Tungiasis among five communities in south-western Trinidad, West Indies.

The prevalences of Tunga penetrans Linn. within the five townships of Granville, Fullerton, Icacos, Coromandel and Cedros in south-western Trinidad, West Indies, as determined by physical examination of all inhabitants present during the survey, were 17.0% (38/223 subjects), 15.7% (36/237), 31.4% (102/325), 17.4% (42/242) and 17.9% (50/280), respectively. The rate in Icacos was significantly higher than in the other four sites (P < 0.001). Not only were males more likely to be infected than females in all five study sites (P < 0.0007) but chigoe-flea burdens were also higher in males than in f

Reduction of tungiasis prevalence, intensity, and morbidity during a two-year long community-based tungiasis control project in a hyperendemic region in Karamoja, Uganda.

Tungiasis is a widespread and debilitating zoonotic Neglected Tropical Disease (NTD). Manual extraction of the sand fleas with non-sterile sharp instruments is the most common but unsafe treatment method in affected communities. Topical application of a dimeticone oil formula (NYDA) has previously been shown to be a safe and effective method of killing embedded sand fleas. The objective of this study was to evaluate a two-year long humanitarian One Health tungiasis control project in 17 villages in Napak district, Karamoja region, Northeastern Uganda. The community-based intervention included …

[Tungiasis: from travel-associated dermatosis to a neglected tropical disease].

Tungiasis is an ectoparasitic skin disease caused by the penetration of female sand fleas (Tunga penetrans) into the epidermis. It is endemic in Latin America, the Caribbean, and sub-Saharan Africa and occurs in Europe predominantly as a travel-associated dermatosis. Characteristic lesions are whitish-yellow papules or nodules with a central dark punctum, predominantly affecting the feet and often accompanied by erythema, pruritus, and pain. Severe infestations may lead to ulceration, nail dystrophy, and secondary bacterial infections. In low-income endemic communities, particularly among chil…

Clinical interventions for tungiasis (sand flea disease): a systematic review.

Tungiasis (sand flea disease) is an epidermal parasitic skin disease occurring in resource-limited communities. There is no standard treatment for tungiasis, and available treatment options are scarce. To our knowledge, this is the first systematic review aimed to assess randomised controlled trials (RCTs) investigating interventions for tungiasis. We systematically searched databases including MEDLINE (EBSCOhost), CENTRAL, CINAHL, PubMed, Web of Science, SciELO, LILACS and Embase (Scopus) for RCTs in any language, from inception of the databases until June 12, 2021. RCTs exploring preventive …

Imported tungiasis in a Japanese student returning from East Africa.

Ectoparastiosis by Tunga penetrans is becoming more common in non-tropical countries as people travel more to endemic areas. We present here a classic case of imported tungiasis including a history of visit to an endemic region and its typical presentation. Diagnosis with dermoscopy, treatment and prevention are discussed.

Tungiasis Stigma and Control Practices in a Hyperendemic Region in Northeastern Uganda.

Neglected tropical diseases are known to be highly stigmatized conditions. This study investigates tungiasis-related stigma and control practices in the impoverished Napak District in rural northeastern Uganda, where tungiasis is hyperendemic and effective treatment is unavailable. We conducted a questionnaire survey with the main household caretakers (n = 1329) in 17 villages and examined them for tungiasis. The prevalence of tungiasis among our respondents was 61.0%. Questionnaire responses showed that tungiasis was perceived as a potentially serious and debilitating condition and that tungi…

Tungiasis-associated morbidity in pigs and dogs in endemic villages of Uganda.

BACKGROUND: Tunga penetrans (Insecta, Siphonaptera, Tungidae) causes severe morbidity among heavily infected humans and animals in Latin America and sub-Saharan Africa. The clinical pathology of tungiasis in animals has never been studied systematically. METHODS: This was a cross-sectional study conducted between January to March 2015, aimed at describing tungiasis-associated clinical pathology in 121 and 20 T. penetrans-infected pigs and dogs, living in nine and five endemic rural villages respectively located in Bugiri District, Busoga, Uganda. RESULTS: The parasite load of infected animal…

Tungiasis in Italy: An imported case of Tunga penetrans and review of the literature.

