INTRODUCTION: Spondylolysis is a common occurrence for adolescent athletes who have low back pain. The injury involves a defect in the pars interarticularis, occurring as a result of repeated hyperextension and rotation.
CLINICAL PRESENTATION: Clinical findings might include tightness of the hip flexors and hamstrings, weakness of the abdominals and gluteals, and an excessive lordotic posture. The validity of several clinical tests were compared alongside magnetic resonance imaging, but were not able to distinguish spondylolysis from other causes of low back pain. Medical referral should be a…

OBJECTIVE: To determine the incidence of spondylolysis and spondylolisthesis among female gymnasts, and to correlate these with lumbar pain, history of trauma and training load.
METHOD: Eighteen competitive-level Olympic-style gymnasts aged 8-17 years, with an average age of 11.3 years, were evaluated on a voluntary basis. The gymnasts answered a questionnaire about their sports activities and underwent a clinical examination and lumbar spine radiography.
RESULTS: The radiographs were analyzed by orthopedists specializing in surgery of the vertebral column. The prevalence of spondylolysis wa…

Low back pain is a common cause of lost playing time in young athletes, and spondylolysis is its most common identifiable cause. Despite technological advances in radiology, which can lead to an early diagnosis with better prognosis, progression to spondylolisthesis is sometimes asymptomatic and may not be detected until late stages. There are wide variations, suggesting lack of consensus as regards the objective of treatment, which consists of clinical, radiological, biomechanical or functional improvement. There is also a lack of agreement regarding the ideal conservative treatment, surgical…

Low back pain, specifically "spondylo" conditions, has traditionally been misunderstood and often times ill-treated. A thorough understandin … of this condition, will help the allied healthcare provider better meet the individualized needs of spondylolysis patients.

STUDY DESIGN: We propose that chronic spondylolytic defects at L5 are influenced by insufficient differential mediolateral distances between … throughout the lumbar columns of normal and spondylolytic individuals.
SUMMARY OF BACKGROUND DATA: Spondylolysis is a common condition, yet its etiology is poorly understood. It is generally considered to be the result of a vaguely defined fatigue fracture through the pars interarticularis. The cause(s) of spondylolysis, however, have not been clearly identifi …

Cervical spondylolysis is a rare defect of unknown etiology. Five cases of cervical spondylolysis as well as two cases of fractures of the pedicles of C2 in infants are presented. Comparison of the cases suggests that a fracture at birth or in infancy may be the cause of some cases of cervical spondylolysis.

Lumbar spondylolysis represents a stress fracture of the pars interarticularis and occurs most commonly at the L5 level. Pars defects can be imaged with plain radiography, bone scintigraphy, computed tomography (CT) and magnetic resonance imaging (MRI). Plain radiographic projections of particular value include the coned lateral view of the lumbosacral junction, which displays the majority of defects, and the anteroposterior view with 30 degrees cranial angulation. The value of oblique radiography is unproven. Planar bone scintigraphy (PBS) is more sensitive than radiography and single photon …

We investigated 185 adolescents under the age of 19 years with spondylolysis. All but five were active in sport. The pars defect was classified into early, progressive and terminal stages. Of the 346 pars defects in 185 patients, 39.6% were early, 29.5% progressive and 30.9% in the terminal stages. Conservative management produced healing in 73.0% of the early, 38.5% of the progressive and none of the terminal defects. These results suggest that spondylolysis is caused by repetitive microtrauma during growth and can be successfully treated conservatively if treatment is started in the early st…

The incidence of spondylolysis is approximately 5-6% in the white population. Males and females are equally affected. In about one third of the cases spondylolysis is associated with spondylolisthesis. For the etiology mechanical and genetic factors are important. At the L4 level mechanical factors play the dominant role, while at the L5 level genetic factors are equally important (size and orientation of the facet joints, dysplasia of the vertebral arch). Increased risk for low back pain is found in the presence of the following factors: spondylolisthesis exceeding 10 mm, a low lumbar index, …

STUDY DESIGN: This retrospective study compared patient outcome after two surgical techniques for pars interarticularis repair: 1) buck screw fixation and 2) the modified Morscher-designed spondylolysis distraction hook.
OBJECTIVES: To determine whether one technique was associated with higher radiographic, clinical, or implant failure.
SUMMARY OF BACKGROUND DATA: Both techniques have been reported as giving good clinical results.
METHODS: Twenty patients were included in this study. All patients had Type IIA spondylolytic defects with either a Grade 0 or Grade I spondylolisthesis. Nine pat…

Spondylolysis usually occurs in the fifth lumbar vertebra, which with the rest of the lumbosacral junction may be hypoplastic. About 10% of spondylolysis occurs in the fourth lumbar vertebra which is usually normal radiologically. The present work studies the hitherto unknown prognostic value of the lysis level in relation to symptoms. A comparison was made between 213 patients with lysis at L5 and 42 patients with lysis at L4. The frequency and intensity of low back pain, the need for treatment, change of work, sickleave, and sick pension caused by low-back symptoms were noted during a period…

A possible interference of various musculo-skeletal anomalies with pregnancy is often discussed by obstetricians. Pregnancy as a risk factor for progression of spondylolisthesis, olisthesis as a risk factor for pregnancy complications, and pregnancy in women with spondylolysis as the cause of increased low-back symptoms, are questions hitherto not analysed. In the present study, a comparison between men, non-pregnant women, and women who had been pregnant, was made with respect to the degree of spondylolisthesis and its subsequent progression over an observation period of at least 20 years. Oc…

We performed a prospective roentgenographic study to determine the incidence of spondylolysis, spondylolisthesis, or both, in 500 unselected first-grade children from 1955 through 1957. The families of the children with spondylolysis were followed in a similar manner. The incidence of spondylolysis at the age of six years was 4.4 per cent and increased to 6 per cent in adulthood. The degree of spondylolisthesis was as much as 28 per cent, and progression of the olisthesis was unusual. The data support the hypothesis that the spondylolytic defect is the result of a defect in the cartilaginous a…
