Repair of transposition of the great arteries, ventricular septal defect and left ventricular outflow tract obstruction.

OBJECTIVES: This study was undertaken to compare the outcomes of the Lecompte procedure and Rastelli repair in the transposition of the great arteries (TGA) with ventricular septal defect (VSD) and left ventricular outflow tract obstruction (LVOTO) and to determine the risk factors associated with unfavorable events. METHODS: Over a 12-year period (April 1990-October 2002), 35 patients underwent complete repair for TGA, VSD, and LVOTO. Twenty-five patients (71%) underwent the Lecompte modification, and mean age and weight were 23.4+/-18.2 months and 10.2+/-3.0 kg. Ten patients (29%) underwent

Fibrous obstruction within the left ventricular outflow tract associated with ventricular septal defect: a pathologic study.

OBJECTIVES: We examined the nature of ridges within the left ventricular outflow tract associated with ventricular septal defects that might be found by echocardiography. BACKGROUND: Echocardiography displays even small ridges well. Surgical removal of such ridges at the time of defect closure is recommended. METHODS: We examined 37 heart specimens with ventricular septal defects with a ridge, noting its nature and relation to the defect and adjacent valves. We excluded left ventricular outflow tract obstruction associated with complex lesions. RESULTS: Defects were perimembranous in 25 spe

Left ventricular outflow tract obstruction due to tricuspid valve prolapse through a high ventricular septal defect.

A rare case is described of fixed subaortic obstruction due to a prolapsed redundant fibrous septal leaflet of the tricuspid valve, herniated through a membranous ventricular septal defect in a boy aged 11 years. The type of obstruction was not suspected before surgery and there was no shunt through the septal defect. Left ventriculography, was attempted and abandoned due to dangerous arrhythmia. Echocardiogram showed definite fixed subvalvular left ventricular outflow obstruction. Corrective surgery was carried out through aortotomy and right ventriculotomy. Ruptured chordae were resected and

Modified Nikaidoh procedure for transposition of great arteries, ventricular septal defect and left ventricular outflow tract obstruction.

The surgical management of transposition of the great arteries with ventricular septal defect and left ventricle outflow tract obstruction is a real challenge in congenital heart surgery. The Rastelli operation has been used for many years with satisfactory early and late results. A newer operation described by Nikaidoh seems to take anatomy more into account and has been performed with promising outcomes. In this report, we present a patient with transposition of the great arteries, ventricular septal defect and left ventricular outflow tract obstruction who was successfully treated with a mo

Unmasking Latent Left Ventricular Outflow Tract Obstruction in Hypertrophic Cardiomyopathy: Invasive and Noninvasive Assessment.

BACKGROUND: Dynamic left ventricular outflow tract (LVOT) obstruction in hypertrophic cardiomyopathy may be missed on resting echocardiography, and it requires provocation for detection. CASE SUMMARY: A 41-year-old man with previously classified nonobstructive hypertrophic cardiomyopathy presented with recurrent syncope. Serial echocardiography had demonstrated no significant dynamic LVOT obstruction. During reassessment, repeated Doppler interrogation with Valsalva maneuver revealed variable pressure gradients ranging from 25 to 46 mm Hg. Exercise stress echocardiography further unmasked sev

Left Ventricular Sarcoma Causing Dynamic Outflow Tract Obstruction and Cardiogenic Shock: A Case Report.

BACKGROUND: Primary cardiac sarcomas are rare and aggressive tumors that often present with nonspecific symptoms, typically diagnosed at advanced stages. Obstruction of the left ventricular outflow tract (LVOT) due to these tumors is an exceptionally rare and life-threatening complication. CASE PRESENTATION: We describe the case of a previously healthy 33-year-old male who presented with progressive exertional dyspnea, orthopnea, and hemoptysis. Upon admission, he quickly developed hemodynamic instability and respiratory failure. Point-of-care transthoracic echocardiography revealed a large i

[Left Ventricular Outflow Tract Obstruction After Transcatheter Aortic Valve Replacement due to Pannus Formation].

We experienced a case of surgical aortic valve re-replacement due to structural valve deterioration caused by pannus formation 4 years after transcatheter aortic valve replacement( TAVR). The patient underwent surgery because the mean transvalvular pressure gradient increased to 48 mmHg on echocardiography. Contrast-enhanced computed tomography (CT) was useful for predicting the site of adhesion to surrounding tissue preoperatively and exploring the presence of the pannus. Intraoperative findings showed the TAVR valve was covered with neointima except around the origins of the left and right c

Late Outcomes of Konno-Rastan and Modified Konno for Left Ventricular Outflow Tract Obstruction.

BACKGROUND: While early outcomes of the Konno-Rastan and modified Konno procedures are known, long-term outcomes remain undetermined. Our objective was to examine long-term clinical and echocardiographic outcomes. METHODS: Retrospective review was performed of patients undergoing Konno-Rastan and modified Konno for complex left ventricular outflow tract (LVOT) obstruction from January 1980 to January 2021. The Ross-Konno procedure was excluded due to its often limited ventriculotomy. RESULTS: A total of 92 patients were identified: 73 Konno-Rastan and 19 modified Konno. The modified Konno gr

A case of surgically resected cardiac rhabdomyoma with progressive left ventricular outflow tract obstruction.

