We present an interesting case of pemphigus vulgaris with severe and rare eye manifestations. An old lady with systemic and ocular manifestations of pemphigus vulgaris was treated with systemic corticosteroids and immunosuppressants, with no effect. Subconjunctival injection of triamcinolone acetate reduced the ocular signs and led to gradual reduction of the systemic medications.

Pregnant rhesus macaques were treated with 0.5 or 2.5 mg/kg triamcinolone acetonide (TAC) or 1.0 or 10.0 mg/kg dexamethasone sodium phosphate (DEX) between 20 and 50 gestational days (GD). Treatment with TAC at 2.5 mg/kg resulted in a fetal loss of 71%; 3/5 recovered fetuses displayed an encephalocele or meningocele. All other treatment groups displayed minor cranial skeletal abnormalities consistent with glucocorticoid-mediated teratogenesis. DEX was shown to have a lower teratogenic potential than TAC in this species.

The environmental and health hazards from chlorofluorocarbons are well documented. Triamcinolone acetonide spray was reformulated, incorporating isobutane as the propellant and used on 50 patients with a wide variety of corticosteroid responsive dermatosis. The efficacy of triamcinolone in the treatment of these dermatoses was not altered by the isobutane.
