Cognitive dysfunction associated with falls in progressive supranuclear palsy.

BACKGROUND: Attentional and executive dysfunctions are associated with falls in community-dwelling elderly individuals and patients with PD. Frontal cognitive dysfunction and falls are frequent symptoms of PSP. We studied to identify the cognitive domains associated with recurrent falls in patients with PSP. METHODS: We performed a battery of neuropsychological tests in 59 individuals with probable PSP. We categorized patients into infrequent fall (≤one fall during the last 12 months, n=29) or recurrent fall (≥two falls during the last 12 months, n=30) groups. RESULTS: UPDRS subscores for ax

Hypodipsia discriminates progressive supranuclear palsy from other parkinsonian syndromes.

BACKGROUND: The objective of this study was to evaluate whether the sensation of thirst differs between patients with progressive supranuclear palsy (PSP), multiple system atrophy with predominant parkinsonism (MSA-P), and Parkinson's disease (PD). METHODS: We administered a standardized thirst questionnaire to age-, sex-, and stage-matched patients with probable PSP, PD, and MSA-P and healthy controls (HC), n = 15/group. In an independent cohort (n = 10/group), we provoked thirst by infusing hypertonic NaCl in age-, sex-, and stage-matched patients with PSP, PD, and MSA-P and recorded plasma

New and reliable MRI diagnosis for progressive supranuclear palsy.

OBJECTIVE: To evaluate the area of the midbrain and pons on mid-sagittal MRI in patients with progressive supranuclear palsy (PSP), Parkinson disease (PD), and multiple-system atrophy of the Parkinson type (MSA-P), compare these appearances and values with those of normal control subjects, and establish diagnostic MRI criteria for the diagnosis of PSP. METHODS: The authors prospectively studied MRI of 21 patients with PSP, 23 patients with PD, 25 patients with MSA-P, and 31 age-matched normal control subjects. The areas of the midbrain tegmentum and the pons were measured on mid-sagittal MRI

Limb apraxia and cognitive impairment in progressive supranuclear palsy.

Limb apraxia occurs in progressive supranuclear palsy (PSP); however, its characteristics and relation to dementia are unclear. We compared 25 probable PSP patients on cognitive and ideomotor apraxia tests with 19 healthy controls. Eleven PSP patients were demented (D-PSP) and 14 not (ND-PSP). D-PSP patients did worse than controls and ND-PSP patients in all cognitive tests; and ND-PSP patients did worse than controls in all except the memory test. In the apraxia test, PSP patients committed mainly sequence errors and executed complex gestures badly. Apraxia correlated strongly with dementia.

Enlargements of somatosensory-evoked potentials in progressive supranuclear palsy.

OBJECTIVES: The outcome of Bipolar 1 Disorder (BD1) is greatly dependent on the adequacy and appropriateness of its treatment. As the treatment choices in BD1 disorder are increasing the aim of this study was to examine the current use of the pharmacological agents in BD1 patients and its association with clinical characteristics. METHODS: Case note review of the pharmacotherapy of a sample of BD1 patients from a specified South London sector of a National Health Service Trust. RESULTS: Half of the patients were on mood-stabilizers (usually lithium) and their use was associated with female g

Neuropathologic differentiation of progressive supranuclear palsy and corticobasal degeneration.

Progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD) are usually sporadic multi-system degenerations associated with filamentous tau inclusions in neurons and glia. As such they can be considered sporadic tauopathies in contrast to familial tauopathies linked to mutations in the tau gene. Mutations have not been found in the tau gene in either PSP or CBD. The clinical syndromes and neuroimaging of typical cases of PSP and CBD are distinct; however, atypical cases are described that have overlapping clinical and pathologic features. Both PSP and CBD have similar biochemical

Achromatic neurons in the cortex of progressive supranuclear palsy.

Achromatic neurons (AN) in the cerebral cortex are a characteristic feature of several neurodegenerative conditions including Pick's disease, corticobasal degeneration and some cases of primary progressive aphasia. Although AN are occasionally seen in some other diseases, their presence in progressive supranuclear palsy (PSP) has not been previously documented. We found significant numbers of AN in the cerebral cortex of five out of seven cases which had been diagnosed pathologically as PSP. The identification of AN was greatly facilitated by the use of neurofilament immunohistochemistry. The

Cholinergic approaches to the treatment of progressive supranuclear palsy.

In spite of the severe loss of cholinergic neurons in the brains of patients with progressive supranuclear palsy (PSP), marginal or null benefits are seen in clinical trials after the administration of physostigmine, a cholinesterase inhibitor, or RS-86, a cholinergic agonist. The possible role of cholinergic therapy in PSP is reevaluated.

Aphasia in Progressive Supranuclear Palsy: As Severe as Progressive Non-Fluent Aphasia.

BACKGROUND: Adynamic speech is characteristic of progressive supranuclear palsy (PSP), but higher language deficits have been reported inconsistently, in the context of clinical and pathological overlaps with progressive non-fluent aphasia (PNFA). OBJECTIVE: The present study tested two hypotheses: 1) PSP and PNFA display impaired single word repetition, object naming, semantic knowledge, and syntactic comprehension; and 2) PSP have reduced speed on timed cognitive tasks. METHODS: Structured clinical and neuropsychological assessments of language were performed on patients with clinically de

Clinical Correlation Between Vertical Gaze Palsy and Midbrain Volume in Progressive Supranuclear Palsy.

