Primary Cueing Tremor: A Task-Specific Tremor of Billiard Players.

OBJECTIVE: To describe a unique form of task-specific tremor (TST) in billiards players. BACKGROUND: Task-specific movement disorders occur during highly learned activities. While task-specific dystonia in billiards players has been reported, TST has not been previously characterized. METHODS: Case series with literature review. CASES: Two right-handed billiards professionals developed tremor specifically during cueing. In Case 1, a 55-year-old experienced right arm tremor triggered by shoulder extension during cue positioning. Tremor resolved upon shooting, improved with alcohol, and was c…

Olfactory dysfunction in essential tremor versus tremor dominant Parkinson disease.

BACKGROUND: Essential tremor (ET) and tremor dominant Parkinson disease (TDPD) variant constitute the main causes of geriatric tremor which differentiation is not always an easy mission. The objective of this work was to study the olfactory performance in ET and PD patients for possible consideration as a differentiating biomarker. METHODS: This study was performed on 36ET, 22 TDPD variant and 24 healthy controls subjects (HCS) submitted to extended n-butanol Sniffin' Sticks test (SST) and olfactory bulbs volumetry (OBV). RESULTS: There were significant decreases in SST threshold, discrimina…

Knowledge about Essential Tremor: A Study of Essential Tremor Families.

BACKGROUND: Essential tremor (ET) is among the most common neurological diseases and it often runs in families. How knowledgeable ET patients and their families are about their disease has been the subject of surprisingly little scholarship. METHODS: To fill this gap in knowledge, we administered a comprehensive 32-item survey (i.e., questions about etiology, pathophysiology, symptoms and signs, natural history, and treatments) to 427 participants, including 76 ET probands, 74 affected relatives (AFRs), 238 unaffected relatives, and 39 spouses of unaffected relatives, all of whom were partici…

Characterizing the Normative Voice Tremor Frequency in Essential Vocal Tremor.

IMPORTANCE: Essential vocal tremor (EVT) is a neurologic voice disorder characterized by periodic fluctuations in pitch and loudness that can hinder intelligibility. Defining the normative range of vocal tremor frequency may assist in diagnosis and provide insight into disease mechanisms. OBJECTIVE: To characterize the normative voice tremor frequency in EVT (in hertz). DESIGN, SETTING, AND PARTICIPANTS: Cross-sectional observational study of 160 patients with EVT. The setting was a tertiary voice center. Participants were identified from a database of consecutive patients diagnosed as havin…

Toward quantitative characterization of essential tremor for future tremor suppression.

Tremor is the most common movement deficit and manifests in a variety of disorders, including Essential Tremor, Parkinson's Disease, Dystonia, and Cerebellar Ataxia. Although medication and surgical interventions have significantly reduced patient suffering, they are only partially effective and can carry undesired side effects, leaving many patients without satisfactory treatment options. Wearable tremor-suppressing devices could provide an alternative to medication and surgery. Multiple research groups have developed orthotic prototypes to low-pass filter tremor, but these devices have not y…

Re-Emergent Tremor of Parkinson's Disease Masquerading as Essential Tremor.

BACKGROUND: The re-emergent tremor of Parkinson's disease (PD) is generally recognized as a postural tremor. PHENOMENOLOGY SHOWN: A PD patient with a re-emergent tremor occurring during a task (spiral drawing), which on the surface produced a tremor that resembled that of essential tremor (ET). EDUCATIONAL VALUE: Researchers and clinicians should be aware of features of this re-emergent tremor to help distinguish it from that of ET.

Rest tremor suppression may separate essential from parkinsonian rest tremor.

Rest tremor at 4-6 Hz is typical for classical rest tremor (PT) of Parkinson's disease (PD). But rest tremor also appears in other tremor syndromes and may therefore cause a misdiagnosis. In this study we evaluated if suppression of tremor during movement onset is a characteristic feature of Parkinsonian Tremor distinguishing PT from Essential tremor (ET) and if this sign can be reliably diagnosed. Clinically diagnosed patients with PT (n = 44) and ET (n = 22) with rest tremor were included. Video sequences were recorded according to a standardized protocol focusing on the change of tremor amp…

Irregularity distinguishes limb tremor in cervical dystonia from essential tremor.

INTRODUCTION: Patients with cervical dystonia (CD) often have limb tremor that is clinically indistinguishable from essential tremor (ET). Whether a common central mechanism underlies the tremor in these conditions is unknown. We addressed this issue by quantifying limb tremor in 19 patients with CD and 35 patients with ET. METHOD: Postural, resting and kinetic tremors were quantified (amplitude, mean frequency and regularity) using a three-axis accelerometer. RESULTS: The amplitude of limb tremor in ET was significantly higher than in CD, but the mean frequency was not significantly differe…

Coherence analysis differentiates between cortical myoclonic tremor and essential tremor.

