We examined acid-producing stomach functions, local hemodynamics and motility of the stomach and duodenum in performing organ preservation surgery: combined gastric vagotomy, selective proximal vagotomy and extended selective proximal vagotomy. Changes of hemodynamics and motility in the early postoperative period are related to the technical peculiarities of performing various types of vagotomy. Restoration of the functional state of the stomach and duodenum takes place in the late postoperative period. Results of organo-saving operations in 807 patients (72% men, 28% women) with duodenal ulcers were analyzed. Bilateral truncal vagotomy (with pyloro-, duodenoplasty, gastroenterostomy, gastroduodenostomy, economical stomach resection) were performed in 27.1% patients, selective--in 1.4%, selective proximal--in 65.5%, anterior selective and posterior truncal vagotomy--in 6.0%. Lethality was 1.6%, long-term results were excellent and good in 75.6% cases. Satisfactory results were seen in 8.6% patients, bad results--in 15.8%.

Intraoperative complications occurred in 28 (2.8%) and early postoperative complications in 157 (15.7%) out of 1,000 operations for vagotomy in duodenal ulcer (109 truncal, 64 selective, 445 isolated SPV, and 372 SPV combined with draining operations). Under analysis are results of the surgical treatment of 300 patients with perforating pyloroduodenal ulcers. The correct selection of patients for the operation gives best nearest and especially long-term results of vagotomy with antrum resection. The amount of recurrences after a simple suturing of the ulcer in 164 patients was 50%, while after vagotomy with/without various draining operations it was 3.7%. Postoperative lethality after vagotomy was not great, long-term results being considerably better than after the operations of simple suturing the ulcer.

In the pylorus ligated rat vagotomy has been attempted by chemoneurolysis with alcohol. The authors present data on 406 patients with ulcer disease subjected to vagotomy. Proximal gastric vagotomy without drainage is the operation of choice for uncomplicated duodenal ulcer. There are few contraindications for PGV as uremia, diabetes, hypertension, age over 65 years and a history of splenectomy. Only in cases of severe pyloric stenosis or bleeding ulcer or perforation in the pyloric area, a pyloroplasty should be added. The Wangensteen pyloroplasty is a safe drainage procedure and especially recommended in case of extensive scarring of the pylorus.

To combat gastroplegia following vagotomy the authors recommend to perform open novocain blockade of the solar plexus by means of a microcatheter, inserted in the round ligament of the liver. The application of the blockade enabled the authors to manage the postoperative period without a sound, to reduce fluids and electrolytes loss by the organism and to minimize the use of anesthetics.

Inclusion of vagotomy and pyloroplasty in the surgical treatment of gastroesophageal reflux associated with hiatal hernia has long been controversial. To evaluate the morbidity of vagotomy in the treatment of reflux esophagitis, a retrospective study of 311 patients treated by the Hill posterior gastropexy technique of hiatal hernia repair was tabulated. Seventy patients with peptic ulcers (55 duodenal and 15 gastric) were treated by truncal vagotomy and doulbe pyloroplasty during the past four years. Clinical and experimental data as presented lead us to believe that transecting the pylorus twice produces an incontinent pyloric sphincter and a larger gastric outlet than is found in other methods of pyloroplasty. Two hundred sixty-five patients who underwent vagotomy and pyloroplasty for duodenal ulcer disease were observed postoperatively, 220 for two to ten years, with an average follow-up of five years.
