Thin basement membrane nephropathy (TBMN) often is diagnosed clinically when there is persistent dysmorphic or glomerular hematuria, but minimal proteinuria, normal kidney function, and no other obvious cause. This study investigated hematuria in patients with TBMN. A total of 112 patients with biopsy examination-proven TBMN were studied. All had hematuria at the time of presentation, with a mean urinary red blood cell (RBC) count of 256 +/- 250 x 10 3 /mL. Seventy-five (67%) patients attended for review over a median of 48 months (range, 3-120 mo) and provided a total of 485 urine specimens.

PURPOSE: Nephrostomy catheters are prone to kinking or damage because the thin, flexible silicone tube is too vulnerable against mechanical stress even when the proximal end is carefully fixed. We developed a simple method to reinforce the outside part of a thin catheter protruding from the skin.
MATERIALS AND METHODS: We treated 7 children with nephrostomy catheters or ureteral stents with a diameter of 8F or smaller. After insertion a large plastic tube was wrapped around the small catheter, and fixed to the skin and to the peripheral collection system with adhesive tape.

To disclose the prevalence of thin membrane nephropathy (TMN) in China and its clinical and pathological features, we prospectively studied TMN cases including 722 cases of renal biopsies in 2 years. 27 cases (male 7 cases, female 20) were diagnosed as primary glomerulonephritis. We developed new types of ultra-thin bronchofiberscopes, BF-2.2T and BF-2.7T to observe and photograph lesions of 2 mm or less in bronchioli. BF-2.2T and BF-2.7T can be bent to achieve a vertical range of 120 degrees. BF-2.7T has an additional channel for biopsy and can be used to collect cells. We employed a forearm flap that had been thinned through primary defatting for nasal covering in three cases to reduce the need for secondary revision. Partial flap loss did occur in one case due to subcutaneous vascular plexus injury, but acceptable results were obtained in the remaining two cases. Donor site concave deformities were substantially reduced by removal of fat from around the flap margin. Great care must be taken during surgery to avoid damage to the small vascular network around the pedicle to prevent flap necrosis.

Twelve patients with known pancreas divisum underwent thin-section computed tomography (CT) to determine the capability of CT to depict this pancreatic anomaly. Focal pancreatic enlargement was present in five patients. Two distinct pancreatic moieties separated by a fat cleft were noted in three patients; a fourth patient had focal atrophy in the distribution of the dorsal pancreas. Between April 1971 and January 1985, the University of California at Los Angeles (UCLA) Division of Surgical Oncology has seen 649 patients diagnosed with thin primary melanomas, as defined by a Clark level less than IV and Breslow depth less than 0.76 mm.