Familial cortical myoclonic tremor with epilepsy (FCMTE) is characterized by a distal kinetic tremor, infrequent epileptic attacks, and autosomal dominant inheritance. The tremor is thought to originate from the motor cortex. In our patient group, a premovement cortical spike could not be established on electroencephalogram (EEG) back-averaging. Corticomuscular and intermuscular coherence analysis can demonstrate a cortical common drive to muscles. We carried out coherence analysis of electromyography (EMG) of forearm muscles and EEG of contralateral motor cortex in 7 FCMTE patients, 8 essenti…

Familial cortical tremor with epilepsy: an under-recognized familial tremor.

The authors report three Japanese families presenting with cortical tremor and epilepsy. The patients showed either tremulous finger movements or seizures as the initial symptoms between 19 and 30 years of the age without progression. Postural tremor resembled essential tremor and responded to the anticonvulsants such as clonazepam, primidone and sodium valproate. Seizures were infrequent. These patients seem to have the same disorder as what the authors have described as 'familial cortical tremor with epilepsy'. Familial cortical tremor must be more common than previously thought and should b…

Olfactory dysfunction in essential tremor with rest tremor: a cross-sectional comparison with parkinson's disease and essential tremor.

BACKGROUND: Olfactory dysfunction is a recognized non-motor feature of Parkinson’s disease (PD) and has also been described in essential tremor (ET). However, olfaction in essential tremor with rest tremor (ET + RT), a recently defined subtype, remains insufficiently explored. OBJECTIVES: To assess olfactory function in patients with ET + RT and to compare their olfactory performance with those of patients with PD, ET, and healthy controls. METHODS: This cross-sectional study included 125 participants: 71 with tremor-dominant PD, 16 with ET, 16 with ET + RT, and 22 age- and sex-matched healthy…

[Resting tremor of Parkinson's disease changing into Holmes' tremor by cerebellar hemorrhage: an examination of the pathophysiological mechanism of tremor].

A 71-year-old male who suffered from Hoehn and Yahr stage III Parkinson's disease with bradykinesia, rigidity and a 5-6-Hz tremor at rest in the right extremities was admitted to our hospital due to the sudden onset of vertigo. Right cerebellar hemorrhage was confirmed by CT. The patient's resting tremor in the right extremities disappeared immediately following the cerebellar hemorrhage. Six days later, MRI showed Wallerian degeneration in the cerebello-rubro-thalamic tract. Approximately 5 months later, a 2-3-Hz Holmes' tremor gradually appeared in the right upper extremity. This tremor was …

Distribution of rest tremor in patients with Essential Tremor: Does it lateralize with simple kinetic, postural, or intention tremors?

BACKGROUND: Essential tremor (ET) is characterized by action tremor, although rest tremor may also occur. Despite this, rest tremor has not been studied extensively. A dearth of studies compare the relationship of rest tremor to other types of tremors in ET. METHODS: Two-hundred-and-thirteen ET cases underwent detailed neurological examination, including evaluations of rest, simple kinetic, postural, and intention tremors. A movement disorders neurologist assessed the prevalence of each type of tremor. Each instance of tremor was classified as either symmetrical (present and equally severe on…

Essential Tremor and Essential Tremor Plus Are Essentially Similar Electrophysiologically.

BACKGROUND: The merits of classifying the heterogeneous group of essential tremors into essential tremor (ET) and essential tremor plus (ETP) are debated. OBJECTIVES: We studied the electrophysiological and spiral characteristics of tremor in ET and ETP. METHODS: We reviewed standardized videos from a tremor database and clinically classified patients into ET, ETP, or dystonic tremor (DT). The following variables were derived from combined tri-axial accelerometry-surface electromyography (EMG)-peak frequency, total power, peak power, full width half maximum, tremor stability index and EMG-co…

Tremor Control Devices for Essential Tremor: A Systematic Literature Review.

BACKGROUND: There is a growing interest in nonpharmacological approaches for essential tremor (ET), including tremor cancelation devices. However, the true efficacy of such devices in ET remains unclear. METHODS: A systematic literature review was conducted using standardized criteria regarding efficacy and comfortability. Devices focused on design or experimental testing in which tremor was simulated in a robot were excluded. RESULTS: Out of 324 articles initially identified, 12 articles were included. Orthoses using biomechanical loading and neuromodulation with electrical stimulation, and…

Internal tremor in Parkinson's disease, multiple sclerosis, and essential tremor.

OBJECTIVE: Internal tremor (IT) is a poorly recognized symptom that has been described in Parkinson's disease (PD). Described as a feeling of tremor in the extremities or trunk without actual movement, ITs are not debilitating but can be bothersome to patients. The origin of the sensation is unknown., and ITs may be prevalent in other diseases than PD. The present study sought to expand knowledge about IT by confirming their presence in PD, and determining their prevalence in Multiple Sclerosis (MS), and Essential Tremor (ET). METHODS: A survey was developed in order to determine the prevalen…
