A young non-smoker with high ADA pleural effusion. It's not always tuberculosis.

Lung cancer is posing an ever-increasing medical and social problem due to its increasing morbidity and mortality. Here we report a case of a young male who was being treated as tubercular pleural effusion but was ultimately diagnosed with non-small cell carcinoma. While considering the diagnosis of pleural effusion, in the context of country endemic for tuberculosis like India, it is not surprising that many clinicians rather prefer to consider pulmonary tuberculosis as the first differential, while keeping diagnosis of malignancy as the least likely differential diagnosis in the young patien

Misdiagnosis of primary pleural DLBCL as tuberculosis: A case report and literature review.

Diffuse large B-cell lymphoma (DLBCL) is the most common type of non-Hodgkin lymphoma (NHL). DLBCL presents with pleural involvement at an advanced stage; however, primary pleural lymphomas without any other site of involvement are rare, and the possibility of misdiagnosis is high, particularly in developing countries, where tuberculosis or other severe pulmonary infections remain a major health concern. Furthermore, lymphoma and tuberculosis share a number of common clinical characteristics, such as fever, night sweats, feeling of satiety after a small meal, fatigue and unexplained weight los

[Comparison of radionuclide studies with pleural morphological changes in patients with tuberculosis pleurisy].

Forty-seven patients with pleurisy of tuberculous etiology underwent a radionuclide study using the reagent 99mTc-labeled technefit, the result of which was compared with the morphological changes in the pleural biopsy or resection specimens. First-degree pleural resorptive dysfunction (PRD) was ascertained in 27.6% of the patients. Morphologically, there were signs of acute edema of all pleural layers in both the area of inflammation and the proximal portions with reduced vessels in the microcirculatory bed (MCB). These patients had medical treatment. 59.6% of patients had second-degree PRD--

Discrete pleural nodules associated with a parasternal mass: an unusual manifestation of tuberculosis.

The case presented describes an unusual appearance of thoracic tuberculosis with multiple pleural nodules associated with a parasternal mass as depicted on computed tomography. The patient is a 22-year-old woman who presented with pleuritic chest pain, a left parasternal mass, and weight loss. The pleura of the left hemithorax was studded with multiple low-attenuation, rim-enhancing nodules, and a left parasternal mass with similar imaging features was seen anterior to an enlarged left internal mammary lymph node. There were no pulmonary parenchymal changes nor any mediastinal or hilar lymphad

Interferon-gamma in 388 immunocompromised and immunocompetent patients for diagnosing pleural tuberculosis.

The level of interferon-gamma (IFN-gamma) in pleural fluid has been reported to be increased in pleural tuberculosis. Nevertheless, its diagnostic value has not yet been well-established, and immunocompromised patients have not previously been evaluated. The aim of this study was to determine the value of the IFN-gamma level in pleural fluid for diagnosing tuberculous pleurisy in immunocompetent and immunocompromised patients. Three hundred and eighty eight consecutive patients were studied prospectively (73 with tuberculous pleural effusions, including nine with concurrent human immunodeficie

[Treatment of pleural tuberculosis with a short 4-month regimen: preliminary results].

The aim of this prospective study was to evaluate the results of a four-month regimen (2HRZ/2HR) in the treatment of isolated tuberculous pleurisy with effusion. It included 68 patients presenting with histologically confirmed tuberculous pleurisy treated with 2HRZ/2HR, physiotherapy and aspiration of the pleura. Patients were seen every 2 months during treatment, and then 1, 3, 4, 6, 12, and 24 months after the end of treatment. Preliminary results show a favourable response in 58 patients (85%) and minimal pleural thickening in 10 patients. There were no failures, no relapses nor any extrapl

[T lymphocytes in the circulating blood and pleural fluid of patients with tuberculosis or cancer].

Inflammatory pleurisies in the adult are particularly rich in CD4 lymphocytes, capable of inducing and facilitating the immune reaction. The aim of this study was to determine the percentage of CD4 and CD8 lymphocytes in pleural fluid and blood in 13 patients with tuberculous pleurisy and 12 patients with pleural neoplasms, to establish the diagnostic value of this analysis. Independent of the etiology of the effusion there is a concentration of CD4 T lymphocytes in the pleural fluid compared to the blood (49.52 +/- 12.36% and 39.28 +/- 6.74%, respectively, p value less than 0.001). However, t

Pulmonary involvement in pleural tuberculosis: how often does it mean disease activity?

