Fourfold increase in fragmentation from road expansion in Siberian intact forests since 2000.

Road development is a major driver of forest degradation worldwide, accelerating habitat fragmentation, biodiversity loss and carbon emissions. Although these consequences have been documented in tropical forests, the spatiotemporal dynamics and ecological impacts of road expansion in boreal intact forest landscapes (IFLs) remain poorly understood, particularly for the central-eastern Siberian IFLs, an ~1.71 million km2 wilderness and carbon sink. To address this gap, we analysed 20,480 Landsat satellite images from 2000 to 2023, and found a 1.6-fold increase in road area and length across thi…

Is it Time to Reconsider the Fluid of Choice in Hypothermia? the Case for Lactated Ringer's Solution.

Current Wilderness Medical Society guidelines for accidental hypothermia recommend 0.9% normal saline (NS) over lactated Ringer's (LR) for fluid resuscitation, citing concerns that a hypothermic liver cannot metabolize lactate. The purpose of this article is to challenge this theoretical premise. We highlight a critical biochemical difference: lactic acid with sodium lactate, the salt found in LR. We propose that the lactate in LR does not contribute to acidosis as it lacks a dissociative proton. In contrast, large-volume NS resuscitation carries the better-documented risk of hyperchloremic no…

Climate change and wildland firefighter health and safety.

The author examines how climate change is impacting wildland firefighters. Climate change has made wildland fires more frequent and more intense. The increase in frequency and intensity of fires has pushed the number of fatalities and injuries higher in recent decades. The most common hazards on fires follow the trend of fire in general in that these hazards become more frequent and intense. Burnovers, heat exhaustion, tree hazards, and many other common fire hazards are more likely. The fire suppression agencies are making every effort to improve health and safety on fires by improving commun

Urban and rural risks of Lyme disease in the Scottish Highlands.

BACKGROUND: This paper investigates the pattern of Lyme disease testing and infection within the Highland region of Scotland. METHODS: Data from all Highland samples tested during 2004-2006 were analysed according to result and patient's residence in relation to the eight fold Scottish Executive's urban/rural classification, and distance from woodland. RESULTS: In total, 1602 patients were tested for Lyme disease, 0.71% of the Highland population. From these, 104 (6.5%) were seropositive. There were more patients tested, and seropositive patients from rural than urban locations, 1113 vs 489,

Chest Ultrasonography in Modern Day Extreme Settings: From Military Setting and Natural Disasters to Space Flights and Extreme Sports.

Chest ultrasonography (CU) is a noninvasive imaging technique able to provide an immediate diagnosis of the underlying aetiology of acute re … nd," and "lung sonography," in different combinations with "extreme sport," "extreme environment," "wilderness," "catastrophe," and "extreme conditions." This manuscript reports the most relevant usages of CU in the extreme setting as well as technological improvements and current limitations. CU application in the extreme setting is further encouraged here.

You Can Observe a Lot by Watching: Hughlings Jackson's Underappreciated and Prescient Ideas about Brain Control of Movement.

John Hughlings Jackson, the 19th-century British neurologist, first described what are today called Jacksonian seizures. He is generally associated with somatotopy, the idea that neighboring brain regions control neighboring body parts, as later represented pictorially in Wilder Penfield's "homunculus," or little man in the brain. Jackson's own views, however, were quite different, though this is seldom appreciated. In an 1870 article, Jackson advanced the hypotheses that each region of the cerebrum controls movements of multiple body parts, but to different degrees, and that the "march" of mo

Penfield's homunculus and other grotesque creatures from the Land of If.

The neurosurgeon Wilder Graves Penfield (1891-1976) helped to develop a surgical treatment for epilepsy and used his results to investigate the functional organization of the brain. He was instrumental in founding the Montreal Neurological Institute at McGill University, which he directed from 1934 to 1960. There he studied, with his collaborators, the effects of stimulation and surgical ablation on different parts of the brain in order to localize their somatosensory functions. To visualize the results of this research, Hortense Pauline Cantlie drew images of a homunculus whose proportions re

Management of trauma in remote and austere settings: civilian implications of military-derived prolonged field care principles - A narrative review.

Prolonged Field Care (PFC) refers to the delivery of advanced trauma care in austere environments when evacuation to definitive treatment is significantly delayed. Although originally developed within military medicine, its principles are increasingly relevant to civilian trauma systems operating in remote, rural, wilderness, disaster, and resource-limited settings. This narrative review provides an updated overview of PFC concepts and their potential applicability to civilian trauma care, focusing on key domains such as airway management, haemorrhage control, crush syndrome, analgesia and sed…

A Tribute to Dr Robert J. White.

Robert J. White is probably best known as the first neurosurgeon to perform successful "cephalic exchange" on monkeys in 1971. However, he was also a pioneer in the field of neurosurgery and contributed tremendously to the field of neuroanesthesia and bioethics. White received medical training at the University of Minnesota, Harvard University, Peter Bent Brigham Hospital, and Mayo Clinic before becoming the first Chief of Neurosurgery at Metrohealth Hospital in Cleveland, Ohio. He made significant strides in the field of spinal cord cooling and hypothermia. White and his team was also the fir

Eye problems on expeditions.

BACKGROUND: Visual loss in the wilderness setting is at best disabling and at worst potentially fatal. However many physicians have a poor knowledge of ophthalmology and the basic skills that could be applied in situations away from definitive care. METHOD: This paper is intended for physicians, interested non-medical people and expedition operators as a practical guide to the treatment and prevention of eye problems on expeditions. RESULTS: Some of the eye conditions described in this paper are unique to the high altitude setting, such as high altitude retinopathy and some could happen in a…

Human Responses to 5 Heated Hypothermia Wrap Systems in a Cold Environment.

National intercomparison of environmental gamma radiation dose rate in China based on ice surface cosmic ray response measurements.

This study reports the first national intercomparison of environmental gamma radiation dose rate in China where each device's response to the ubiquitous cosmic rays was measured on top of a frozen lake ice surface. A total of 78 ambient gamma dose rate instruments from 38 institutions were tested at four typical sites, including a low-terrestrial-background site on the ice surface of the Bosten Lake to measure the detector-specific cosmic ray responses, and wilderness, road, indoor sites nearby. The measured dose rates on the ice surface show significant variations for different detector types…
