Utility of catheterized specimens in reducing overdiagnosis of urinary tract infections in women.

AIMS: To evaluate the utility of catheterized samples in reducing overdiagnosis of UTI based on voided specimens among patients presenting with a range of urinary symptoms. We also aimed to determine variables that may modify the predictive value of the voided midstream urine culture. METHODS: Patient charts were reviewed to identify female patients referred to our voiding dysfunction clinic with a range of complaints warranting urine studies (5/2014-8/2016). Patients with a positive voided urine culture who also had a catheterized urine culture in our system were included. Multiple logistic

Urinary catheterization from benefits to hapless situations and a call for preventive measures.

Catheter-associated complications are common, expensive, and often preventable by reducing unnecessary catheter usage. These complications range from most common nosocomial infection to uncommon conditions such as urethral diverticula and ischemic necrosis of the penis. Often, removal of a single known essential cause may be sufficient to prevent a disease. This review raises issues associated with urinary catheterization and emphasizes on the need of preventive measures a physician should take to reduce disappointing situations. The main objective of this literature review is to intercept or …

Urinary catheterization of male rabbits: a new technique and a review of urogenital anatomy.

Rabbits are widely used as an animal model for urologic research studies in which urinary bladder catheterization is required. However, standard manual retrograde urinary catheterization proved to be difficult to perform on anesthetized male rabbits in a research study, with frequent misplacement of the catheter into the vesicular gland. Attempts to reposition the catheter into the bladder after initial entry into the vesicular gland frequently failed and resulted in exclusion of the animal from the study. We assessed the normal anatomy of the lower urinary tract of male rabbits to determine t…

The effect of urinary bladder catheterization on patient care in an internal medicine department.

INTRODUCTION: Recommendations for urinary catheterization in newly hospitalized patients are inconsistent and unclear. METHODS: The authors studied prospectively consecutive patients who were catheterized in an internal medicine department over a 3-month period, with follow-up for 6 months or until the catheter was removed. Patient records were reviewed to determine if catheterization was definitely not indicated by commonly accepted criteria. After chart review, a category of possibly not indicated was defined as having no demonstrable effect on patient care. RESULTS: There were 17.7% patie…

Intermittent catheterization with hydrophilic catheters as a treatment of chronic neurogenic urinary retention.

AIMS: Neurogenic bladder can be effectively managed with intermittent catheterization (IC) to improve or restore continence, but there is no consensus on which type of catheter is preferred. Hydrophilic catheters were developed to reduce urethral friction, thereby minimizing trauma and sticking, and making them more acceptable to the patient, and easier and safer to use. The objective of this article was to review the literature on the benefits of hydrophilic catheters in patients with neurogenic bladder. METHODS: A large body of experimental and observational evidence, including randomized c

Medical management of urethral and colonic perforation associated with urinary catheterization in a kitten.

A 6-week-old male kitten was evaluated because of stranguria and possible urethral blockage; a urinary catheter placed during general anesthesia penetrated the urethral and colonic walls and entered the colon. Treatment was conservative, with fluids administered i.v., administration of piperacillin, and supportive care. The kitten never became febrile or clinically ill and continued to thrive. There was no development of clinical signs consistent with stricture, diverticulum, or fistula formation. Complications from urethral perforation include infection and urethral stricture. Reconstructive

Standards to prevent complications of urinary catheterization in children: should and should-knots.

PURPOSE: To share standards for urinary catheterization developed at Children's Hospital, Washington, DC, to prevent complications from using feeding tubes, inserting intermittent catheters too far into the bladder, or not inserting Foley catheters far enough into the bladder. POPULATION: Infants and children requiring urinary catheterization. CONCLUSIONS: Using appropriate length urinary catheters and/or inserting catheterizing devices appropriate lengths based on sex, age, and purpose will help prevent knotting of catheters in the bladder and urethral trauma. PRACTICE IMPLICATIONS: Short …

In vivo reduction of bacterial populations in the urinary tract of catheterized sheep by iontophoresis.

