Evidence-based treatment and stuttering--historical perspective.

PURPOSE: To illustrate the way in which both fluency shaping (FS) and stuttering management (SM) treatments for developmental stuttering in adults are evidence based. METHOD: A brief review of the history and development of FS and SM is provided. It illustrates that both can be justified as evidence-based treatments, each treatment seeking evidence of a different kind: FS seeks evidence concerning treatment outcome, and SM seeks evidence concerning the nature of the stutter event. CONCLUSION: Although outcome evidence provides the principal support for FS, support for SM comes principally fr…

Disfluency Characteristics of 4- and 5-Year-Old Children Who Stutter and Their Relationship to Stuttering Persistence and Recovery.

Purpose The purpose of this study is to document disfluency behaviors expressed by 4- and 5-year-old children who stutter and to identify whether stuttering characteristics at this age are predictive of later stuttering recovery or persistence. Method We analyzed spontaneous speech samples from 47 children diagnosed with developmental stuttering when they were 4-5 years old. Based on their eventual diagnosis made the final year of participation in the longitudinal study when the children were 6-9 years old, the children were divided into two groups: children who eventually recovered from stutt…

Self-disclosure of stuttering: A systematic review.

PURPOSE: Self-disclosure of stuttering occurs when people who stutter (PWS) actively acknowledge to others that they stutter. Although the benefits of self-disclosure have been examined for three decades, several factors have been identified that may mediate its impact. The purpose of this study was to (a) conduct a systematic review of the literature on self-disclosure of stuttering, and (b) summarize the effects of self-disclosure to examine potential factors influencing its impact. METHOD: A systematic review of studies examining self-disclosure of stuttering was conducted. Studies publish…

Developmental stuttering with common and complex phenotypes.

AIM: To describe the phenotypic spectrum associated with stuttering. METHOD: Individuals with current or resolved developmental stuttering self-referred. Surveys assessed stuttering characteristics (onset, negative impact, family history) and health (early development, other conditions). Speech and non-verbal intelligence were assessed using conversation and the Wechsler standardized scales. Sample sizes varied across assessments (n = 266-327). Latent class analysis identified unobserved groups based on assessment data. RESULTS: A total of 327 participants (231 male, 71%) with a median age o…

Stuttering after Intravenous Anesthesia: A Case Report.

Stuttering is a form of speech disorder characterized by involuntary prolongation and repetition of sound, words, syllables or phrases as well as involuntary silent pauses or blocks. We report a case of a healthy twenty-six-year-old male patient without significant past history, who underwent short intravenous anesthesia for incision and drainage for perianal abscess. Postoperatively, the patient presented with prominent stuttering after six hours of surgery. To our knowledge, this is the first reported case of stuttering following short intravenous anesthesia without any airway manipulation. …

Altered functional connectivity in persistent developmental stuttering.

Persistent developmental stuttering (PDS) is a speech disorder that impairs communication skills. Despite extensive research, the core causes of PDS are elusive. Converging evidence from task-induced neuroimaging methods has demonstrated the contributions of the basal ganglia and the cerebellum to PDS, but such task-state neuroimaging findings are often confounded by behavioral performance differences between subjects who stutter and normal controls. Here, using resting-state functional magnetic resonance imaging, we investigated functional connectivity within cerebellar-cortical and basal gan…

Stuttering priapism: insights into pathogenesis and management.

Priapism is defined as a persistent, painful erection that continues beyond, or is unrelated to, sexual stimulation. It may be categorized as either ischemic (low/absent flow) or nonischemic (high flow). Stuttering priapism is a variant of the ischemic type that is characterized by repetitive, transient, painful, self-limiting episodes of priapism. It is associated with various hematological disorders, including sickle cell disease and pharmacological treatments. The consequences of ineffective treatment of priapism are erectile dysfunction and impaired quality of life due to chronic pain and …

[Stuttering: effects of genes and early treatment].

As stuttering by most persons resolves after a time, an initial wait-and-see policy has been maintained up to now. Two recent important developments in the area of stuttering have occurred: the discovery of gene mutations that appear to be of relevance to the developmental-neurological abnormality and growing evidence that early intervention helps recovery. The mutated genes found are GNPTAB, GNPTG and NAGPA. They are involved in lysosomal decomposition. Published study results show that early treatment using either the Demands and Capacities Model (indirect treatment aimed at the child's surr…

Normal interhemispheric inhibition in persistent developmental stuttering.

Imaging studies suggest a right hemispheric (pre)motor overactivity in patients with persistent developmental stuttering (PDS). The interhemispheric inhibition (IHI) studied with transcranial magnetic stimulation is an established measure of the interplay between right and left motor areas. We assessed IHI in 15 young male adults with PDS and 15 age-matched fluent-speaking subjects. We additionally studied the ipsilateral silent period (iSP) duration. We found no significant between-group difference for IHI or for iSP duration. We conclude that the interplay between the primary motor cortices …

Jaw-phonatory coordination in chronic developmental stuttering.

UNLABELLED: A deficiency in sensorimotor integration in a person who stutters may be a factor in the pathophysiology of developmental stuttering. To test oral sensorimotor function in adults who stutter, we used a task that requires the coordination of a jaw-opening movement with phonation onset. The task was adapted from previous limb coordination studies, which show that movement coordination depends on intact proprioception. We hypothesized that adult stutterers would show deficient jaw-phonatory coordination relative to control participants. The task required initiation of phonation as a j…
