Definitions of the term 'challenge' as applied to the African trypanosomiases are reviewed. Data from one West and one East African site show simple linear relationships between the incidence of trypanosomiasis in both humans and animals, and either the amount of man-tsetse contact, or the Apparent Density of flies. African trypanosomiasis of the Rhodesian variety occurred in an American who recently traveled to Tanzania. Skin findings included a fluctuant, indurated, tsetse-fly bite site (chancre) and a fleeting, erythematous, macular eruption. The diagnosis was confirmed by the presence of the organism in peripheral blood smears. The patient's condition responded to intravenous suramin. African trypanosomiasis should be included in the differential diagnosis of patients who have visited areas where this disease is endemic, and in whom the appropriate skin findings are present.

Comparative studies were carried out on serums from rabbits inoculated with several Trypanosoma strains and 50 serums from sleeping sickness cases. The techniques used were antigen-antibody reaction in gel according to Ouchterlony, and electrosynerese for which directions are outlined. Electrosynerese has been shown to be a much faster and more sensitive method of detecting precipitating antibodies in trypanosomiasis.

Human African trypanosomiasis (HAT, or sleeping sickness) is a protozoan parasitic infection caused by Trypanosoma brucei rhodesiense or Trypanosoma brucei gambiense. These are neglected tropical diseases, and T.b. rhodesiense HAT is a zoonosis. We review current knowledge on the burden of HAT in sub-Saharan Africa, with an emphasis on the disability-adjusted life year (DALY), data sources, and methodological issues relating to the use of this metric for assessing the burden of this disease. We report the first apparent case of a splenectomized individual who developed severe trypanosomiasis with central nervous system involvement. The patient was a 41-year-old man who participated in an east African safari.
