Role of endoscopic sphincterotomy of the minor papilla in pancreas divisum.

BACKGROUND: Pancreas divisum (PD) is a common anatomical variant of the pancreatic duct system. Only a little percentage of patients with this altered anatomy develop symptoms: acute recurrent pancreatitis (ARP), chronic pancreatitis (CP) and pancreatic-type pain alone. These have been supposed to arise from an obstruction to outflow of the pancreatic dorsal duct due to a stenosis of the minor papilla. Endoscopic sphincterotomy of the minor papilla (MiES) can be considered an effective treatment for patients with PD and ARP supported by stenosis or obstruction of the minor papilla. On the othe

Indications and results for transduodenal sphincteroplasty in the era of endoscopic sphincterotomy.

BACKGROUND: Transduodenal sphincterotomy (TS) has fallen into disuse since endoscopists developed techniques to treat sphincter problems nonsurgically. However, some patients experience recurrent sphincter strictures after endoscopic sphincterotomy (ES), with the ampulla endoscopically inaccessible, and pancreas divisum (PD); these patients are referred to a surgeon because they are unsuitable for ES. METHODS: The medical records of patients who underwent TS at the First Department of Surgery of the Medical School, University of Rome "La Sapienza," between January 1997 and December 2005 were

Endoscopic sphincterotomy in the management of bile duct stones in children.

OBJECTIVE: Endoscopic sphincterotomy (ES) is a widely accepted method of extracting bile duct stones (BDS) in young as well as in elderly patients. The present study was undertaken to assess the safety and efficacy of ES for the treatment of BDS in children, seven of whom were critically sick because of suppurative cholangitis or pancreatitis. METHOD: Over a period of 33 months, ES was performed in 16 consecutive children aged 7-16 yr with BDS. Nine patients had gallbladder in situ, and seven had previously undergone cholecystectomy. The coexisting abnormalities were gallstones and hepatic du

Endoscopic sphincterotomy and laparoscopic cholecystectomy in an infant with cholecysto-choledocholithiasis.

Laparoscopic cholecystectomy (LC) and endoscopic sphincterotomy (EST) are widely accepted procedures for cholecysto-choledocholithiasis in adults. However, their use in infants has not been reported. An 8-month-old girl presented with high fever and obstructive jaundice. Ultrasound scan showed acute cholecystitis with stones in the bile duct. After 2-week-long antibiotic therapy the acute cholecystitis and hepatic impairment resolved. An endoscopic retrograde cholangiopancreatography (ERCP) confirmed choledocholithiasis and cholecystolithiasis. Risk factors for the development of biliary calcu

The efficacy of endoscopic sphincterotomy in patients with sphincter of Oddi dysfunction.

BACKGROUND: Dysfunction of the sphincter of Oddi (SOD) is one of the causes of postcholecystectomy syndrome and biliary pain. Endoscopic sphincterotomy (EST) is recommended in some patients refractory to conservative treatment. This study was designed to evaluate the efficacy of, and complications from EST in patients with SOD. METHODS: Between 1990 and 1993, 24 patients clinically diagnosed as having papillary stenosis were treated by endoscopic sphincterotomy. All patients had dilated common bile duct (CBD) in addition to 1) elevation of transaminases and alkaline phosphatase (n = 13); 2) e

Endoscopic sphincterotomy for suspected choledocholithiasis in patients with and without stones.

OBJECTIVE: To characterize a subgroup of patients treated with endoscopic sphincterotomy (ES) for a suspected common bile duct lithiasis (CBDL) that was not confirmed and to compare it with the subgroup with confirmed CBDL. METHODS AND RESULTS: Over 18 months, ES was successful in 245 consecutive patients (age 23-97, mean 75 yr, SE 17 yr) with suspected CBDL; 159 patients had CBDL (group 1), and 86 did not (group 2), as confirmed by CBD exploration. Fifty-nine percent of the patients in group 1 and 76.7% of the patients in group 2 had gallbladder in situ. Both groups were different for age (p

[Endoscopic sphincteroplasty (not sphincterotomy) using the clips--a preliminary report].

Reports exist that incomplete division of the papilla by endoscopic sphincterotomy may lead to restenosis, cholangitis, and stone recurrence. In view of evolution of surgical sphincterotomy to sphincteroplasty, we studied feasibility of endoscopic sphincteroplasty. In 36 dogs, at laparatomy, the papilla was divided using a diathermy sphincterotome to create retroperitoneal perforation. Twelve dogs underwent approximation of the bile duct and duodenal walls with silk sutures. Another 12 dogs had the walls approximated with clips using an endoscopic clip appliance. The other 12 dogs had no appro

[Late results of endoscopic sphincterotomy in patients with intact gallbladder].

Late complications and the recurrence rate of gall-bladder stones were registered by means of annual follow-up examinations over a maximum period of five and a half years in 91 out of 116 patients in whom endoscopic sphincterotomy (EST) with the gall-bladder in situ was performed. It turned out that only one patient had colic after EST in the group of patients with patent ductus cysticus (n = 36). It was not necessary to cholecystectomise any patients. In contrast, there were 13 cases of patients with incompetent ductus cysticus (n = 55) having a colic or cholecystitis and 5 cases where the pa

Surgical management of complications of endoscopic sphincterotomy with precut papillotomy.

