describe the length of division of the internal sphincter during closed lateral sphincterotomy as "to just above the dentate line," resulting in significant rates of incontinence. This study reviews our experience using a "tailored" lateral sphincterotomy by selecting the height of sphincter to be divided with the aim of preserving more sphincter. METHODS: From 1976 to March 1996, the files of 440 patients who had sphincterotomies were reviewed by an independent research assistant. After exclusions, a residual group of 352 patients had undergone tailored left late…
