Functional and radiographic evaluation of an interspinous device as an adjunct for lumbar interbody fusion procedures.

In the last decades, several interspinous process devices were designed as a minimally invasive treatment option for spinal stenosis. In order to minimise surgical trauma, interspinous process devices were recently discussed as an alternative posterior fixation in vertebral interbody fusions. Therefore, the purpose of this study was to evaluate the effect of a newly designed interspinous device with polyester bands (PBs) on range of motion (RoM) and centre of rotation (CoR) of a treated motion segment in comparison with an established interspinous device with spikes (SC) as well as with pedicl

Indications and limitations of the fixator TGF "Gex-Fix" in proximal end humeral fractures.

The incidence of fractures of the humerus has increased exponentially in recent years. The most used classifications for humerus fracture are morphological (Neer), biological (AO/ASIF) and descriptive (Hertel). The types of surgical treatment for humerus fracture include prosthetic replacement and synthesis using different devices, including the Tension Guide Fixator (TGF), Gex-Fix. External fixation for displaced proximal humeral fractures avoids dissection and soft tissue stripping and has been reported by some authors to be associated with higher union rates, a lower incidence of avascular

Minimally invasive dynamic hip screw for fixation of hip fractures.

We compared a minimally invasive surgical technique to the conventional (open approach) surgical technique used in fixation of hip fractures with the dynamic hip screw (DHS) device. Using a case-control design (44 cases and 44 controls), we tested the null hypothesis that there is no difference between the two techniques in the following outcome measures: duration of surgery, time to mobilisation and weight bearing postoperatively, length of hospital stay, mean difference of pre- and postoperative haemoglobin levels, position of the lag screw of the DHS device in the femoral head, and the tip-

Intramedullary Tibial Nail Fixation of Simple Intraarticular Distal Tibia Fractures.

The optimal treatment strategy for distal tibia fractures, especially those with intraarticular extension, remains controversial. Although open reduction and internal fixation with a plate and screw device is commonly performed for these injuries, the risk of soft tissue complications using this approach is significant. Staged treatment protocols and alternative means of fixation have been proposed to address these undesired events. Although potentially more technically demanding than fixation of diaphyseal or extraarticular tibial fractures, intramedullary nail (IMN) fixation of simple intraa

Rigid fixation of phalangeal and metacarpal fractures.

Rational use of rigid fixation in hand surgery requires awareness of the advantages as well as the potential pitfalls of this relatively complex method of fracture management. A rational decision also necessitates familiarity with closed techniques of internal fixation which, in most cases, provide superior alternatives for the phalanges and metacarpals. The optimal application of screws, plates, and tension-band wires to small bone fractures can be ascertained only by critical assessment of large series of cases employing these devices and comparisons with similar series using other technique

External fixation of distal radius fractures. Indications and technical principles.

External fixation of unstable intra-articular fractures of the distal radius has become an effective tool in the management of these difficult injuries. Careful assessment of the fracture pattern, appropriate patient selection, meticulous surgical technique, the appropriate choice of fixation device and pins, recognition of the need for augmentation with limited internal fixation or bone grafting, and aggressive postoperative rehabilitation provide the foundation for successful management of these fractures while minimizing complications. The surgeon must remember that the pin-bone interface i

Zygomatic repositioning and Le Fort II distraction with intraoral devices in Apert syndrome: A case report.

The aim of this case report is to describe the surgical technique and outcome using internal intraoral distraction devices in LeFort II distraction with zygomatic repositioning (LF2ZR). In Apert syndrome the midface is characterized by a complex hypoplasia, with the central part being more affected than the lateral orbito-zygomatic complex. In LF2ZR, the zygomas are repositioned and internally fixated, and the central midface is further advanced through a LeFort II distraction. In previous publications, the distraction has been performed using external halo-based devices. It seems that the LF2

Treatment of osteopetrosis combined with fracture around femoral internal fixation: a case report.

Osteopetrosis is a rare disease caused by bone resorption disorders due to a decrease in osteoclasts or functional abnormalities, which can easily lead to secondary fractures. Currently, there are few reports on the treatment of fractures around internal fixation in such patients. Here, we report on the treatment experience of a rare case of a patient with osteopetrosis who suffered a fracture around the internal fixation of the right femur. A 59-year-old female patient suffered a right femoral intertrochanteric fracture in 2014 and was diagnosed with osteopetrosis. She underwent right femoral

Stabilization of anterior mandibular fracture using different osteosynthesis devices: perioperative clinical notes.

PURPOSE: There is still no definitive consensus about the ideal technique in the treatment of anterior mandibular fractures. Therefore, this study aimed to determine clinical and radiographic outcomes of four different internal fixation devices used for this type of fractures. METHODS: This was a cohort study that included 64 fracture cases. Fractures were fixed using four types of open reduction internal fixation devices: single 2.0-mm mini-locking plates, double 2.0-mm miniplates, double lag screw and double microplates. Investigated variables were surgical duration, wound dehiscence, infec

Strategies for fixation of periprosthetic fragility fractures of the neck of femur below a well-functioning hip resurfacing arthroplasty: A case-series.

