Deep brain stimulation in headache.

BACKGROUND: Deep brain stimulation of the posterior hypothalamic area was first introduced in 2000 to treat drug-refractory chronic cluster headache (CH). FINDINGS: So far, hypothalamic stimulation has been employed in 79 patients suffering from various forms of intractable short-lasting unilateral headache forms, mainly trigeminal autonomic cephalalgias. The majority were (88.6%) chronic CH, including one patient who suffered from symptomatic chronic CH-like attacks; the remaining were short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT), one…

Laboratory tests of headache disorders - dawn of a new era?

Context The classification of headache disorders has improved over the years, but further work is needed to develop and improve headache diagnosis within headache subtypes. The present review is a call for action to implement laboratory tests in the classification and management of primary and some secondary headaches. Background In this narrative review we present and discuss published tests that might be useful in phenotyping and/or diagnosis of long-lasting headache disorders such as migraine, tension-type headache, trigeminal autonomic cephalalgias, trigeminal neuralgia and persisting seco…

Suzetrigine as a novel treatment for headache disorders: A case series.

Suzetrigine is a selective voltage-gated sodium channel blocker that was recently approved by the US Food and Drug Administration for the treatment of acute pain. We retrospectively evaluated the efficacy and tolerability of suzetrigine for the treatment of different headache disorders in patients refractory to standard treatments in an academic headache center. These patients had a variety of headache and facial pain diagnoses, including chronic migraine, medication-overuse headache, trigeminal autonomic cephalalgias, nummular headache, occipital neuralgia, and persistent idiopathic facial pa…

The Exacerbation of Hemicrania Continua Mimics Trigeminal Neuralgia.

We report the case of a 46-year-old man with hemicrania continua presenting as exacerbations mimicking trigeminal neuralgia. The patient was tentatively diagnosed with trigeminal neuralgia, and treatment with various combinations of drugs was performed after the onset of pain. However, when the condition of the patient did not improve, we suspected hemicrania continua, and treatment with indomethacin was initiated. There was a marked alleviation of his pain within 24 hours. Thus, clinicians should be aware that the duration and frequency of exacerbations of hemicrania continua are variable.

Evaluation of routine magnetic resonance imaging of patients with chronic orofacial pain.

The aim of this study was to evaluate the reported presence of magnetic resonance imaging (MRI) pathologies (demyelination, space-occupying lesions, or trigeminal neurovascular contact within the transition zone) in patients with orofacial pain. Patient histories, demographic characteristics, and clinical features were compared between those with and without a reported MRI pathology. A retrospective service evaluation of all patients who had undergone MRI scanning to aid the diagnosis of orofacial pain conditions between 2012 and 2016 was conducted. Data were collected and statistical analyses

Modulation of trigeminal neuropathic pain by optogenetic inhibition of posterior hypothalamus in CCI-ION rat.

Posterior hypothalamus (PH), an important part of the descending pain processing pathway, has been found to be activated in trigeminal autonomic cephalalgias. However, there are very few studies conducted and information regarding its implications in trigeminal neuropathic pain (TNP). Therefore, we aimed to ascertain whether optogenetic inhibition of PH could affect the outcomes of a chronic constriction injury in the infraorbital nerve (CCI-ION) rat model. Animals were divided into the TNP animal, sham, and naive-control groups. CCI-ION surgery was performed to mimic TNP symptoms, and the opt…

Recurrent headache mimicking cluster headache: a diagnostic challenge.

A 37-year-old man presented with recurrent, severe left-sided headaches occurring at the same time each evening, accompanied by ipsilateral lacrimation, conjunctival injection, and nasal congestion. The clinical features were suggestive of cluster headache (CH), but poor response to high-flow oxygen and triptans raised concern for a secondary cause. Neuroimaging revealed an ethmoid sinus mucocele with adjacent bone remodeling, mimicking trigeminal autonomic cephalalgia. Surgical drainage led to complete symptom resolution, suggesting a secondary headache etiology. This case highlights the need

SUNCT, SUNA and short-lasting unilateral neuralgiform headache attacks: Debates and an update.

BACKGROUND: Short-lasting unilateral neuralgiform headache attacks (SUNHA) have the features of both short-lasting unilateral neuralgiform pain, such as trigeminal neuralgia or stabbing headache, and associated trigeminal autonomic symptoms, such as paroxysmal hemicrania or cluster headache. Recognizing and adequately treating SUNHA is essential but current treatment methods are ineffective in treating SUNHA. METHODS: We reviewed the changes in the concept of short-lasting unilateral neuralgiform headache attacks and provide a narrative review of the current medical and surgical treatment opt

Indomethacin-Induced Headache: Causing the Problem You Are Trying to Solve.

PURPOSE OF REVIEW: Indomethacin is an important medication in the headache medicine toolbox given its utility for both of the diagnosis and treatment of several primary headache disorders. Despite its prevalence in earlier rheumatologic studies, the possibility of drug-induced headache is a not commonly discussed in headache literature. RECENT FINDINGS: Herein, we describe a case of drug-induced headache after indomethacin trial for the treatment of an undifferentiated trigeminal autonomic cephalgia. Recognition of indomethacin-induced headache has important implications for patient education

Photophobia in headache disorders: characteristics and potential mechanisms.

Photophobia is present in multiple types of headache disorders. The coexistence of photophobia and headache suggested the potential reciprocal interactions between visual and pain pathways. In this review, we summarized the photophobic characteristics in different types of headache disorders in the context of the three diagnostic categories of headache disorders: (1) primary headaches: migraine, tension-type headache, and trigeminal autonomic cephalalgias; (2) secondary headaches: headaches attributed to traumatic brain injury, meningitis, non-traumatic subarachnoid hemorrhage and disorder of …

Sudden onset headaches in paediatric emergency departments: diagnosis and management.

Headache is one of the most common pain syndromes in the paediatric population. Headaches are classified as primary (migraine, tension-type headaches, trigeminal autonomic cephalalgia and other primary headaches) or secondary (e.g. post-traumatic). Non-febrile, non-traumatic headaches represent 1% of all paediatric emergency departments (PED) visits. Many patients present with an acute, moderate to severe pain, sometimes with a sudden onset. Sudden onset headache can be the main symptom of life-threatening neurological conditions as well as a sign of primary headaches such as thunderclap or st

Unusual trigeminal autonomic pain heralding hemichorea due to zoster sine Herpete vasculopathy.

BACKGROUND: Varicella zoster virus primary infection is responsible for chickenpox, whereas secondary infection or reactivation can lead to a variety of clinical scenarios. If latent infection is established in trigeminal ganglion, the reactivation can determine viral migration to cerebral arteries, which causes a cerebral vasculopathy and subsequently an ischemic stroke. PATIENTS: Here we report on a child experiencing recurrent episodes of headache mimicking a trigeminal autonomic cephalalgia, in the absence of any skin rash, which were followed by the occurrence of an ipsilateral hemipares

Intravenous methylprednisolone as a transition treatment in red ear syndrome: A case report.

Red ear syndrome (RES) is a rare condition of unknown etiology characterized by episodic attacks of unilateral ear pain, redness, and burnin … oscopy also failed. Given that the Red-Ear syndrome shares similar pathophysiological pathways with trigeminal autonomic cephalalgias, the patient was treated with high-dose intravenous methylprednisolone, and since then, he has been symptom-free for 6 months. High-dose steroid therapy might be a good alternative in late-onset RES as a transition treatment.
