METHODS: One hundred and sixty-five eyes underwent clear cornea punch trabeculectomy under a 5.0 mm wide scleral flap that was closed with two 10-0 Vicryl sutures after phacoemulsification and insertion of a single-piece PMMA lens. During the past 3 years, 12 patients (14 eyes) were encountered with severe reactions to virgin silk sutures used in cataract surgery. These reactions included nodular episcleritis, peripheral corneal ulceration, and wound necrosis with dehiscence, sometimes resulting in endophthalmitis or epithelial downgrowth. Thirty gastrotomies were performed in 15 dogs using 2/0 atraumatic needles. Various kinds of threads were used: Dexon as an absorbable suture, Black Perlon and Black Braided Silk as nonabsorbable sutures. Ten suture rows were made with each material. Single-row interrupted sutures were made with no suture in the mucosa. No suture insufficiency occurred. Dexon proved to be the most suitable material, although the nonabsorbable sutures did not cause complications, either.

Bacterial endophthalmitis developed in four patients after cataract surgery sutures were cut. Wound dehiscence was present in three cases, but in one the wound remained intact. Vitreous cultures identified Streptococcus viridans in two cases and Staphylococcus aureus and St. epidermidis in one case each. Antibiotic therapy was administered by intraocular, periocular, topical and systemic routes and three patients underwent vitrectomy. Visual function recovered to 20/50 in two cases and to 20/100 in a third. Retinal detachment in the fourth case resulted in loss of all visual function.

BACKGROUND: Numerous transosseous fixation techniques for flexor tendon injuries in Zone 1 of the hand have been described in the literature. While relatively high maximal loads to failure are documented in different biomechanical experiments, several tests revealed a low 2 mm gapping resistance of the tendon-to-bone repairs. We therefore aimed to investigate the effect on gap formation adding a peripheral suture to an established transosseous fixation technique. In addition, we analyzed the influence of different suture materials (braided vs. non-braided) on the stability of the core suture.
