Orofacial neuropathic pain and enhanced excitability of non-nociceptive-like and nociceptive-like trigeminal Aβ-afferent neurons in mice following infraorbital nerve chronic constrictive injury.

Orofacial neuropathic pain is a debilitating pain condition. It is poorly treated, partially due to an incomplete understanding of its underlying neurophysiological mechanisms. Orofacial neuropathic pain is often caused by injury or dysfunction of trigeminal nerve branches, including the infraorbital nerve (IoN). Clinically, a main symptom of orofacial neuropathic pain is mechanical allodynia, excruciating pain induced by gentle touch. While previous studies on neuropathic pain have mainly focused on the involvement of C- and Aδ-afferents, the potential contribution of Aβ-afferents to orofacia

C-X-C motif chemokine ligand 1 and its receptor C-X-C motif chemokine receptor 2 in trigeminal ganglion contribute to nerve injury-induced orofacial mechanical allodynia.

BACKGROUND: Orofacial ectopic pain induced by trigeminal nerve injury is a serious complication of dental treatment. C-X-C motif chemokine ligand 1 (CXCL1) and its primary receptor C-X-C motif chemokine receptor 2 (CXCR2) contribute to the development and maintenance of neuropathic pain in the spinal nervous system, but their roles in trigeminal neuropathic sensation are still poorly understood. OBJECTIVES: This study aimed to investigate the exact role of CXCL1 and CXCR2 in the regulation of orofacial ectopic mechanical allodynia and their potential downstream mechanisms in the trigeminal ga

Trigeminal neuropathic pain.

Typical trigeminal neuralgia is characterized by episodic, unilateral, lancinating, triggerable, often shock-like facial pains, and pain-free intervals. Trigeminal neuropathic pain could be defined as constant unilateral facial pain of variable intensity, is non-triggerable, and unremitting. Atypical trigeminal neuralgia is an overlap syndrome with both episodic and constant pain. Patients with clear pathology of the trigeminal system often have sensory loss and atypical pains. The leading theory of causation of typical trigeminal neuralgia points to minimal compression and demyelination at th

Early Diagnosis of Iatrogenic Post-Traumatic Trigeminal Neuropathies: From Clinical Gaps to Medico-Legal Implications-A Comprehensive Structured Narrative Review.

Background: Iatrogenic injuries of the inferior alveolar nerve (IAN) and lingual nerve (LN) are among the most relevant complications in oral surgery. Although most cases show spontaneous recovery, a significant proportion of patients develop persistent post-traumatic trigeminal neuropathies (PTNs). The aim of this narrative review is to critically analyze current diagnostic approaches for IAN and LN injuries, highlighting the limitations of conventional assessment tools and evaluating the potential role of emerging neurophysiological and imaging techniques in improving early diagnosis, progno

Iatrogenic trigeminal post-traumatic neuropathy: a retrospective two-year cohort study.

With the growing demand for dental work, trigeminal nerve injuries are increasingly common. This retrospective cohort study examined 53 cases of iatrogenic trigeminal nerve injury seen at the Department of Oral and Maxillofacial Surgery, University Hospitals of Leuven between 2013 and 2014 (0.6% among 8845 new patient visits). Patient records were screened for post-traumatic trigeminal nerve neuropathy caused by nerve injury incurred during implant surgery, endodontic treatment, local anaesthesia, tooth extraction, or specifically third molar removal. The patients ranged in age from 15 to 80ye

Orthognathic surgery-related nerve injuries: analysis and treatment modalities.

BACKGROUND: Orthognathic surgery, while highly effective in correcting dentofacial deformities, inherently poses a risk of neurosensory complications. Despite the growing volume of these surgical procedures, real-world data remain limited concerning the precise incidence, clinical course, and efficacy of various therapeutic interventions for nerve injuries in this specific context. This study aims to delineate the incidence, anatomical distribution, and treatment outcomes of trigeminal nerve injuries observed after orthognathic surgery within a tertiary care center. METHODS: This retrospectiv

Microsurgical reconstruction of the trigeminal nerve.

Head and neck tumor surgery or traumatic injuries in the maxillofacial region often result in discontinuity defects of peripheral branches of the trigeminal (fifth cranial) nerve, causing loss of sensation to those areas of the face, mouth, or jaws supplied by this important nerve. Injuries to the peripheral branches of the trigeminal nerve can be repaired by microsurgical techniques, either at the time of the original injury or ablative operation if conditions are favorable, or at a later date. Repair of a peripheral nerve injury has a good chance of a satisfactory outcome if done in a timely

Connexin 43 contributes to ectopic orofacial pain following inferior alveolar nerve injury.

BACKGROUND: Clinically, it is well known that injury of mandibular nerve fiber induces persistent ectopic pain which can spread to a wide area of the orofacial region innervated by the uninjured trigeminal nerve branches. However, the exact mechanism of such persistent ectopic orofacial pain is not still known. The present study was undertaken to determine the role of connexin 43 in the trigeminal ganglion on mechanical hypersensitivity in rat whisker pad skin induced by inferior alveolar nerve injury. Here, we examined changes in orofacial mechanical sensitivity following inferior alveolar ne

Dental innervation: functions and plasticity after peripheral injury.

