Outcome of double-bundle ACL reconstruction using hamstring tendons.

The objective of this article is to describe the clinical outcome of the transitional double-bundle procedure for anatomical ACL reconstruction. Subjects included 78 patients (average age 25 years) who had undergone ACL reconstruction with the transitional double-bundle procedure with multi-stranded hamstring tendons. The femoral socket for the anteromedial (AM) graft was created at 5-6 mm from the mid-sagittal line of the intercondylar notch at 2:00 or 10:00, and that for the posterolateral (PL) graft was drilled adjacent to the AM socket at 3:00 or 9:00. For the tibial side, two tunnels were

Multiple variations of the extensor tendons of the forearm.

Anatomic variations of the extensor tendons of the forearm are frequent and its knowledge is important to assess the diseased and traumatized hand. During routine cadaveric dissection in the Department of Anatomy, Kasturba Medical College, Mangalore, India, we came across unusual variations in the left upper limb of a 51-years-old male cadaver. The variants are, the complete absence of the extensor pollicis brevis (EPB), the abductor pollicis longus (APL) having six slips of insertion with additional muscle slip from the brachio-radialis (BR). The extensor digitorum communis (EDC) had five ten

Bilateral spontaneous rupture of Achilles tendons: a case report.

Spontaneous bilateral rupture of Achilles tendon is rare. Rupture of the Achilles tendon has been described in patients on oral corticosteroid therapy. The sudden dorsiflexion of the plantar-flexed foot is the usual mechanism of injury. Spontaneous bilateral rupture is common in the degenerated tendon, which is often seen in patients with long-term corticosteroid therapy. This case is unusual because the patient has never taken steroids. We discuss the mechanism of injury and other probable causes.

Traumatic rupture of both peroneal longus and brevis tendons.

Injuries of peroneal tendons are rare. Diagnosis of traumatic rupture is often late and presents as chronic ankle instability. A case of a complete traumatic rupture of both peroneal longus and brevis tendons with acute clinical and radiological diagnosis is presented. Surgical repair was performed by direct end-to-end suture on the 4th day after trauma, with excellent functional outcome at 1-year follow-up.

Functional outcomes following surgical repair of wrist extensor tendons.

The long term results following the repair of open injuries to extensors carpi radialis longus and brevis and extensor carpi ulnaris have not previously been reported. A retrospective case note review was performed and patients were called back for assessment following surgical repair. Grip strength and pinch strength were reduced by 9.9% (p=0.017) and 11.5% (p=0.049). Wrist movement was also reduced. This demonstrates that the division of these tendons should not be regarded as trivial as they may have long-term adverse effects on wrist function. Information gained from this study may be bene

Radical forearm flap with vascularized tendons for hand reconstruction.

We treated six patients with soft-tissue defects involving the skin and extensor or flexor tendons using a cutaneotendinous radial forearm flap. There were five male patients and one female patient. Their ages at operation ranged from 22 to 65 years (mean 50 years). In one patient, the flexor pollicis longus tendon was reconstructed with a palmaris longus tendon graft. Four patients each had two extensor tendons reconstructed. This was performed either with the palmaris longus tendon folded in the center (n = 2) or with the palmaris longus and flexor carpi radialis tendons (n = 2). In the rema

Dislocation of the extensor tendons over the metacarpophalangeal joints.

Twenty-seven patients who had received treatment for dislocation of the extensor tendons over the metacarpophalangeal joint were reviewed. Sixteen patients had traumatic dislocations, seven had spontaneous dislocations, and four had congenital dislocations. The long finger was most frequently affected. The other fingers were affected almost equally. Displacement of the extensor tendon always occurred in the ulnar direction in the long and ring fingers. The index and little fingers exhibited different patterns of dislocation: two patients had ulnar dislocation of both the common and proprius te

[Lesions of zone II flexor tendons: splints and rehabilitation].

Rehabilitation of the hand after flexor tendon injuries in zone 2 is a challenge for both surgeons and therapists. Prevention of adherences is based on early controlled motion programs. Two different and therefore often complementary protocols are presented: 1. Kleinert: an active extension--passive flexion method; 2. Duran: a controlled passive method, as well as the splints required at this early stage of reeducation. The angles of flexion for the wrist and the metacarpal joints are discussed. When the splint is removed, active exercises and activities are proposed according to the interval

[Subcutaneous rupture of tendons and muscles of the extremities].

Under study there were 278 patients with 285 subcutaneous ruptures of tendons and muscles of the upper and lower extremities. The tendon of the long head of the humeral biceps muscle and Achilles tendon are most liable to subcutaneous ruptures (123 patients--43.2% and 97 patients--34% respectively). Pathohistological examinations have shown that subcutaneous ruptures of tendons and muscles usually take place against the background of dystrophic alterations reducing their mechanical strength. The authors find it expedient to use polymer materials (kapron, lavsan) for the restoration of injured

A repair operation for recurrent dislocation of peroneal tendons.

A lesion similar to that described by Bankart in recurrent dislocation of the shoulder was seen in seven patients with recurrent dislocation of the peroneal tendons. Detachment of the periosteum had resulted in the formation of a false pouch on the surface of the lower end of the fibula; into this pouch the peroneal tendons could easily dislocate. Reattachment of the periosteum to drill holes in the fibula prevented dislocation, and this anatomical method of repair is described. This lesion is one of the causes of recurrent dislocation of the peroneal tendons.

Tendon Anatomy and Tendon Disorders of the Wrist.

Wrist pain is common and can be attributed to tendon pathologies. This review aims to give a structured review of tendon anatomy, discuss anatomical pitfalls, and provide an overview of typical tendon pathologies of the wrist based on the current literature.Typical tendon pathologies of the wrist include de Quervain tenosynovitis, proximal and distal intersection syndrome, extensor and flexor pollicis longus tendon ruptures, and extensor carpi ulnaris subsheath injury. Typical pitfalls are multiple bundles of the abductor pollicis longus tendon or the centrally increased signal of the extensor

[Quadriceps Tendon and Patellar Tendon Rupture].

The vascularity of the quadriceps tendon influences the risk of tendon rupture. Quadriceps and patellar tendon ruptures are rare but serious injuries, with an incidence of 1.37/100000 and 0.68/100000, respectively. Risk factors include age, degeneration, and systemic diseases such as kidney disease and rheumatoid arthritis. While quadriceps tendon ruptures predominantly affect people over 50, patellar tendon ruptures are more common in younger, physically active people, often with pre-existing tendon pathologies. The most common mechanism of rupture of the quadriceps tendon is indirect trauma,

Biceps tendon and triceps tendon injuries.

Injuries to the biceps and triceps tendons about the elbow are relatively infrequent. Typically, they are traumatic events that occur as a result of a forceful eccentric contraction. Early recognition of these injuries and prompt intervention are the cornerstones to a successful outcome. Acute anatomic repair of complete injuries offers predictably good results. Conservative management, on the other hand, is typically reserved for partial injuries with little functional compromise, and for patients unfit for surgery. The challenges posed by chronic injuries can be addressed with a variety of s