We reviewed 574 endoscopic sphincterotomy procedures. Fifty-six precut papillotomies were performed. Presenting conditions included choledocholithiasis, cholangitis, benign and malignant papillary strictures, and stenosing papillitis. Complications were identified in 16 percent: perforation in 9 percent, pancreatitis in 5 percent, bleeding in 2 percent, and pancreatic abscess in 2 percent. One patient died. Six patients required operation for complications. Perforation of the duodenum or common bile duct seen within 8 hours was managed with drainage and closure of the perforation with minimal

Fate of the gallbladder with cholelithiasis after endoscopic sphincterotomy for choledocholithiasis.

Endoscopic sphincterotomy is the treatment of choice for choledocholithiasis after cholecystectomy. Its role has been expanded to treat choledocholithiasis in patients with gallbladders still in place. The authors report their experience with endoscopic sphincterotomy, with emphasis on the safety of the procedure, in high-operative-risk patients with choledocholithiasis and gallbladder in situ. Stones were successfully removed in 72 of 75 patients (96%); 1 required an emergency operation and 2 an elective one. Complications included bleeding, pancreatitis and cholangitis; there were no associa

Endoscopic sphincterotomy and surgical approaches in the treatment of the 'sump syndrome'.

The 'sump syndrome' is a rare, late complication of choledochoenterostomies. We observed 13 patients with this syndrome. The clinical features included recurrent cholangitis, liver abscesses, and recurrent pancreatitis. Five patients were treated by surgery, eight patients endoscopically. Reviewing an average follow up period of 22 months (range 6-60) after therapeutic endoscopy and 57 months (range 1-126) after different surgical procedures, we conclude that endoscopic sphincterotomy with removal of the debris is as efficient a therapy of the 'sump syndrome' as surgical sphincteroplasty and e

Endoscopic sphincterotomy of the papilla of Vater: an analysis of 300 cases.

Endoscopic sphincterotomy was performed on 300 patients with biliary and/or pancreatic disease during the period 1978-1983. The most frequent indications were choledocholithiasis after cholecystectomy (59%), choledocholithiasis without cholecystectomy (17%) and presumed motility disorders of the sphincter of Oddi (15%). In choledocholithiasis, stones passed spontaneously or were extracted from the bile duct in 147 of 164 patients (90%) in whom the outcome was determined by cholangiography immediately after stone extraction or by a second retrograde cholangiogram. In presumed motility disorders

Effect of endoscopic sphincterotomy and endoscopic papillary balloon dilation endoscopic retrograde cholangiopancreatographies on the sphincter of Oddi.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP), with its clinical ad-vantages of less trauma and faster recovery, has become the primary treatment for choledocholithiasis. AIM: To investigate the effects of different ERCP procedures on the sphincter of Oddi. METHODS: The clinical data of 91 patients who underwent ERCP at Yixing Hospital of Traditional Chinese Medicine between February 2018 and February 2021 were analyzed retrospectively. The patients were divided into endoscopic sphincterotomy (EST, n = 24) and endoscopic papillary balloon dilation (EPBD, n = 67) groups. T

Association between endoscopic sphincterotomy and the development of hepatopancreatobiliary cancer.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (ES) has been hypothesized to increase the risk of hepatopancreatobiliary (HPB) cancer, but prior studies have shown conflicting results. This study aimed to determine whether ERCP with ES is associated with the longitudinal development of HPB cancer. METHODS: This matched cohort study conducted from 2010 to 2022 used linked medical record data from clinics, hospitals, and other facilities in Southeast Minnesota and Southwest Wisconsin. A total of 188,243 patients were included: 4003 with cholelit

Endoscopic sphincterotomy for adults with biliary sphincter of Oddi dysfunction.

BACKGROUND: The sphincter of Oddi comprises a muscular complex encircling the distal part of the common bile duct and the pancreatic duct regulating the outflow from these ducts. Sphincter of Oddi dysfunction refers to the abnormal opening and closing of the muscular valve, which impairs the circulation of bile and pancreatic juices. OBJECTIVES: To evaluate the benefits and harms of any type of endoscopic sphincterotomy compared with a placebo drug, sham operation, or any pharmaceutical treatment, administered orally or endoscopically, alone or in combination, or a different type of endoscopi

Management of gallstone pancreatitis: cholecystectomy or ERCP and endoscopic sphincterotomy.

BACKGROUND: Currently, cholecystectomy is recommended for patients with gallstone-induced pancreatitis. ERCP with endoscopic sphincterotomy (ES) within 24 to 48 hours is also suggested for the treatment of acute gallstone pancreatitis. The aim of this study was to determine outcome after cholecystectomy versus ES alone in patients with gallstone pancreatitis. METHODS: One hundred seventeen patients with gallstone pancreatitis were included in this prospective observational study. Inclusion criteria were typical abdominal pain; serum amylase level 3 times or greater than normal; and gallbladde
