Trophoblastic disease and choriocarcinoma.

Gestational trophoblastic disease (GTD) is a group of diseases associated with pregnancies that demonstrate abnormal development of trophoblastic cells. GTD includes hydatidiform moles (HM) that may continue to further develop into gestational trophoblastic neoplasms (GTN), such as choriocarcinoma (CC). Gestational CC is a malignant mass development that may arise from HM, from other (normal) pregnancies or from other gestational events (such as ectopic pregnancies). The aim of this review is to outline current understating of the genetics and epigenetics of GTD and gestational CC and the link

The trophoblast model of cancer.

John Beard, the British embryologist and histologist, first proposed his trophoblast model of cancer in 1902. The model has subsequently been expanded by Kelley, and in current times, Gonzalez and Isaacs. The trophoblast model of cancer can be stated as a specified, scientifically testable model, including its core predictions that 1) adult stem cells are ectopic trophoblasts that have migrated to other tissues early in embryogenesis; 2) pancreatic enzymes are the key signal that converts the trophoblast into the stable placenta; 3) cancer arises from trophoblasts that have escaped regulatory

Gestational trophoblastic disease.

BACKGROUND: Molar pregnancy represents a significant burden of disease on the spectrum of Gestational Trophoblastic Disease (GTD). The incidence appears to be quite high in South Asia. The objective of this study was to determine the frequency of GTD, and clinical presentation, management and outcome of patients with molar pregnancy. METHODS: This retrospective, descriptive study was conducted at Nuclear Institute of Medicine and Radiotherapy (NIMRA), Jamshoro from 1st Jan to 31st Dec 2009. All patients diagnosed and registered as GTD were included in the study. The clinical records of all mo

Gestational trophoblastic disease.

Most women with gestational trophoblastic disease are of reproductive age. Because the disease is readily treatable with favourable prognosis, fertility becomes an important issue. Hydatidiform mole is a relatively benign disease, and most women do not require chemotherapy after uterine evacuation. A single uterine evacuation has no significant effect on future fertility, and pregnancy outcomes in subsequent pregnancies are comparable to that of the general population, despite a slight increased risk of developing molar pregnancy again. If women develop persistent trophoblastic disease, single

Gestational trophoblastic disease.

Gestational trophoblastic disease encompasses a range of pregnancy-related disorders, consisting of the premalignant disorders of complete and partial hydatidiform mole, and the malignant disorders of invasive mole, choriocarcinoma, and the rare placental-site trophoblastic tumour. These malignant forms are termed gestational trophoblastic tumours or neoplasia. Improvements in management and follow-up protocols mean that overall cure rates can exceed 98% with fertility retention, whereas most women would have died from malignant disease 60 years ago. This success can be explained by the develo

Gestational trophoblastic disease.

This review summarizes the primary management of molar pregnancies, surveillance after evacuation, and the evaluation and management of malignant gestational trophoblastic neoplasia (GTN). Most women with gestational trophoblastic disease can be successfully managed with preservation of their normal reproductive function. It is important to manage molar pregnancies properly to minimize acute complications and identify malignant sequelae promptly. Current International Federation of Gynecology and Obstetrics (FIGO) guidelines for making the diagnosis and staging of GTN allow uniformity for repo

Gestational trophoblastic disease.

Gestational trophoblastic disease consists of a broad spectrum of conditions ranging from an uncomplicated partial hydatidiform molar pregnancy to stage IV choriocarcinoma with cerebral metastases. Fortunately, with the advent of combination chemotherapy, the patient with advanced-stage disease has a significant chance of achieving complete remission. In addition, several studies have demonstrated that patients with a history of gestational trophoblastic neoplasia do not experience an increased risk of complications with future pregnancies. Patients who have undergone chemotherapy do not seem

Chorangioma with trophoblastic proliferation.

Only two examples of the entity of chorangiocarcinoma, in which there is a proliferation of both the vascular and epithelial components of the placental villi, have been reported in the literature. To test the hypothesis that chorangiocarcinomas are actually more common than implied by the literature, histological sections of chorangiomas were reviewed. Syncytiotrophoblast and cytotrophoblast proliferation, with nuclear atypia, similar to that found in trophoblastic neoplasia, were seen in 15 of 23 cases. Thus, 65% of chorangiomas fulfilled the diagnostic criteria to warrant re-assignment as "

Gestational trophoblastic disease.

BACKGROUND: Gestational trophoblastic disease consists of a group of interrelated diseases, including molar pregnancy, placental site trophoblastic tumor, and choriocarcinoma. METHODS: Advances in the diagnosis and management of gestational trophoblastic diseases over the past 5 years were reviewed. RESULTS: Molar pregnancy is now categorized as complete or partial on the basis of gross and microscopic histopathologic and karyotypic findings. Early detection of persistent gestational trophoblastic tumor (GTT) depends on careful postmolar gonadotropin follow-up and consideration of the diagno

Gestational trophoblastic disease.

Gestational trophoblastic disease is an aberration of pregnancy that occurs in one of 1,500 pregnancies in the United States. Abnormalities of uterine size, vaginal bleeding, and first trimester toxemia are common presentations. After a D & C, follow-up consists of close monitoring of the serum beta-hCG and adherence to contraception. Approximately 15% of patients have malignant disease requiring chemotherapy. Prognosis has changed from fatal to highly curable with preservation of the uterus for future childbearing. Nurses provide education and support to the patient and family as they deal wi

Gestational trophoblastic tumors.

Although virtually 100% of women who develop gestational trophoblastic tumors enter a long-term complete remission, there are many aspects of trophoblastic disease that arouse interest. Epidemiological studies have shown a large geographical variation in the percentage of conceptions that result in a hydatidiform mole and have stimulated studies on the immunological differences of the low and high risk populations. Chromosomal analysis is now complementing the pathological differentiation between complete and partial moles. There is still debate as to which factors are positively associated wi

Trophoblastic pulmonary embolism.

Trophoblastic embolism is subclinical in normal pregnancy, pronounced in eclampsia, and massive in hydatidiform mole. Self-limited acute respiratory distress arises in 3% to 10% of molar pregnancies at the time of uterine evacuation. Infrequently death occurs; the principal findings are trophoblastic emboli in the pulmonary arterioles, edema of the lungs, and dilatation of the right side of the heart. Hyperthyroidism may develop, and fibrin may line the alveolar walls. Pathogenetic mechanisms include heart failure, hyperthyroidism, dilutional anemia, and pulmonary arteriolar blockage. Infusion

Trophoblastic tumors in Greenland.

From 1950 through 1974, 37 cases of hydatidiform mole not followed by malignancy and 11 cases of invasively growing trophoblastic tumors (IGTT) occurred among indigenous Greenlandic women. The overall incidence of benign mole was 1:850 births, only slightly higher than most incidences in low-risk areas like Western Europe, North America, Australia, and Israel. In contrast, the overall incidence of IGTT, 1:2861 births, and the minimum incidence of histologically confirmed choriocarcinoma, 1:5245 births, are among the highest population-based incidence on record. A marked increase in incidence o

Gestational trophoblastic neoplasms.

A review of the classification, biological behavior, clinical diagnosis, and treatment of gestational trophoblastic neoplasms is presented. The events which followed the utilization of chemotherapy as a successful means of therapy are included. Especially important has been the recent recognition that high risk patients, i.e., patients with HCG titers greater than 100,000 IU/24 hr, duration of symptoms greater than four months, and metastasis to the brain and/or liver, with metastatic trophoblastic disease can be managed successfully if treated initially with combination chemotherapy and adjuv