INTRODUCTION: Metal-on-metal Hip Resurfacing (HR) was performed in many young individuals as it conserved bone stock and had low wear rates, before it became less popular due to the detection of Adverse Reactions to Metal Debris. As such, many patients in the community have well-functioning HRs and as they age, the incidence of fragility fractures of the neck of femur around the existing implant is expected to increase. These fractures are amenable to surgical fixation as adequate bone stock remains in the head of the femur and the implants are well fixed. CASE-SERIES: We present a series of

"Biological Fixation" of Flexor Digitorum Longus: A New Modification of Tendon Transfer for Treatment of Posterior Tibialis Tendon Dysfunction.

Numerous surgical procedures for the treatment of posterior tibialis tendon (PTT) dysfunction have been developed, including tendon transfers, osteotomies, and arthrodesis. Among these methods, foot and ankle surgeons most frequently use flexor digitorum longus (FDL) tendon transfer in conjunction with medial translational osteotomy of the calcaneus. Formerly, some less invasive techniques have been described to reduce surgical-related complications after FDL tendon transfer. Herein, we introduced a new modification of short-stump FDL tendon fixation to the navicular bone and reported the func

Percutaneous Endoscopic Gastropexy for Gastric Volvulus in a High-Risk Older Adult Patient.

Gastric volvulus is a rare condition that may recur after endoscopic reduction. Although surgical fixation is often performed to prevent recurrence, this may be difficult in older adults with poor operative tolerance, necessitating alternative approaches. Here, we report a case of gastric volvulus without hiatal hernia in a 78-year-old woman that was successfully managed with percutaneous endoscopic gastropexy after endoscopic reduction. Four-point fixation was performed using devices commonly employed for gastric wall anchoring during percutaneous endoscopic gastrostomy. No recurrence was obs

[Experimental anatomical study of transpedicular stabilization of the spine].

Currently transpedicular fixators (TPF) are widely used for stabilization in treatment of degenerative spine disease. Despite indisputable advantages of transpedicular stabilization (durability and comfort in use) there are certain drawbacks of their application: 1) degeneration of adjacent segments (development of protrusions, herniations, instability); 2) absence of cyclic load in tissues of stabilized segment which may lead to their degeneration and osteolysis. We studied effects of loading on mobility of lumbar spine using cadaver specimens with preserved bony, ligamentous and articular st

Pelvic unlocking closed reduction device for treatment of severe traumas combined with pelvic fractures: a retrospective case series of 13 patients.

BACKGROUND: Closed reduction and internal fixation is the key to early and minimally invasive treatment of pelvic fractures in patients with severe trauma. Although the pelvic unlocking closed reduction device has been promoted to a certain extent, the therapeutic effect and surgical strategies for treating severe trauma have not been clearly clarified. Our study was aimed to explore the therapeutic effect and surgical strategy skills of the pelvic unlocking closed reduction device in treating severe traumas combined with pelvic fractures. METHODS: Data were retrospectively collected from 13

Technical Challenges and Surgical Considerations in Sacrospinous Ligament Fixation for Apical Prolapse Repair.

Background/Objectives: Sacrospinous ligament fixation (SSLF) is a well-established native tissue vaginal procedure for uterine/vault prolapse. Despite favorable success rates, the procedure presents technical challenges due to the deep operative field and proximity to critical neurovascular structures. To review current evidence regarding anatomical considerations, surgical technique, fixation strategies, suture materials, device-assisted methods, and perioperative complications in SSLF. Methods: A structured narrative review of the contemporary literature was conducted, focusing on comparativ

Ovine tests of a novel spinal cord neuromodulator and dentate ligament fixation method.

BACKGROUND: To improve methods for the treatment of intractable pain, we are developing a novel intradural spinal cord stimulator that could be either attached to the dentate ligaments of the human spinal cord or fitted around the dorsal arc of the cord itself. PURPOSE: Our goal was to carry out the first in vivo tests of these attachment methods in an ovine model using custom-built devices and instrumentation. For eventual translational studies, we also explored methods of mimicking a human dentate ligament attachment technique in this large animal model. METHODS: As a starting point, we in

New capsular tension segment with 2-point fixation for zonular weakness.

We describe clinical applications and surgical techniques for a new type of capsular tension segment (CTS) for use during cataract surgery. The Ambati CTS is distinguished from other CTS devices by having 2 eyelets close to each other, which allows it to distribute tension to 2 points, avoiding too much stress at a single point on the anterior capsulotomy, which prevents peaking of the capsulorhexis, and potentially reducing the risk for anterior capsule tear. Two of these CTS devices could possibly be used to provide 4-point fixation of a capsular bag in eyes with near-complete zonular instab