BACKGROUND: Supranuclear vertical gaze palsies and slowed vertical saccades are characteristic clinic features of progressive supranuclear palsy (PSP). The "hummingbird sign," reflective of midbrain atrophy, is a classic radiographic sign of PSP. Correlation between eye movement abnormalities and radiographic findings in PSP has been reported previously. However, due to the use of clinical criteria not commonly employed in neuro-ophthalmic practice and neuroimaging techniques that are not widely available, it remains unclear whether correlation between midbrain structure and characteristic ocu

Argyrophilic Grain Disease Clinically Presenting as Progressive Supranuclear Palsy with Progressive Gait Freezing.

BACKGROUND: Argyrophilic grain disease (AGD) is a four-repeat tauopathy characterized by the accumulation of argyrophilic grains. Its primary clinical manifestation is late-onset amnestic dementia. While the extension of argyrophilic grains to the substantia nigra may be related to its potential association with parkinsonism, biochemical analyses are lacking. OBJECTIVES: To elucidate the extent of AGD to the midbrain, including the substantia nigra, using histopathological examination and biochemical analysis in a pathologically proven case of AGD presenting with parkinsonism and cognitive im

Procerus sign as a diagnostic clue for progressive supranuclear palsy.

The procerus sign is associated with some diseases that initially manifest as atypical parkinsonism, especially progressive supranuclear palsy (PSP). It is described as a vertical contraction of the muscles in the glabellar region. This sign helps to clinically differentiate atypical parkinsonism diseases, especially in the early stages, when many conditions are similar, in terms of clinical presentation and poor response to levodopa. Thus, the procerus sign can be an important clue in the suspicion of PSP.

Striatal Tau Pathology Underlies Monoaminergic Disruption in Progressive Supranuclear Palsy.

PURPOSE: Progressive supranuclear palsy (PSP) is a tauopathy marked by widespread tau aggregation and prominent monoaminergic dysfunction, yet the relationship between regional tau burden and monoaminergic impairment remains unclear. In this study, we determine whether regional tau burden is associated with nigrostriatal monoaminergic dysfunction in PSP using PET imaging. PATIENTS AND METHODS: A subset of 20 patients with PSP and monoaminergic impairment, identified by 18 F-FP-DTBZ PET, was recruited and subsequently underwent 18 F-APN-1607 PET to assess tau deposition. All images were spatia

Multifactorial etiology of progressive supranuclear palsy (PSP): the genetic component.

Progressive supranuclear palsy (PSP) is mainly a sporadic disease. It has a multifactorial etiology and an interaction between environmental and genetic factors causes disease. While elucidation of environmental risks for PSP is still in its infancy, much has been learned about the genetic etiological component of PSP during the past few years. This article reviews genes that convey risk for PSP. All genes have been identified in association studies. Only those genes with the standard threshold for genome-wide significance of P < 5E-8 are covered. These genes include MAPT, KANSL1, PLEKHM1, STX

Paraneoplastic progressive Supranuclear palsy: a case report and literature review.

Progressive supranuclear palsy-Richardson syndrome (PSP-RS) is a sporadic atypical parkinsonian disorder typically resistant to levodopa. While most cases are neurodegenerative, various conditions (genetic, vascular, infectious, or paraneoplastic) can mimic this phenotype, termed PSP-like or PSP mimics. Although paraneoplastic PSP is extremely rare, it is important to recognize because of its potential reversibility. The diagnosis can be particularly challenging, especially when classical tumor markers or imaging fail to identify a clear neoplastic origin. Here, we describe a 39-year-old man w

A case report of palatal tremor in progressive supranuclear palsy.

INTRODUCTION: Progressive supranuclear palsy (PSP) is a sporadic, adult-onset neurodegenerative disease and the second most common form of neurodegenerative parkinsonism after idiopathic Parkinson's disease. Palatal tremor (PT), characterized by brief, involuntary, rhythmic contractions of the soft palate, is a rare movement disorder and an even more exceptional finding in PSP. CASE REPORT: We present an 85-year-old, Filipino male who manifested with progressive gait imbalance, postural instability leading to multiple falls, ocular motor dysfunction, axial rigidity, bradykinesia, and cognitiv

Alzheimer's Disease-Related Cerebrospinal Fluid Biomarkers in Progressive Supranuclear Palsy.

Progressive Supranuclear Palsy (PSP) is the most common four-repeat tauopathy. PSP cases are typically characterized by vertical gaze palsy and postural instability; however, various phenotypes have been reported, making antemortem diagnosis based on clinical symptoms challenging. The development of biomarkers reflecting brain pathology and the ability to diagnose patients based on these biomarkers are essential for developing future intervention strategies, including disease-modifying therapies. However, despite many dedicated efforts, no highly specific fluid biomarker for PSP has yet been e
