BACKGROUND: The importance of transitioning care from pediatric to adult practitioners is an often overlooked aspect of chronic disease. The benefits of a planned transition in health care are that patients will learn valuable life-long lessons on healthcare maintenance, when and how to seek medical attention for new issues that may arise, and improvements on their overall well-being. The purpose of this SAGES White Paper was to summarize the available knowledge for several pediatric surgical conditions to aid in transition of care for this patient population.
METHODS: The members of the SAGE

Incarcerated pregnant people face significant barriers when seeking health care services in prisons and jails, but little is known about their transitions from state prison health care systems to outside hospitals. This project analyzed current policies and procedures for care transitions for incarcerated people and presents policy recommendations to address issues of concern. We conducted in-depth interviews with stakeholders at a state prison, academic hospital, and private hospital to identify the barriers and facilitators to care transitions. Themes emerging from these interviews were oper

The characteristics of epidermolysis bullosa (EB) demand higher than average provider support for transition from pediatric to adult care. We administered an online Qualtrics survey to members of the Epidermolysis Bullosa Clinical Research Consortium (EBCRC), a group of providers who care for patients with EB, in order to examine their practices and perspectives on transition of care (TOC) and identify barriers to successful implementation. Sixteen of eighteen medical centers completed the survey. Eighty-eight percent of center representatives expressed concerns about their patients transition

Pulmonary embolism (PE) is a condition with a high rate of morbidity and mortality if it is not recognized and treated in a timely fashion. Point-of-care ultrasound (POCUS) is a useful tool that can help clinicians make prompt diagnosis. We present a case where we diagnosed massive PE through visualizing an intracardiac thrombus in transit, and we highlight some important ultrasonographic features.

Girls with Turner syndrome (TS) require special consideration during transition from childhood to adult care. During the transition years, treatment for short stature will be completed and sexual development induced in parallel with the peer group. The timing of sexual development may have later repercussions with respect to psychosocial development and partnership status. Late presentation of TS, which is so common, can result in additional difficulties with the transition process.

This is the third of three papers that summarize the second symposium on Transition in Epilepsies held in Paris in June 2016. This paper focuses on treatment issues that arise during the course of childhood epilepsy and make the process of transition to adult care more complicated. Some AEDs used during childhood, such as stiripentol, vigabatrin, and cannabidiol, are unfamiliar to adult epilepsy specialists. In addition, new drugs are being developed for treatment of specific childhood onset epilepsy syndromes and have no indication yet for adults. The ketogenic diet may be effective during ch

PURPOSE/OBJECTIVE: The aftermath of treatment for critical illness and/or critical injury in the intensive care unit (ICU) often includes persisting cognitive and emotional morbidities as well as severe physical deconditioning (a constellation termed post-intensive care syndrome, or PICS), but most patients do not receive psychological services before they enter the inpatient rehabilitation facility (IRF). Although a burgeoning literature guides the efforts of critical care providers to reduce risk factors for PICS - for example, reducing the use of sedatives and enacting early mobilization, t

OBJECTIVES: We estimated the incidence of homelessness during the transition to adulthood and identified the risk and protective factors that predict homelessness during this transition.
METHODS: Using data from the Midwest Evaluation of the Adult Functioning of Former Foster Youth, a longitudinal study of youths aging out of foster care in 3 Midwestern states, and a bounds approach, we estimated the cumulative percentage of youths who become homeless during the transition to adulthood. We also estimated a discrete time hazard model that predicted first reported episode of homelessness.

OBJECTIVES: Hydrocephalus is a prototypical chronic condition that follows children into adulthood. The objectives of this study were to (1) review how the health care needs of young adults with hydrocephalus are not being met, (2) estimate the numbers of adults with childhood-onset hydrocephalus, (3) describe a novel program to provide care for young adults with hydrocephalus and other chronic pediatric conditions, and (4) propose national strategies to promote successful hydrocephalus transition care.
RESULTS: Adults with hydrocephalus need continuous access to expert surgical and medical p

OBJECTIVE: Throughout the 1990s, the Soviet-style model in central and eastern Europe that provided free health services has been subject to radical reforms. Socio-economic inequalities have also increased but there is little information on inequalities in health care utilization. This paper examines the pattern of illness behaviour in Bulgaria, seeking evidence of inequalities in access to services and eliciting users' pathways to care.
DESIGN: Analysis drew on a representative population survey in Bulgaria (1997). The financial determinants of service use were tested in a multivariate model

INTRODUCTION: Older adults frequently face fragmented transitions after leaving the Emergency Department (ED), driving up acute care usage. This study evaluated the effectiveness of an ED-initiated Community Care Team Transitional Care Programme (CCT-TCP) on post-discharge healthcare utilisation and all-cause mortality among older adults.
METHODS: A quasi-experimental retrospective cohort study utilized 1:1 propensity score matching to pair older patients (≥65 years) in CCT-TCP with non-eligible controls (n = 678 per group) between January 2023 and June 2024 at a Singapore acute hospital. Dif…

Transition services for young people with long-term conditions often fall short. This qualitative study explored perspectives on service features that enable effective transition in epilepsy and juvenile idiopathic arthritis. Patients, parents, clinicians and service commissioners took part in semi-structured interviews (n = 18). Thematic analysis was used to identify key features, barriers and facilitators of effective transition across participant groups. Analysis led to the development of nine sub-themes which mapped to overarching domains of communication, capability, continuity and capaci

BACKGROUND: Youth aging out of foster care experience troubling rates of homelessness.
OBJECTIVE: This study examines whether policies directed towards youth transitioning to adulthood from foster care, Extended Foster Care (EFC) and Supervised Independent Living Placements (SILPs), reduce the risk of homelessness.
PARTICIPANTS AND SETTING: Participants were 595 youths aging out of foster care in California.
METHODS: Data from interviews conducted with the youths, and state child welfare administrative data on their involvement with the child welfare system, were used to examine predictors

OBJECTIVES: Microtransitions-defined as brief, nonurgent transitions in care-occur in post-acute and long-term care (PALTC) settings (eg, room changes, nonmedical outings, and medical outings). They occur at least as frequently as major transitions (eg, hospitalizations, discharges) and carry similar risks for adverse clinical outcomes if not properly managed. Limited evidence exists to guide safe microtransition coordination. This study aimed to establish best practice recommendations for staff facilitating microtransitions and evaluate the level of feasibility and clinical benefit of these r

AIMS: To develop a conceptual model integrating Robinson's Theory of Trust and Watson's Caritas Processes to guide trust-building approaches and caring practices that support adolescents and young adults with HIV during the healthcare transition from paediatric to adult care.
DESIGN: This is a discursive, theory synthesis paper that integrates theoretical frameworks and conceptually relevant literature to inform model development. The paper emphasises the importance of trust and caring science in the context of long-term engagement in care.
METHODS: An inductive theory-building approach was

INTRODUCTION: In recent years, more attention has been given to the transition of adolescents in urological care. In 2010, the current team investigated the needs of children with chronic bladder conditions, in preparation for transfer to adult care. A transition protocol was developed and implemented. The current study evaluated current transition care, in order to further improve the transition process.
OBJECTIVE: To give insight into the current transition care of adolescents with chronic bladder conditions since the introduction of a transition protocol in 2011.
