Traction Cybernetic Avatar System for Colon ESD.

Colorectal Endoscopic Submucosal Dissection (ESD) is a highly specialized and technically complex procedure designed for the en bloc resection of colonic tumors. The intricate and confined anatomical structure of the colon imposes considerable challenges during ESD. Implementing an effective traction mechanism can streamline the procedure, shorten operative duration, and reduce the incidence of complications. This study introduces a novel ESD traction system integrating a scaffold and a lesion traction mechanism, designed to provide controlled and adequate tension, thereby facilitating the pro…

Vitreopapillary Traction Causing Optic Nerve Head Elevation.

Purpose: The purpose of the study is to describe a case of vitreopapillary traction causing optic nerve head elevation. Observations: This case report describes a 64-year-old male who presented with left cloudy vision for 3 days. Dilated fundus exam showed normal right optic nerve with glial tissue nasally and left optic nerve head elevation and peripapillary hemorrhages in the left eye. Magnetic resonance imaging of the brain and orbits, erythrocyte sedimentation rate, and C-reactive protein were normal. Optical coherence tomography showed bilateral dense vitreous adhesions to the optic disc …

Bilateral Subdural Hematoma Caused by Cervical Traction.

Cervical traction therapy is commonly used for cervical spondylosis; however, complications, such as subdural hematomas can occur. Possible mechanisms include cerebrospinal fluid (CSF) leakage and bridging vein damage. A 51-year-old Japanese woman developed a persistent headache after 2 weeks of cervical traction therapy. Magnetic resonance imaging revealed bilateral subdural hematomas. Bed rest, oral analgesics, and daily infusion therapy failed to improve symptoms, but 2 epidural blood patches led to recovery. Although no direct CSF leakage was found, a spinal epidural lesion may have contri…

Acute onset traction-associated ulceration and alopecia.

Traction alopecia is a protracted form of hair loss resulting from persistent tension on the hair follicle . We report a case of a 9-year-old African American girl who presented with extensive scalp ulceration and alopecia 11 days after placement of a synthetic hair braid. Over a period of months, with consistent wound care and low-tension hairstyling, most of the hair regrew. We propose that the ulceration and hair loss in this case represents an acute variant of traction alopecia due to pressure-induced ischemia and necrosis.

Myopic Traction Maculopathy: Diagnostic and Management Strategies.

Pathologic myopia (PM) is an ocular disorder characterized by a spherical equivalent (SE) of more than - 6.0 diopters (D) or by an axial length (AL) of more than 26.5 millimeters (mm). PM is associated with myopic maculopathy (MM). The ATN classification describes all the aspects of MM which regroups atrophic, tractional and neovascular consequences to the sclera, choroid and retina of highly myopic eyes. The advent of OCT allowed to define the ultrastructural characteristics of the tractional changes in MM, described by the term myopic traction maculopathy (MTM). They include foveoschisis/mac…

The new Myopic Traction Maculopathy Staging System.

PURPOSE: To describe a comprehensive OCT-based classification of myopic traction maculopathy (MTM). METHODS: Two hundred eighty-one eyes with MTM (visited from 2006 to 2018), were retrospectively reviewed for age, best-corrected-visual-acuity (BCVA), axial length (AL), optical coherence tomography (OCT), and wide-field color fundus-photographs. The study was divided in two Phases. Phase 1: MTM types were categorized with OCT and correlated with age and BCVA. The type of staphyloma was described. Phase 2: the evolution of MTM was studied evaluating at least three OCT exams of each eye taken at…

"Isolated" Suprascapular Neuropathy: Compression, Traction, or Inflammation?

