Vasodilator therapy for primary pulmonary hypertension.

Primary pulmonary hypertension is an uncommon but serious disease that often results in debilitating symptoms and early death. One approach to treatment has been to attempt reduction of pulmonary arterial pressure and right ventricular afterload by using vasodilator agents to decrease pulmonary arteriolar resistance. Use of a variety of vasodilators has yielded only limited and infrequent success. Although an occasional patient may respond to vasodilator therapy with improved pulmonary hemodynamics and symptomatic status, most patients do not. Reasons for lack of success include the presence o

[Vasodilator therapy in ischemic heart disease].

A comparative study of venodilating and veno-arterial dilating drugs was conducted for clinical patients and for experimental animals. The subjects were 21 patients with ischemic heart disease associated with left ventricular failure. Fourteen patients received venodilators alone (Group I), and seven were administrated venoarterial dilators alone (Group II). At the time of the maximum effect, the two groups showed similar improvement. In Group I, the left ventricular filling pressure (LVFP) was reduced from 24 to 16 mmHg and the cardiac index (CI) was increased from 2.5 to 2.8 l/min/m2. In Gro

Diagnostic Criteria of Flow-Mediated Vasodilation for Normal Endothelial Function and Nitroglycerin-Induced Vasodilation for Normal Vascular Smooth Muscle Function of the Brachial Artery.

Background Diagnostic criteria of flow-mediated vasodilation (FMD), an index of endothelial function, and nitroglycerin-induced vasodilation (NID), an index of vascular smooth muscle function, of the brachial artery have not been established. The purpose of this study was to propose diagnostic criteria of FMD and NID for normal endothelial function and normal vascular smooth muscle function. Methods and Results We investigated the cutoff values of FMD and NID in subjects with (risk group) and those without cardiovascular risk factors or cardiovascular diseases (no-risk group) in 7277 Japanese …

The heterogeneity of dopamine-mediated vasodilation in human intrarenal arteries.

INTRODUCTION: Adequate renal perfusion is critical for maintaining kidney function, and its impairment contributes significantly to acute ki … e segment-specific DA responses and molecular basis. RESULTS: DA (1 nM-10 µM) consistently induces vasodilation across all human intrarenal arteries, challenging the conventional notion that "renal-dose DA" causes vasoconstriction. IA and AA segments exhibit more pronounced vasodilation than ILA, mediated by dopamine receptor D1 (DRD1)-dependent protein kinas …

S-nitrosoglutathione preserves vasodilation and attenuates myocardial ischemia-reperfusion injury.

Ischemia/reperfusion (I/R) injury exacerbates myocardial damage following acute myocardial infarction, underscoring the need for effective cardioprotective therapies. S-nitrosoglutathione (GSNO), an endogenous S-nitrosothiol, functions as a nitric oxide (NO) reservoir with vasodilatory and cytoprotective properties; however, its therapeutic potential in myocardial I/R injury remains incompletely characterized. We hypothesized that GSNO protects against I/R-induced cardiac injury by improving coronary vasodilation. The effects of GSNO were evaluated in an isolated Wistar rat heart model of regi…

Klebsiella pneumoniae inhibits vasodilation through capsule and T6SS-dependent pathways.

Vasodilation is a defence mechanism during inflammation and infection that is regulated by nitric oxide produced by endothelial nitric oxide synthase (eNOS) and the generation of endothelium-dependent hyperpolarization. Several viral infections have been shown to alter vascular biology, but effects of bacterial pathogens are unclear. Here using an ex vivo blood vessel model and human primary endothelial cells, we show that Klebsiella pneumoniae, a prevalent bloodstream pathogen, inhibits vasodilation pathways. The type VI secretion system (T6SS) effector VgrG4 activates the mitochondrial recep…

Potassium as an electro-metabolic signal for local coronary vasodilation.

This study tested the hypothesis that K+ serves as an in vivo signal coupling coronary blood flow with the oxidative requirements of the myocardium. Experiments were performed in swine in which coronary parameters and arterial and coronary venous [K+] were measured under baseline conditions, during exogenous administration of K+ (1-5 mM; n = 4), during increases in myocardial oxygen consumption (MVO2) to dobutamine (n = 7) and exercise (n = 6), alterations in coronary perfusion pressure (CPP; n = 8), and systemic hypoxemia (PaO2 to ~ 30 mmHg; n = 7). Exogenous intracoronary K+ increased blood

Endothelial TRPV4 channel mediates the vasodilation induced by Tanshinone IIA.

Tanshinone IIA (TSA), the main lipo-soluble component from the dried rhizome of Salvia miltiorrhiza, has been shown to induce vasodilation. However, the underlying mechanisms remains unclear. This study aimed to investigate the effect of TSA on the vasodilation of small resistant arteries ex vivo. Vascular myography revealed that endothelial denudation reduced significantly the vasodilatory effect of TSA. Blocking transient receptor potential vanilloid 4 (TRPV4) channels prevented TSA-induced vasodilation. Whole-cell patch-clamp analysis revealed that the current passing through TRPV4 channels…

Pulmonary vasodilation during cardiopulmonary resuscitation - A randomized, controlled porcine study.

BACKGROUND: During resuscitation pulmonary artery pressure (PAP) increases. This reduces left ventricular filling, leading to decreased blood flow. Inhaled nitric oxide (iNO) produces selective pulmonary vasodilation. We hypothesized that iNO would lower PAP during resuscitation resulting in increased survival. METHODS: 30 pigs (40 kg) were subjected to cardiac arrest for 9.5 min after myocardial ischemia induced by coronary artery occlusion of the left anterior descending artery and ventricular fibrillation. During resuscitation, the pigs were randomized to 40 ppm iNO or placebo. The primary…

Heating-induced peripheral limb microvascular vasodilation reduces arterial wave reflection.

Arterial wave reflection augments cardiac afterload increasing myocardial demands. Mathematical models and comparative physiology suggest that the lower limbs are the primary source of reflected waves; however, in vivo human evidence corroborating these observations is lacking. This study was designed to determine whether the vasculature of the lower or upper limbs contributes more to wave reflection. We hypothesized that lower limb heating will result in larger reductions in central wave reflection compared with upper limb heating due to local vasodilation of a larger microvascular bed. Fifte…