Tungiasis is an animal and human parasitic disease caused by fleas of the genus Tunga (Siphonaptera, Tungidae), endemic in equatorial and subtropical regions and rarely described in European countries, where clinicians and general pathologists could be not aware of this parasitic disease. To our knowledge, only 75 cases of human tungiasis (not all described in detail) were previously reported in Italy. We described a new case in a 34-year-old Italian flight attendant who developed a granuloma-like, ulcerated nodule in the subungual region of his left 5th toe, partially detaching the nail, abou…

Treatment of Tungiasis with dimeticone: a proof-of-principle study in rural Kenya.

Tungiasis (sand flea disease) is a neglected tropical disease, prevalent in resource-poor communities in South America and sub-Saharan Africa. It is caused by an inflammatory response against penetrated female sand fleas (Tunga penetrans) embedded in the skin of the host. Although associated with debilitating acute and chronic morbidity, there is no proven effective drug treatment. By consequence patients attempt to remove embedded sand fleas with non-sterile sharp instruments, such as safety pins, a procedure that represents a health threat by itself. In this proof-of-principle study we compa…

High prevalence of tungiasis in a poor neighbourhood in Fortaleza, Northeast Brazil.

Tungiasis has been reported to occur in many Latin American, Caribbean and African countries. However, epidemiological data are still very scanty and do not exist at all for Brazil. To fill this gap, a cross-sectional study has been undertaken in a shantytown (favela) in Fortaleza, Ceará State, northeast Brazil. All 327 households of a circumscribed sub-area were visited, and 1185 out of the 1460 household members were thoroughly examined for the presence of tungiasis. Thirty-four% (95% CI: 30.9-36.4) were found to be infected with Tunga penetrans with a significant preponderance of the male s…

A case of imported tungiasis in Scotland initially mimicking verrucae vulgaris.

A case is described of Tunga penetrans infection in the feet of a tourist returning from South America to Scotland. The condition had initially mimicked verrucae vulgaris, but microscopic examination of the lesions together with the travel history allowed the diagnosis of tungiasis to be made. This is the first case reported in Scotland.

Tungiasis in a southern tamandua (Tamandua tetradactyla) from Formosa province, Argentina.

Tunga penetrans causes tungiasis, a parasitic disease of humans and domestic animals. In this work we report the presence of tungiasis in the southern tamandua (Tamandua tetradactyla) from Formosa, Argentina. An individual of southern tamandua was found dead on the roadside and it presented lesions consisted with neosomes located on its four limbs. We identified neosomes as T. penetrans. Records of T. penetrans in wild mammals acquire relevance because monitoring wildlife could help prevent possible outbreaks of tungiasis and other zoonosis.

Case Report: Evidence of Tungiasis in the Amazon Rain Forest of Ecuador.

Tungiasis occurs in tropical and subtropical areas in Central and South America and sub-Saharan Africa and is a Neglected Tropical Disease. We diagnosed three cases of tungiasis in a Dutch family visiting indigenous communities in the Amazon basin of Ecuador. Eight days after returning, they presented papular, pruritic, and painful lesions on the feet, with a whitish halo and a blackish central point with abundant whitish eggs upon extraction. For the first time, we demonstrate that Tunga spp. transmission is present in remote indigenous communities in the Amazon rainforest of Ecuador. It will…

Prevalence and clinical aspects of tungiasis in south-west Nigerian schoolchildren.

Tungiasis is highly prevalent in resource-poor communities in sub-Saharan Africa, but studies among schoolchildren are scanty. We assessed prevalence, parasite load and clinical aspects of tungiasis in schoolchildren in south-western Nigeria. A total of 545 schoolchildren was examined. Of these, 133 (24.4%; 95% CI, 20.9-28.2%) were infested with Tunga penetrans Boys showed a higher prevalence than girls (31% versus 18%, P < 0.0001); in those infested, the median number of lesions was two, with no gender difference (2 boys, interquartile range, 1-4; 2 girls; interquartile range, 1-4; P = 0.34).…

Animal Reservoirs of Zoonotic Tungiasis in Endemic Rural Villages of Uganda.

BACKGROUND: Animal tungiasis is believed to increase the prevalence and parasite burden in humans. Animal reservoirs of Tunga penetrans differ among endemic areas and their role in the epidemiology of tungiasis had never been investigated in Uganda. METHODS AND FINDINGS: To identify the major animal reservoirs of Tunga penetrans and their relative importance in the transmission of tungiasis in Uganda, a cross sectional study was conducted in animal rearing households in 10 endemic villages in Bugiri District. T. penetrans infections were detected in pigs, dogs, goats and a cat. The prevalence…
