Duplication or polydactyly of the thumb is among the most common congenital deformities that involve the hand. The purpose of this presentation is to review the Mayo Clinic experience with thumb duplication and to present preferred operative procedures to achieve the best clinical outcome and esthetic appearance for the hand. We propose that a combination or "best of parts", utilizing the components of both duplicates with retention of the ulnar duplicate as the base for reconstruction will provide the most functional clinical result. A clinical grading system and analysis of results from our …

The article presents a case of 42-year-old patient with Recklinghausen disease treated in the outpatient clinic for 5 years for hyperaesthesia of a thumb. On physical examination the thumb was painful but smooth, soft with no features of infection. Neuralgia and Raynaud syndrome were suspected. Additional investigations did not show significant abnormalities, however, successive forms of conservative treatment (including denervation of the thumb) were not successful. Instant, persistent pain caused by even a slight touch or thermal changes suggested glomus tumor of the thumb. Physical examinat…

Eighteen cases of turret exostoses of the hand have been reported, all associated with dorsal injuries. We present a case of turret exostosis involving the volar aspect of the thumb.

The thumb accounts for 40 to 50% of hand function. Reconstruction of soft-tissue contractures include release and coverage with skin grafts or various local, regional, distant, or free flaps. Thumb length, so important for prehension and opposition, can be restored by phalangealization, pollicization, or toe-to-thumb transfer. Secondary techniques such as metacarpal distraction-lengthening or osteoplastic reconstruction are rarely indicated.

Most rheumatoid patients will present with one or more thumb deformities at some stage of their clinical history. The goal of treatment is restoration and maintenance of stable and painless motion. Treatment is based on the type and stage of the deformity. The boutonniere thumb is the most common deformity. Metacarpophalangeal arthrodesis is preferred for isolated metacarpophalangeal involvement. For advanced cases in a low-demand patient, metacarpophalangeal arthroplasty with interphalangeal arthrodesis is performed. In the higher demand hand with an uninvolved carpometacarpal joint, arthrode…

Avulsion amputations of the thumb are generally thought to have a worse prognosis after replantation than other amputations. We report the results of 17 thumbs that had an avulsion amputation and were replanted. Fourteen of the 17 survived (82 percent). Our experience indicated that the survival rate was improved by restoring continuity of at least two veins and two arteries, using a Y-shaped vein graft and the princeps pollicis artery for the source of arterial circulation. Nerve grafts were used to bridge defects in avulsed digital nerves. When possible, avulsed tendons were reattached to th…

Two cases are described, one of replantation of an amputated thumb, the other of late reconstruction by toe transfer. A possible reason for the good results in thumb replantation is mentioned, and the advantages of toe transfer in thumb reconstruction are discussed. Suggestions are made as to the course of action to take when confronted with a patient with an amputated thumb.

Congenital fusion of the carpal bones is fairly common and may be associated with other congenital deformities in the hand. This report deals with an unusual congenital anomaly of the little fingers associated with fusion of the triquetrum and lunate. Both little fingers were adducted and rotated giving crude resemblance to thumbs on the ulnar borders of the hands. The deformity was easily corrected by osteotomy through the base of the fifth metacarpals.

During the period 1966--75 operation to restore thumb opposition was performed in thirty four patients. Out of twenty seven patients who could be traced, twenty one had obtained a satisfactory result, five had considerably improved, while one had to be classified as a failure. Complications from the operation and the motor tendon are discussed. Among 147 cases with injury to the median nerve treated during the same period, operation to obtain opposition was indicated in 14%.

The unique anatomical characteristics of the thumb offer a broad range of motion and the ability to oppose thumb and finger, an essential function for grasping. The motor function of the thumb and its orientation make it particularly vulnerable to trauma. Pathologic lesions encountered in this joint are varied, and imaging techniques play a crucial role in injury detection and characterization. Despite advances in diagnostic accuracy, acute thumb injuries pose a challenge for the radiologist. The complex and delicate anatomy requires meticulous and technically flawless image acquisition. Stand…

BACKGROUND: Toe-to-thumb transfer is an established procedure for reconstruction of traumatic thumb amputations. The four types of toe-to-thumb transfers are the second toe, the great toe, the wrap-around great toe and the trimmed great toe transfers. The purpose of this study is to conduct a systematic review of the literature to compare outcomes amongst different methods of toe-to-thumb transfers.
METHODS: A literature search using 'toe-to-thumb transfer' combined with 'thumb injury' and 'thumb reconstruction' as keywords and limited to humans and the English language identified 633 studies…

PURPOSE: Trigger thumb is a relatively uncommon condition in children. If it occurs or persists after 1 year of age, surgical release is the most traditional treatment method. The aim of this prospective study is to describe a technique for the percutaneous release of trigger thumb and to assess the clinical outcome of the presented technique in the pediatric age group.
METHODS: This study includes 31 thumbs of 26 consecutive children with a mean age of 2.6 years. An 18-gauge needle that was connected to 10-cc saline filled syringe was used as the surgical instrument for release. Contrary to …

The indications for reconstruction of the thumb in traumatic mutilations of the hand are presented in relation to the various conventional and microsurgical techniques. Several clinical situations are distinguished according to the severity of the hand mutilation and the number of severed fingers. Paucidigital amputations (one or two fingers including the thumb) should be treated by pollicisation or other techniques (Matev, osteoplastic reconstruction, second toe or "custom-made" transfers) depending on whether or not the other fingers need to be preserved. Pluridigital amputations (the thumb …

In some cases, congenital or acquired posterior instabilities of the MP joint of the thumb may cause pain or create functional disorders. It is possible to stabilize the metacarpophalangeal joint by partial resection of the ligament joining the two sesamoids which makes it possible to move and suture them toward each other. An enlargement plasty of the pulley then is required. This technique prevents hyperextension but without restricting flexion or opposition. The digital nerves must be carefully identified and protected.