BACKGROUND: Several hypotheses have been proposed for the pathophysiology of suprascapular nerve (SSN) palsy, including compression, traction, and nerve inflammation. OBJECTIVE: To provide insight into the pathophysiology of isolated nontraumatic SSN palsy by performing critical reinterpretations of electrodiagnostic (EDX) studies and magnetic resonance (MR) images of patients with such diagnosis. METHODS: We retrospectively reviewed all patients referred to our institution for the past 20 yr with a diagnosis of nontraumatic isolated suprascapular neuropathy who had an upper extremity EDX st…

Bitemporal hair loss related to traction alopecia.

We present a 24-year-old woman that had received a diagnosis of alopecia areata in the past and was treated with topical 19 corticosteroids with little improvement. Instead, the patient exhibited bitemporal alopecia of one year of evolution related to 20 traction alopecia. Traction alopecia is characterized by localized hair loss related to persistent excessive traction. Although it is 21 initially a reversible condition, if this excessive traction is not removed permanent alopecia may develop.

Perioperative lower extremity peripheral nerve traction injuries.

Peripheral nerve traction injuries may occur after surgical care and can involve any of the lower extremity large peripheral nerves. In this review, the authors discuss injuries after knee or hip surgical intervention. The diagnosis, including electrodiagnostic studies, is time sensitive and also relies on a detailed history and physical examination. Successful prevention and treatment involve familiarity with risk and predisposing factors as well as prophylactic measures.

Staged surgical management of hypopharyngeal traction diverticulum.

A 50-year-old woman who had undergone cervical spine fixation 6 years earlier presented with dysphagia, regurgitation of undigested food, ha … le to resume a normal diet immediately and she experienced minimal symptoms. It is established that traction diverticulum is appropriately treated by removing the inciting anatomic factor(s). We propose that staged surgical management begin with the removal of the nidus followed by marsupialization of the diverticulum pouch. Standard staple diverticulotomy …

[Preoperative halo-traction in severe paralytic scoliosis].

AIM: During the past decades the treatment of severe paralytic scoliosis has developed towards surgical treatment. However there is controversial discussion about the need of pre-operative Halo-traction. The aim of this study was to built two groups of patients -- one group with and another one without pre-operative Halo-traction -- and to compare the results after surgical correction of scoliotic deformity with data from literature. METHOD: Between 2000-2003 twenty-five patients with severe neuromuscular spine deformity were treated surgically. Eight patients had preoperative Halo-traction, …

[Traction vitreoretinal opticopathy in proliferative diabetic vitreoretinopathy].

UNLABELLED: In patients with proliferative diabetic vitreoretinopathy the authors noticed tractive vitreoretinal distortion of the optic disk to the nasal side. As a result of this distortion a reduction in visual acuity, pallor of the optic disk, and changes in the visual evoked potentials were registered. The disease in called "tractive vitreoretinal opticopathy." METHOD: We examined 15 diabetics with vitreoretinal traction and temporal pallor of the optic disk, as well as a reduction in visual acuity. Six patients were observed retrospectively and 9 patients prospectively before and after …

Nerve transfer in brachial plexus traction injuries.

Brachial plexus palsy due to traction injury, especially spinal nerve-root avulsion, represents a severe handicap for the patient. Despite recent progress in diagnosis and microsurgical repair, the prognosis in such cases remains unfavorable. Nerve transfer is the only possibility for repair in cases of spinal nerve-root avulsion. This technique was analyzed in 37 patients with 64 reinnervation procedures of the musculocutaneous and/or axillary nerve using upper intercostal, spinal accessory, and regional nerves as donors. The most favorable results, with an 83.8% overall rate of useful functi…

Compression-traction radiography of lumbar segmental instability.

Compression-traction radiography was used to assess movements in the sagittal plane of the lumbosacral joint in a control group of asymptomatic normal individuals and in a patient group with a symptomatic lytic spondylolisthesis. Sagittal translatory movements were found in both groups, but the amount of displacement was significantly larger (P less than 0.001) in the patient group as compared with the controls. The average displacement in the patient group, using a relative measurement technique, was 5.2%, ranging from 0.2 to 15.4%; and the corresponding values in the controls resulted in a m…
