Percutaneous biopsy is a valuable investigation in the management of allograft rejection for all solid organs. Pancreas transplants have not been biopsed percutaneously, though open and percystoscopic biopsies have proved useful. We have compared percutaneous needle core biopsy with fine-needle aspiration cytology for the diagnosis of rejection in 18 patients receiving combined kidney and pancreas transplants and in one who was transplanted with the pancreas alone. A series of 114 penetrating corneal transplants performed in a public hospital in Santiago de Chile by the first author is reported. The first cause of corneal grafting was herpes simplex keratitis (37%), the second keratoconus (22%). Signs of rejection were observed in 32.5% of the grafts and of these, 32% lost transparency. Graft clarity was good in 74% of the grafts and poor in 17.5%. Poor transparency was due to graft rejection in 65% of the cases. To accomplish two objectives, morphological and functional improvements, in plastic operation of major ear anomalies, the authors presented a surgical method utilizing exclusively auto materials and several pedicles of skin, fascia and periostium. A partially implantable middle ear implantable hearing aid (Partial-MEI) is also an indication to those anomalies. Incidentally, we implanted the first Partial-MEI to a patient with unilateral atresia-microtia. The case was presented and demonstrated Partial-MEI implantation as one of the approaches for major ear anomalies to improve hearing.

A fistula in the upper arm between the end of the relocated basilic vein and brachial artery most closely satisfied criteria for an vascular access. The autogenous saphenous vein was found to be an unsuitable arterio-venous fistula for chronic dialysis. Exposed polytetrafluroethylene (PTFE) can be used as a subcutaneous arteriovenous conduit if suitable vessels have been exhausted.

Peliosis hepatis, an uncommon liver lesion characterised by blood-filled cavities bordered by hepatocytic plates, was found in 12 patients three to 17 months after renal transplantation. Hepatomegaly and portal hypertension were present in five of the six patients with major peliosis hepatis, and were absent in the other six with minor hepatic lesions. The long-term function of 110 cadaver kidney transplants preserved by hypothermic perfusion was compared with that of 79 fresh cadaver kidneys. PURPOSE OF REVIEW: In the United States, the leading indication for kidney transplant is primary kidney dysfunction arising from chronic hypertension and diabetes. However, an increasing indication for kidney transplantation is secondary kidney dysfunction in the setting of another severe organ dysfunction, including pancreas, liver, heart, and lung disease. In these settings, multiorgan transplantation is now commonly performed. With the increasing number of multiorgan kidney transplants, an assessment of guidelines and trends for in multiorgan kidney is necessary.

Clinical islet transplantation is an effective therapy in restoring physiological glycemic control in type 1 diabetics. However, allogeneic islets derived from cadaveric sources elicit immune responses that result in acute and chronic islet destruction.
