Trauma-induced tetanus: case series and an up-to-date pharmacoeconomic overview in Nepal.

INTRODUCTION AND SIGNIFICANCE: Tetanus is an acute, fatal, and vaccine-preventable infectious disease which is caused by anaerobic bacteria Clostridium tetani. This case series emphasizes on the importance of vaccine, and impact of financial status in the recovery of the patient and pharmacotherapeutic review of tetanus. METHODS: This case series was performed at the Department of Internal Medicine at a Tertiary Teaching Hospital. The authors here report two cases of rare and fatal tetanus that were diagnosed clinically, their treatment modality, and impact of socioeconomic status on the prog

Prophylaxis against tetanus in non-immune patients with wounds: the role of antibiotics and of human antitetanus globulin.

The potential value of oral erythromycin for antitetanus prophylaxis in non-immune patients with open wounds was assessed. Serum obtained by … ive burns. All the patients in the severely injured group showed an early appearance or increase in tetanus antitoxin to protective titres. Two of the three severely burned patients showed, respectively, a delayed appearance or an increase in antitoxin; the other burned patients showed a reduction from the initial pre-ATG titre, followed by a return to that titre a …

Serologic immunity to diphtheria and tetanus in the United States.

BACKGROUND: Serologic data on diseases that are preventable by vaccine are useful to evaluate the success of immunization programs and to id … o 1994. MEASUREMENTS: Serum samples obtained at a single time point were tested for diphtheria and tetanus antitoxin. RESULTS: Overall, 60.5% of Americans 6 years of age or older had fully protective levels of diphtheria antibody (> or =0.10 IU/mL) and 72.3% had protective levels of tetanus antibody (> 0.15 IU/mL). Ninety-one percent of Americans 6 to 11 years of …

Non-Wound Tetanus Mimicking Botulism in a Low-Resource Setting: Early Recognition and Successful Intensive Care Management.

INTRODUCTION: Tetanus is a vaccine-preventable, toxin-mediated neurologic emergency caused by Clostridium tetani. Although classically associated with contaminated wounds, the portal of entry may be trivial or unrecognized, resulting in so-called non-wound presentations that can delay diagnosis. Early tetanus may also mimic other toxin-mediated syndromes, including botulism, particularly when cranial or bulbar symptoms predominate. CASE PRESENTATION: A 41-year-old man in Somalia presented with rapidly progressive trismus, neck stiffness, dysarthria, perioral paresthesia, and marked masseter r

Tetanus: an uncommon cause of dysphagia.

A 53-year-old woman was examined at our medical center because of progressive dysphagia of 14 days' duration and a severe inability to open her mouth and swallow saliva. A barium esophagogram showed no obstruction, but pooling of barium in the hypopharynx suggested a neuromuscular disorder. The clinical diagnosis of tetanus was confirmed by electromyography. With appropriate therapy, the patient recovered during a period of 6 weeks. This case illustrates both an uncommon cause of dysphagia and an uncommon initial manifestation of tetanus.

Tetanus neonatorum--experience with intrathecal serotherapy at Muhimbili Medical Centre, Dar es Salaam, Tanzania.

In an attempt to lower the mortality rate of neonatal tetanus a study was undertaken to determine whether intrathecal serotherapy influences mortality from this disease. Sixty-six babies with tetanus neonatorum were studied. The mortality rate among babies who received intrathecal anti-tetanus serum (ATS) was 45% compared with 82% in the control group given intramuscular ATS (P congruent to 0.002). Infants who received intrathecal ATS also had fewer complications than controls (P less than 0.001) and the duration of hospital stay for the survivors was 19.3 days compared with 28.7 days for the

Tetanus neonatorum. (A preliminary report on the assessment of different therapeutic regimens).

BACKGROUND: Inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm of intermediate malignant potential, commonly arising in the lungs and intra-abdominal organs. Involvement of the urinary bladder is exceptionally rare, particularly in children, and may clinically and radiologically mimic malignant tumors. CASE PRESENTATION: We report the case of a 10-year-old girl who presented with painless macroscopic hematuria and syncope, necessitating blood transfusion. Initial imaging revealed a bladder mass, and biopsy initially suggested rhabdomyosarcoma. Definitive histopathological

Diphtheria antitoxin levels in US blood and plasma donors.

Plasma samples from 500 blood donors were titrated for diphtheria antitoxin (DAT) by the toxin neutralization (TN) test. Only 1.6% of donors had <0.01 IU/mL DAT, the minimum protective level against diphtheria; 15% had levels between 0.01 and <0.1 IU/mL, indicating basic protection, and 83.4% had levels > or =0.1 IU, indicating full protection. Three hundred fifty samples were studied by ELISA for diphtheria toxoid IgG antibodies to assess the utility of the assay as a quick, convenient method for evaluating diphtheria immunity. Although the correlation between TN and ELISA titers for the 350

Vaccine antigen interactions after a combination diphtheria-tetanus toxoid-acellular pertussis/purified capsular polysaccharide of Haemophilus influenzae type b-tetanus toxoid vaccine in two-, four- and six-month-old infants.

The geometric mean anti-PRP antibody titer in the DTaP/PRP-T group was 4.3 micrograms/ml compared with 7.0 micrograms/ml in the separate vaccine group (P < 0.05), and the …

Fentanyl therapy controls autonomic hyperactivity in tetanus.

This report describes the use of fentanyl in severe tetanus after failure of established therapeutic modalities (heavy sedation, neuromuscular blockade and ventilation). Cardiovascular instability accompanying severe tetanus secondary to sympathetic overactivity and raised catecholamine levels is associated with a mortality of over 50%. In this clinical situation, a variety of drugs with a primary or secondary action on the cardiovascular system has been used with varying success. The following case of severe generalised tetanus in the adult associated with autonomic hyperactivity, was success

Age-specific seroprevalence of poliomyelitis, diphtheria and tetanus antibodies in Spain.

In 1996, a seroepidemiological study was undertaken in Spain, with the main aim of estimating the population's immunity against poliomyeliti … ity against diphtheria steadily declined to a low of 32.3% in subjects aged 30-39 years. Similarly, tetanus antitoxin concentrations indicating basic protection were present in 98.9% of the under-14 years age group; thereafter, immunity declined, until reaching 54.6% in the 30-39 years age group.

Clinical findings and response to treatment of 17 cases of tetanus in horses (2012-2021).

Tetanus is a distressing and often fatal disease caused by exotoxins released by the bacterium Clostridium tetani. Clostridium tetani is a commensal of the gastrointestinal tract of humans and domestic animals, and its spores are highly resistant to environmental changes, acid, and alkali and may persist in the soil for many years. The disease is characterized by generalized muscular rigidity and spasms, hyperesthesia, convulsions, respiratory arrest, and death. Horses are the most susceptible domestic animals. Treatment is typically directed towards elimination of the source of the toxin, neu