Cardiac rhabdomyoma is the most prevalent cardiac tumors in the pediatric population, in close association with tuberous sclerosis complex. It is usually detected antenatally or postnatally by echocardiography. Clinical presentations depend greatly on the size and position of the tumor mass. Interestingly, rhabdomyoma has a propensity to regress spontaneously and is not usually operated upon, unless the patient becomes hemodynamically compromised. Herein, we report an unusual case of surgically treated cardiac rhabdomyoma in a baby boy presented at birth with a progressive enlarging intraventr

[Transient left ventricular apical ballooning with dynamic outflow tract obstruction complicated by persistent hypotension].

The pathogenesis of tako-tsubo syndrome remains, nowadays, unclear: it is likely that the relevance of the different proposed mechanisms differs among specific clinical contexts. In this paper, we describe the case of a 61-year-old woman admitted to the intensive cardiac care unit for pre-syncope and persistent hypotension. Clinical and echocardiographic data were suggestive of tako-tsubo syndrome with severe dynamic left ventricular outflow tract (LVOT) obstruction and severe mitral regurgitation. Coronary angiography was normal. Inotropic agents were not administered, because absolutely cont

Selective management strategy of interrupted aortic arch mitigates left ventricular outflow tract obstruction risk.

BACKGROUND: Left ventricular outflow tract obstruction (LVOTO) is an important problem after interrupted aortic arch (IAA) repair, especially when early reoperation is required during infancy. Several anatomic factors have been identified that increase LVOTO risk; surgical strategies such as concomitant resection of the conal septum or left ventricular outflow tract (LVOT) bypass (single-stage Yasui operation, or staged Norwood procedure, followed by the Rastelli procedure) have been proposed for such patients. METHODS: From 2002 to 2013, a total of 77 neonates underwent IAA repair. Based on

Left ventricular outflow tract obstruction: should cardiac screening be offered to first-degree relatives?

OBJECTIVES: To determine whether offering cardiac screening to relatives of patients with left ventricular outflow tract obstructions (LVOTOs) would be justified. BACKGROUND: LVOTOs have been recognised as a group of congenital heart diseases with 'high heritability'. One of the LVOTOs, the bicuspid aortic valve, is often asymptomatic, but has become known to be associated with sudden, unexpected cardiac death. However, the need for cardiac screening of first-degree relatives of patients with LVOTO has not been determined owing to the lack of studies in well-defined cohorts of consecutive pat

[Konno procedure for congenital aortic stenosis associated with complex left ventricular outflow tract obstruction].

Two successful cases of Konno procedure for congenital aortic stenosis and left ventricular outflow tract obstruction (LVOTO) were reported herein. A 3-year-old child previously underwent definitive repair of complete atrioventricular septal defect. Follow-up echocardiography revealed progression of valvular aortic stenosis and subaortic tunnel stenosis. Second patient was a 30-year-old male with congenital aortic stenosis, severe LVOTO and funnel chest. Both patients underwent Konno procedure, and their postoperative courses were uneventful. The Konno procedure is effective and stenotic lesio

Left ventricular outflow tract obstruction and sudden death risk in patients with hypertrophic cardiomyopathy.

AIMS: Left ventricular outflow tract obstruction (LVOTO) is associated with reduced survival in patients with hypertrophic cardiomyopathy (HCM). The influence of LVOTO on survival from SD in relation to other recognized clinical risk markers is unknown. METHODS AND RESULTS: A total of 917 patients with HCM (554 males, 43+/-15 years) were studied; 288 (31.4%) had LVOTO at rest (> or =30 mmHg). During follow-up [median 61 (30;99) months], 54 (5.9%) patients died suddenly (SD), survived ventricular fibrillation, or had an appropriate ICD discharge; 25 (2.7%) died from heart failure or were trans

Reversible steroid-induced hypertrophic cardiomyopathy with left ventricular outflow tract obstruction in two newborns.

Dexamethasone is now frequently used in the treatment of bronchopulmonary dysplasia. We report on two premature babies receiving dexamethasone who developed symptomatic myocardial hypertrophy and left ventricular outflow tract obstruction, as documented by M-mode echocardiography and Doppler studies. A normal heart was recorded on echocardiography before and after dexamethasone treatment. The patients had no known risk factors associated with the development of obstructive hypertrophic cardiomyopathy.

Left ventricular outflow tract obstruction in TGA: treatment with LV-to-PA valved conduit.

Progressive or recurrent left ventricular outflow tract obstruction after a previous Mustard or Senning operation represents a rare but challenging problem. The obstruction can be resected in some patients, but abnormal attachment of the mitral valve or a long fibromuscular tunnel represents a difficult surgical problem. Between 1979 and 1993, we encountered this type of left ventricular outflow tract obstruction in 10 patients, 4 to 13 years after the atrial repair. They ranged in age from 5 to 15 years (mean, 8.8 years) and weighed between 11.5 and 47 kg (mean, 25.3 kg). Operations were perf