PURPOSE: Iontophoresis kills microbes in vitro and, therefore, may be a useful method for eliminating microbial populations associated with catheter-induced urinary tract infections in vivo. MATERIALS AND METHODS: Catheters were modified to deliver current to platinum electrodes in the catheter tip. Female sheep were catheterized with this iontophoretic catheter and left ambulatory. In 5 sheep (experimental group) 400 microA was applied to the catheter and withheld in 4 sheep (control group) for 20 to 21 days. The animals were then sacrificed. During the study, types and concentrations of bac

Intermittent self catheterization for patients with urinary incontinence or difficulty emptying the bladder.

Urinary incontinence, difficulty voiding and recurrent urinary tract infections are common in general practice. In patients with multiple sclerosis, spina bifida, intervertebral disc lesions, spinal injuries or tumours, the symptoms may be associated with a high residual volume of urine owing to a neuropathic bladder. Similar complaints may occur in elderly people or in women with gynaecological problems owing to atonic urinary retention. Provided that a significant residual volume of urine is found on abdominal examination, ultrasound, x-ray or catheterization, both groups of patients may be

The artificial urinary sphincter and urinary catheterization: what every physician should know and do to avoid serious complications.

The artificial urinary sphincter is an effective tool for urinary incontinence that is being seen in more patients because of the increased numbers of artificial sphincters being implanted and increased patient lifespan. If a patient with an artificial urinary sphincter is catheterized without manipulation of the artificial sphincter, serious irreversible complications may result. All of these complications are easily preventable by physician and nurse awareness of the artificial urinary sphincter and knowledge of how it works. Therefore, it is important for every physician to be knowledgeable

Impact of systematic urinary catheterization protocol in delivery room on covert postpartum urinary retention: a before-after study.

We investigated whether implementation of a routine catheterization procedure in labor improves covert postpartum urinary retention (cPUR) rates. We conducted a prospective before-after study. 121 women admitted to delivery room in the observational group, and 82 in the intervention group, in a tertiary university hospital in Southern France were included. All patients in the intervention group were systematically catheterized 2 hours after delivery. cPUR was screened for in both groups. The primary end-point was cPUR (post-void residual bladder volume >150 ml when voided volume is >150 mL). T

Profiles of the bacterial community in short-term indwelling urinary catheters by duration of catheterization and subsequent urinary tract infection.

BACKGROUND: Urinary catheterization, even of short duration, increases the risk of subsequent urinary tract infection (UTI). Whether the bacteria found on the surface of catheters placed for <3 days are associated with UTI risk is unknown. METHODS: We screened the biofilms found on the extraluminal surface of 127 catheters placed for <3 days in women undergoing elective gynecologic surgery, using targeted quantitative polymerase chain reaction and an untargeted 16S rRNA taxonomic screen. RESULTS: Using quantitative polymerase chain reaction, Enterococcus spp were found on virtually all cathe…

A rare case of fatal rectal perforation and sepsis following traumatic urinary catheterization.

This case report details a fatal rectal perforation and sepsis in a comorbid 96-year-old male after traumatic urinary catheterization, highlighting the risks of IDC management in elderly patients with complex health backgrounds. Despite maximal medical therapy, including escalated antibiotics and ICU care, the patient died from septic shock linked to improper catheter insertion by a non-specialist nurse in the community. This case emphasizes the urgent need for better catheterization practices, specialized nursing education, and clear guidelines to prevent such outcomes.

Water or antiseptic for periurethral cleaning before urinary catheterization: a randomized controlled trial.

BACKGROUND: Despite a lack of empiric data to support the practice, traditionally, antiseptic solutions have been used to clean the periurethral area before inserting an indwelling catheter. The purpose of this study was to compare urinary colonization rates of subjects whose periurethral area was cleaned with water versus chlorhexidine 0.1% before the insertion of an indwelling urinary catheter. METHODS: Obstetric patients who required urinary catheterization as part of their routine care were randomly assigned to either the "water" or "chlorhexidine" group with a sealed envelope.