Oral tissues including the periodontal ligament, gingiva, and tooth pulp have a relatively dense sensory innervation and a rich vascular supply. Teeth and supporting tissues are susceptible to tissue injury and inflammation, partly due to lack of collateral blood and nerve supply and to their low compliance. This review focuses on dental nerve functions and adaptive changes in the trigeminal ganglion and tooth pulp after peripheral injuries. An overview of the peptidergic innervation of oral tissues is presented, followed by a discussion of plasticity in neuropeptide expression in trigeminal p

Implant-related nerve injuries.

Injuries to branches of the trigeminal nerves are a known complication during dental implant placement. These injuries tend to be more severe than those experienced during other dentoalveolar procedures. This article reviews the types of nerve injuries and areas and situations of which clinicians should be cognizant when placing dental implants. Strategies to avoid injuries, and a management algorithm for suspected nerve injuries, are also discussed.

Maxillary nerve compression in cynomolgus monkey Macaca fascicularis: altered somatic sensation and peripheral nerve firing.

BACKGROUND: Trigeminal nerve is a major source of the sensory input of the face, and trigeminal neuropathology models have been reported in rodents with injury to branches of the maxillary or mandibular division of the trigeminal nerve. Non-human primates are neuroanatomically more closely related to human than rodents; however, nerve injury studies in non-human primates are limited. RESULTS: We describe here a nerve injury model of maxillary nerve compression (MNC) in the cynomolgus macaque monkey, Macaca fascicularis, and the initial characterization of the consequences of damage to this tr

Increased HCN Channel Activity in the Gasserian Ganglion Contributes to Trigeminal Neuropathic Pain.

UNLABELLED: Orofacial neuropathic pain caused by trigeminal nerve injury is a debilitating condition with limited therapeutic options. Hyperpolarization-activated cyclic nucleotide-gated (HCN) channels regulate neuronal excitability and are involved in the development and maintenance of chronic pain. However, the effect of HCN channel activity in the Gasserian ganglion on trigeminal neuropathic pain has not been examined. We evaluated nociceptive behaviors after microinjection of the HCN channel blockers ZD7288 or ivabradine into the Gasserian ganglion in rats with trigeminal nerve injury. Bot

Central VEGF-A pathway plays a key role in the development of trigeminal neuropathic pain in rats.

The study reported here investigated the role of the central vascular endothelial growth factor-A (VEGF-A) pathway in the development of trigeminal neuropathic pain following nerve injury. A Sprague-Dawley rat model of trigeminal neuropathic pain was produced using malpositioned dental implants. The left mandibular second molar was extracted under anesthesia and replaced with a miniature dental implant to induce injury to the inferior alveolar nerve. The inferior alveolar nerve injury produced a significant upregulation of astrocytic VEGF-A expression in the medullary dorsal horn. The nerve in

Trigeminal ganglion neuronal activity and glial fibrillary acidic protein immunoreactivity after inferior alveolar nerve crush in the adult rat.

Nerve injury to the mandibular division of the trigeminal nerve has been shown to cause satellite cell reactions that extend beyond the mandibular division of the trigeminal ganglion into the maxillary and ophthalmic divisions. The goal of this study was to determine whether any physiological abnormalities correlated with this dispersal of satellite cell reaction. We investigated the electrophysiological and satellite cell glial fibrillary acidic protein immunoreactivity (GFAP-IR) changes that occur within the trigeminal ganglion 3, 10 and 59 days after a crush injury of the inferior alveolar

Management of inferior alveolar nerve and lingual nerve injuries in the UK - a cross-sectional study.

Post operative nerve injury following mandibular third molar (M3M) potentially impacts a significant number of patients. A lack of consensus for the management of trigeminal nerve injuries exists. It is important to know how clinicians manage these injuries, and how confidently. A 16-question online survey using SurveyMonkey was developed and sent to all current UK members of three oral srelated societies (ABAOMS, BAOS and BAOMS) from January 2021 to March 2021. The survey consisted of open free text, binomial and variable scale responses related to the management of inferior alveolar nerve an

Dural neurogenic inflammation induced by neuropathic pain is specific to cranial region.

Up to now, dural neurogenic inflammation (DNI) has been studied primarily as a part of migraine pain pathophysiology. A recent study from our laboratory demonstrated the occurrence of DNI in response to peripheral trigeminal nerve injury. In this report, we characterize the occurrence of DNI after different peripheral nerve injuries in and outside of the trigeminal region. We have used the infraorbital nerve constriction injury model (IoNC) as a model of trigeminal neuropathic pain. Greater occipital nerve constriction injury (GoNC), partial transection of the sciatic nerve (ScNT) and sciatic

Schwann cells and trigeminal neuralgia.

Schwann cells are components of the peripheral nerve myelin sheath, which supports and nourishes axons. Upon injury of the trigeminal nerve, Schwann cells are activated and cause trigeminal neuralgia by engulfing the myelin sheath and secreting various neurotrophic factors. Further, Schwann cells can repair the damaged nerve and thus alleviate trigeminal neuralgia. Here, we briefly describe the development and activation of Schwann cells after nerve injury. Moreover, we expound on the occurrence, regulation, and treatment of trigeminal neuralgia; further, we point out the current research defi