OBJECTIVE: To evaluate in chest X-rays and high-resolution computed tomographies of patients with pleural tuberculosis, the incidence of parenchymal and mediastinal lung lesions suggestive of active disease. METHODS: Prospective study (2008-2009) evaluating the radiographic and tomographic abnormalities of 88 HIV-negative patients with pleural tuberculosis (unilateral effusion). The images were reviewed by 3 independent specialists, and the observed changes were classified according to previously established criteria: presence or absence of signs suggestive of disease activity, and nonspecifi

Low Adenosine Deaminase (ADA) Does Not Exclude Tuberculosis: A Case of Loculated Pleural Effusion Diagnosed by a Medical Thoracoscopic Pleural Biopsy.

Tubercular pleural effusion is a common cause of exudative pleural effusion, particularly in endemic regions, and adenosine deaminase (ADA) is widely used as a supportive diagnostic marker. However, lower-than-expected ADA levels can lead to diagnostic uncertainty. We report a 21-year-old male who presented with prolonged fever, weight loss, and dry cough. He was initially managed as a case of parapneumonic effusion due to lymphocytic exudative pleural fluid with low ADA levels. Persistent symptoms and imaging findings of multiple loculated pleural collections with associated lung collapse pro

Comparative accuracy of pleural fluid unstimulated interferon-gamma and adenosine deaminase for diagnosing pleural tuberculosis: A systematic review and meta-analysis.

OBJECTIVE: We compared diagnostic accuracy of pleural fluid adenosine deaminase (ADA) and interferon-gamma (IFN-γ) in diagnosing tuberculous pleural effusion (TPE) through systematic review and comparative meta-analysis. METHODS: We queried PubMed and Embase databases to identify studies providing paired data for sensitivity and specificity of both pleural fluid ADA and IFN-γ for diagnosing TPE. We used hierarchical summary receiver operating characteristic (HSROC) plots and HSROC meta-regression to model individual and comparative diagnostic performance of the two tests. RESULTS: We retriev

Evaluation of Cepheid's Xpert MTB/Rif test on pleural fluid in the diagnosis of pleural tuberculosis in a high prevalence HIV/TB setting.

BACKGROUND: Diagnosis of pleural tuberculosis (TB) using routinely available diagnostic methods is challenging due to the paucibacillary nature of the disease. Histopathology and pleural tissue TB culture involves an invasive procedure which requires expertise and appropriate equipment, both often unavailable in many health units. Xpert MTB/Rif test has been widely evaluated in sputum specimens but data on its performance in pleural TB is scarce. We evaluated the accuracy of Cepheid's Xpert MTB/Rif test on pleural fluid in the diagnosis of pleural TB in Uganda. METHODS: Consenting adult patie

Host genetic polymorphisms, immune response, and susceptibility to pleural tuberculosis: A systematic review.

BACKGROUND: Pleural tuberculosis is one of the most frequent extrapulmonary manifestations of Mycobacterium tuberculosis infection and is characterized by a pronounced immune-mediated response with a low bacillary burden. Host genetic factors appear to play a central role in its immunopathogenesis. METHODS: A systematic review was conducted in accordance with PRISMA guidelines and prospectively registered in PROSPERO (CRD420251051395). Searches were performed across MEDLINE/PubMed, Embase, and SciELO databases, covering publications from 1970 to December 2023. The search strategy combined ter

Metabolic biomarkers differentiate extrapulmonary tuberculosis from pulmonary TB and non-TB pleural effusions.

Tuberculosis (TB) is one of the leading causes of mortality in humans with clinical manifestations as pulmonary and extrapulmonary forms. TB pleural effusion (PITB) is one of the main forms of extrapulmonary TB, which is challenging to diagnose due to the overlapping clinical symptoms with other non-TB pleural effusions and poses severe complications in the treatment and management of disease. New biomarkers are urgently needed to improve the diagnostic accuracy for PlTB. Here we applied high-throughput targeted liquid chromatography-mass spectrometry (LC-MS/MS) metabolomics using a 96-well pl
