Vasculitis, defined as an inflammation of blood vessels resulting in ischemic tissue lesions, is a feature of many different diseases and symptoms in childhood. A uniform classification based on characteristic clinical symptoms and histopathological signs is as yet not possible. Most vasculitic syndromes are rarely seen in childhood. This review deals with the more frequently encountered vasculitic syndromes such as polyarteritis nodosa, Kawasaki disease and Henoch-Schönlein purpura. A survey of clinical and histopathological findings as well as recent insights in the pathogenesis and pathophy…

Panarteritis nodosa, Wegener's granulomatosis and idiopathic necrotizing glomerulonephritis, regarded by some as a vasculitis restricted to the glomerulus, are those forms of primary vasculitis most often encountered in renal biopsy. The histopathologic differential diagnosis can be difficult and has to rely on the finding of vascular inflammation in preglomerular vessels and of granulomata. In a majority of renal biopsies only a necrotizing and crescentic glomerulonephritis without inflammation of arteries or arterioles can be seen. Also the clinical manifestation of these three diseases can …

1. Vasculitis syndromes in children present with special features. The hypersensitivity vasculitides predominate with Henoch-Schoenlein purpura as a typical representative. Within the polyarteritis-nodosa-group we can distinguish an infantile form with involvement of coronary vessels. This disease resembles Kawasaki's syndrome. 2. The different vasculitis syndromes show a great variety in their clinical presentation; overlapping syndromes are possible. Diagnosis, however, is mostly based on clinical features, supported by laboratory data. In doubtful cases histologic and immunohistologic findi…

Pulmonary vasculitis occurs most commonly in the collagen vascular diseases and in granulomatous pulmonary disease. In the collagen vascular group, vasculitis causes diffuse interstitial inflammation and subsequent fibrosis, resulting in interstitial radiographic patterns, especially in the lower lung fields. Vasculitis accompanied by granulomatosis typically produces focal inflammation and is, therefore, manifested as nodules and masses. The more typical collagen vascular diseases include rheumatoid arthritis, systemic lupus erythematosus, systemic sclerosis, and dermatomyositis. The most com…

Thirty-eight patients with essential mixed cryoglobulinemia (EMC: 16 type II and 22 type III) were evaluated. Almost all patients had clinic … lomerulonephritis in one case. Skin biopsy performed in 11 patients showed typical leukocytoclastic vasculitis as the most common finding. A picture consistent with overlapping vasculitis was found in 2 cases (one type II and one type III) and thrombus-like deposits were observed in 3 cases (2 type II and one type III). These thrombotic lesions, not associat …

Serum cryoglobulin levels were estimated in thirty-one patients with cutaneous vasculitis, twenty-nine patients with other skin disorders and forty-seven normal subjects. Eighteen of the normal sera contained mixed cryoglobulins—the upper limit of the normal range being 80 μg/ml. Twelve of the vasculitis patients had serum cryoglobulin levels above the normal range. All but one of these cryoglobulins were mixed cryoglobulins. A low serum C3 level was found in only one patient. The possible role of mixed cryoglobulins in the pathogenesis of vasculitis is discussed.

Seven of forty-two patients with livedoid vasculitis whose cases were reviewed at the Mayo Clinic were found to have abdominal aortic disease, in the form of aortic calcification and atherosclerosis or aneurysm. None of these patients had any associated systemic mesenchymal disease (lupus erythematosus, periarteritis nodosa, rheumatoid arthritis, Sjögren's syndrome, or carcinomatous diseases). Three patients received graft replacements for aneurysm or vascular occlusive disease. Removal of the aneurysm in one case was associated with resolution of the livedoid vasculitis. Abdominal aortic chan…

BACKGROUND: Immunoglobulin A vasculitis with nephritis (IgAVN) is the most serious complication affecting long-term prognosis. Understanding the risk factors and markers for the development of IgAVN is essential. The aim of this study is to identify IgAVN-associated factors and to evaluate the usability of Pediatric Vasculitis Activity Score (PVAS) at diagnosis as an early marker for the development of IgAVN.
METHODS: We conducted a retrospective case-control study of 314 patients divided into two groups: those with nephritis (IgAVN) and without nephritis (non-IgAVN). The groups were compared…

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are rheumatic diseases characterized by small-to-medium vessel vasculitis. Three different entities can be distinguished: granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA) and eosinophilic granulomatosis with polyangiitis (EGPA). While lung and renal involvement are typical manifestations of both GPA and MPA, EGPA usually shows paranasal sinus and lung involvement as well as a history of bronchial asthma. Furthermore, EGPA is frequently associated with cardiac disease and peripheral neuropathy. Cyclo…

Patients presenting with pancytopenia and a painful purpuric rash should be evaluated for levamisole-induced vasculitis and counseled about cocaine cessation as continued exposure can lead to permanent deformity of the involved areas.
