Trichophyton indotineae in Virginia: diagnostic identification and case report.

BACKGROUND: Trichophyton indotineae is an emerging Trichophyton species associated with extensive dermatophytosis frequently demonstrating terbinafine resistance. While early cases were reported in South Asia, T. indotineae has since spread globally, with several cases reported in the United States since 2023. To our knowledge, we describe the first cases reported in Virginia. CASE SUMMARY: Patients A and B each presented to the dermatology clinic with a diffuse, pruritic rash unresponsive to previous courses of topical and antifungal therapy. Both presentations raised concern for T. indotine…

Consensus recommendations for Trichophyton indotineae: A modified Delphi study.

BACKGROUND: Trichophyton indotineae (T. mentagrophytes genotype VIII) is spreading globally and is often terbinafine resistant. Guidance for diagnosis and treatment remains inconsistent. METHODS: We performed a four-round modified Delphi study. A steering group developed a literature-informed, 42-item questionnaire on diagnosis and management. Consensus was predefined as ≥70% agreement. RESULTS: Thirty-four experts were invited; 20 completed rounds 1 and 2 and 19 completed rounds 3 and 4. Consensus was achieved for 34/42 items (80.95%). The panel advised suspecting T. indotineae infection in…

Clinical Isolate of a Multi-Antifungal-Resistant Trichophyton rubrum.

The multi-antifungal drug-resistant strain (NUBS21012) of Trichophyton rubrum was isolated from a patient with recurrent tinea corporis. The resistant strain encoded Phe at codon 393 instead of Leu (L393F) in the squalene epoxidase (SQLE) gene. The expression of genes encoding ATP-binding cassette transporter proteins increased in the strain compared to that of other strains. This result provides evidence that ATP-binding cassette transporter proteins are closely associated with azole resistance.

[Terbinafine-resistant dermatophytoses and onychomycosis due to Trichophyton rubrum].

BACKGROUND: In recent years, therapy-refractory courses of dermatophytoses have increasingly become the focus of attention. The most frequent pathogens are Trichophyton (T.) rubrum and T. mentagrophytes. In addition to local therapy, first-line treatment includes terbinafine, an allylamine antifungal agent that acts by inhibiting squalene epoxidase and thus interfering with ergosterol synthesis. In refractory cases, terbinafine resistance due to point mutation in the squalene epoxidase gene has been frequently detected. OBJECTIVES: The aim is to present specific aspects in the epidemiology of…

Hedgehog-Transmitted Trichophyton erinaceid Causing Painful Bullous Tinea Manuum.

Trichophyton is a species of fungus that commonly infects hedgehogs. As the popularity of keeping hedgehogs as pets increases, there have been reports of Trichophyton species infection in humans. Diagnosis and appropriate treatment of Trichophyton infection can be delayed owing to physicians' low index of suspicion and unfamiliarity with the physical manifestations. We describe a case of a 27-year-old healthy man who developed tinea manuum after handling a pet hedgehog. Presenting symptoms included a worsening bullous eruption and severe pain. Both fungal and histopathological study confirmed …

[Extensive inflammatory and childhood dermatophytosis caused by Trichophyton rubrum].

We report an observation of extensive and atypical dermatophytosis to Trichophyton rubrum coexisting with fingernails and tinea manuum candidiasis in a 13-year-old girl presenting inflammatory cutaneous lesions, involvement of scalp, as well as total dystrophic onychomycosis of feet associated with a perionyxis of hands. The results of the mycological examination of specimen showed fungal elements of septate hyphae and pilar parasitism. The fungal culture allowed the diagnosis of certainty of the dermatophytosis to T. rubrum. The therapeutic success of this infection was obtained thanks to the…

Pothomorphe umbellata: antifungal activity against strains of Trichophyton rubrum.

Trichophyton rubrum is a dermatophyte, which can cause infections in human skin, hair and nail. Pothomorphe umbellata (L.) Miq. (Piperaceae) is a native Brazilian plant, in which phytochemical studies have demonstrated the presence of steroids, 4-nerolidylcatechol, sesquiterpenes and essential oils. The objective of this study was to analyze the in vitro activity of extracts and fractions of P. umbellata on resistant strains of T. rubrum. The microdilution plate method was utilized to test Tr1, H6 and ΔTruMDR2 strains of T. rubrum; ΔTruMDR2 strain was obtained from H6 by TruMDR2 gene rupture, …

Epidemiological aspects of Trichophyton rubrum var. raubitschekii in Japan.

Trichophyton rubrum var. raubitschekii is a rare anthropophilic dermatophyte isolated around the world from tinea corporis, tinea cruris, tinea pedis and tinea unguium. In this study, the isolation rate of T. rubrum var. raubitschekii was studied in 200 cases of tinea pedis and tinea unguium in Japan. The 200 clinical isolates were shown to be of downy type as their colonies on Sabouraud's dextrose agar were white to cream, suede-like to downy, with a yellow-brown to wine-red reverse, and they produced few macroconidia. The type strain of T. rubrum var. raubitschekii (CBS 100084) and one clini…

Tinea corporis purpurica and onychomycosis caused by Trichophyton violaceum.

We report two cases of tinea corporis purpurica of the legs, presumably caused by self-inoculation of the mycete from the toenails, in two elderly women (80 and 78 years). Trichophyton violaceum was isolated from the skin and nails. Histological examination of a biopsy specimen from the leg lesions confirmed the diagnosis. The source of infection was an Ethiopian carer who had tinea capitis in the first case, and was undiagnosed in the second patient. Cases of purpuric variants of tinea corporis are rare and this is the first report of probable self-inoculation of T. violaceum from onychomycos…

Membrane transporter proteins are involved in Trichophyton rubrum pathogenesis.

Trichophyton rubrum is a dermatophyte responsible for the majority of human superficial mycoses. The functional expression of proteins important for the initial step and the maintenance of the infection process were identified previously in T. rubrum by subtraction suppression hybridization after growth in the presence of keratin. In this study, sequences similar to genes encoding the multidrug-resistance ATP-binding cassette (ABC) transporter, copper ATPase, the major facilitator superfamily and a permease were isolated, and used in Northern blots to monitor the expression of the genes, which…

Tinea capitis due to Trichophyton soudanense mimicking bacterial folliculitis.

We report the case of a 36-year-old Senegalese male with non-scarring alopecia of the scalp, including nodules and pustules, diagnosed as tinea capitis caused by Trichophyton soudanense. This dermatophyte is endemic in Central Africa and is becoming more frequent in Europe because of immigration. It has seldom been isolated in Italy. Tinea capitis is common in childhood and it is rare in adults, in which female sex is preferred. In adults, alopecic patches have to be distinguished from those due to other dermatoses inducing alopecia.

Microsatellite markers reveal geographic population differentiation in Trichophyton rubrum.

A worldwide selection of more than 200 isolates of the anthropophilic dermatophyte Trichophyton rubrum were analysed using seven microsatellite markers. Fifty-five multilocus genotypes were recognized, allowing a subdivision of the species into two populations. Both populations reproduced strictly clonally, showed a different predilection on the human host (scalp vs foot) and displayed geographic differentiation. Genotypes of one population originated predominantly from Africa, whilst the second population showed a worldwide distribution excluding the African continent. Genotypic diversity was…
