Patient Out-of-Pocket Costs and Healthcare Utilization Related to Mastectomy vs Breast Conservation: A Claims-Based Study.

BACKGROUND AND OBJECTIVES: Out-of-pocket (OOP) costs and healthcare utilization remain unknown for women facing breast conservation and mastectomy. METHODS: Women aged 18-64 who underwent upfront breast cancer surgery were identified from the IBM MarketScan Commercial Claims Database (2014-2017). Surgical groups included lumpectomy+radiation; unilateral mastectomy + /-reconstruction, and bilateral mastectomy + /-reconstruction. Cumulative OOP payments were assessed at 12-month intervals over 4 years postoperatively and adjusted to 2017 USD$. Multivariable regression identified factors associa

Effects of Workplace Variables on Workers Intentions to Misuse Prescription Opioids.

OBJECTIVE: Workers in industries with high rates of opioid dispensing as well as those with high rates of non-fatal work-related injuries are at greater risk for opioid misuse, which can lead to addiction, overdose, or death. METHODS: Using secondary cross-sectional data collected from 856 healthcare workers, this pilot study examines a conceptual model for workers' intentions to seek out prescription opioids and intentions to use opioids at higher doses over longer periods. RESULTS: Results showed significant protective effects of beliefs, injunctive and subjective norms, and behavioral con

Comparison of perception of stress and consumption of anxiolytics in hospital and out-hospital conditions: a cross-sectional study.

BACKGROUND: The workplace is a place where medical workers are exposed to extreme stress, particularly during medical emergencies or events of epidemic or pandemic proportions. Anxiolytic therapy is often used to overcome professional challenges. Deepening knowledge about the prevalence of the use of anxiolytics and the perception of stress among medical workers enables the timely recognition of problems and the preparation of measures to improve the working conditions and quality of life of medical workers. The study's primary objective was to investigate whether there were differences in the

Intensity of Healthcare Utilization Among Pediatric Hospice Patients Receiving Concurrent Care.

BACKGROUND: As children with life-threatening con- ditions near end-of-life, their disease burden and care requirements tend to increase, leading to high intensity of care in the hospital. Previous studies have examined patterns of healthcare usage among these children receiving hospice, but little is known about the impact of pediatric hospice involvement on healthcare utilization compared with usual care. We aimed to examine whether pediatric patients enrolled on concurrent hospice care utilize healthcare services at the same rate as prior to hospice enrollment. METHODS: A retrospective cha

Knowledge, attitudes, and predictors of urinary incontinence and overactive bladder as components of lower urinary tract symptoms among female healthcare professionals.

INTRODUCTION: Lower urinary tract symptoms (LUTS), including urinary incontinence (UI) and overactive bladder (OAB), are prevalent urogynecology conditions that affect women's quality of life. Despite their clinical importance, these symptoms are often underreported and inadequately managed, even among healthcare professionals. This study aimed to determine the prevalence and severity of UI and OAB among female healthcare professionals, assess their knowledge and attitudes toward UI, and identify predictors associated with these conditions. METHODS: A multicentre cross-sectional study was con

Cost-effectiveness of pharmacogenetic screening in the management of major depressive disorder in the Spanish Healthcare System.

BACKGROUND: Major depressive disorder (MDD) represents a sizable economic burden in Spain. Pharmacogenetic (PGx) screening to guide the choice of antidepressant medication (ADM) in MDD patients yields higher response and remission rates, which could reduce both healthcare and indirect costs. METHODS: We built a cost-effectiveness probabilistic Markov model with microsimulation using Tree Age Pro 2022, simulating a patient cohort from the SNHS starting ADM for MDD, and comparing PGx screening before starting ADM versus no screening (No PGx). We carried out a probabilistic sensitivity analysis

Hospital infection society prevalence survey of Healthcare Associated Infection 2006: comparison of results between Northern Ireland and the Republic of Ireland.

As part of the Third Healthcare Associated Infection (HCAI) Prevalence Survey of the United Kingdom and Ireland, HCAI point prevalence surveys were carried out in Northern Ireland (NI) and the Republic of Ireland (RoI). Here we explore the potential benefits of comparing results from two countries with different healthcare systems, which employed similar methodologies and identical HCAI definitions. Forty-four acute adult hospitals in the RoI and 15 in NI participated with a total of 11 185 patients surveyed (NI 3644 patients and RoI 7541). The overall HCAI prevalence was 5.4 and 4.9 in NI and

Parental knowledge, awareness, and intended healthcare-seeking responses toward congenital heart disease in the West Bank, Palestine: a cross-sectional study.

BACKGROUND: Congenital heart disease (CHD) is a major contributor to infant morbidity and mortality worldwide. Although prenatal screening, neonatal assessment, pulse oximetry screening, and routine pediatric evaluation contribute to CHD detection, some cases may remain unrecognized until after discharge or later in infancy. Parental recognition of warning signs may support timely healthcare-seeking, but it should be considered one component of a broader diagnostic and referral pathway. This study assessed parental knowledge, awareness, and intended healthcare-seeking responses toward CHD amon

Describing the indirect impact of COVID-19 on healthcare utilisation using syndromic surveillance systems.

BACKGROUND: Since the end of January 2020, the coronavirus (COVID-19) pandemic has been responsible for a global health crisis. In England a number of non-pharmaceutical interventions have been introduced throughout the pandemic, including guidelines on healthcare attendance (for example, promoting remote consultations), increased handwashing and social distancing. These interventions are likely to have impacted the incidence of non-COVID-19 conditions as well as healthcare seeking behaviour. Syndromic Surveillance Systems offer the ability to monitor trends in healthcare usage over time. MET

The administrative tasks burden in inpatient units: an observational study to quantify the impact on nursing staff's working time.

Background Administrative tasks (ATs) are perceived by healthcare workers as something that takes up an inappropriate number of nurses' working time and limits their ability to provide an adequate amount of 'direct care' to their patients. Understanding the kind of ATs mistakenly attributed in this period of staff shortage can be significant in the analysis of the congruence between the nurse's skills and the work/organizational process. In the inpatient units there are some ATs performed by nurses, such as organizing documentation and forms, answering phone calls, planning visits and appointm
