Intratesticular varicocele (ITV) is a relatively rare condition. Currently, there is no domestic literature available on this topic. This paper presents an overview of the epidemiology, diagnosis, differential diagnosis, impact on male reproductive health, and treatment of ITV with a review of recent foreign literature, aiming to gain a deeper insight into this condition.

We describe herein a rare case of a varicocele complicating spontaneous arteriovenous fistula. A 40-year-old man was referred to our hospital in November 2006, complaining of a non-tender mass in the left scrotum at the age of 15 and thereafter. Antegrade scrotal sclerotherapy is a simple and easy technique for the treatment of varicocele. The success rate varies between 87% and 95%. The initial reflux grade and the number of collateral vessels of the spermatic vein are the most important factors to predict the outcome of the technique. The postoperative complication rate is about 7% and the common ones are scrotal hematoma and epididymo-orchitis of slight severity. Testicular athrophy is a rare event (0.6%). This technique offers a considerable cost reduction compared to other therapeutic options currently available for varicocele.

OBJECTIVE: To evaluate whether there is an association between scrotal skin temperature and impaired semen quality in patients with varicocele testis.
SUBJECTS AND METHODS: Two groups of men (all newly enlisted soldiers), 39 with an asymptomatic varicocele testis and 38 normal control subjects, were compared for semen quality and scrotal skin temperature.
RESULTS: The semen quality in the men with varicocele was significantly lower than that in the control subjects; however, there was no significant difference in scrotal skin temperature between the groups.

In 27 patients with clinically identified idiopathic left varicocele, a retrograde venography and a combined pressure measurement in the vena cava, vena renalis and vena spermatica was performed. During Valsalva maneuver the pressure values increased. We could not confirm increased pressure values in the left renal vein dependent on the patient's position. A discussion of the present role of varicocele in the management of the subfertile couple is presented, with emphasis on detection techniques and the increasing awareness of the presence of bilateral varicocele. The indications and techniques of surgical management are discussed, with particular emphasis on the use of the operating microscope to dissect the spermatic cord at the level of the external inguinal ring under local anesthesia on an outpatient basis with significant cost savings to the patient.

Twenty-four boys age 11-17 with moderate to large sized varicocele were studied. The duration of varicocele ranged from 11 to 20 months prior to surgery. Histological and ultrastructural studies of testicular specimens demonstrated bilateral abnormalities and the detrimental effect of varicocele on the future fertility of these young patients. These findings strongly support the necessity of an early surgical treatment of varicocele.

Author in the histologically normal testis of a patient aged 31 suffering of varicocele by electron microscopy revealed the presence of polynuclear spermatids and spermiums, i.e. disturbance of the spermiogenesis. The cause of this phenomenon has to be cleared by further studies.

OBJECTIVE: To determine the effect of experimental left varicocele on the anatomy of the veins serving the rat testis and to compare that anatomy to known patterns of vascular drainage from the human testis with and without varicocele.

Rapid sequence scintigraphy was used to study testicular arterial perfusion and venous stasis in 53 patients with varicocele-associated infertility, 17 with idiopathic testicular failure and 9 treated for varicocele.
