The case is reported of a 15-year-old girl who presented following trauma and was found to have a massive congenital diaphragmatic hernia through the foramen of Bochdalek. The spleen, which was intrathoracic, had ruptured. No other similar cases have been reported in the literature hitherto.

In the two cases presented here, the sonographic findings were unique. At low gain settings the rounded masses appeared sonolucent and smooth walled. As the gain was increased, multiple internal echoes appeared close to the inner wall, indicating the thick and irregular wall lining. It is presumed that these echoes were produced by the trabeculae. It is felt that the sonographic findings in epidermoid cysts of the spleen may be diagnostic, obviating the need for arteriography.

Wandering spleen may result from congenital failure of splenic attachments or acquired laxity of attachments due to stretching. The wandering spleen is quite vulnerable to the complication of torsion, which is seen most often as an acute abdominal emergency due to splenic infarction. A more unusual variant is chronic torsion which causes intermittent mild to moderate abdominal pain, gastrointestinal disturbances and, in one case, hypersplenism.
