Evaluating progress in reducing workplace violence: trends in Washington State workers' compensation claims rates, 1997-2007.

OBJECTIVES: This study reports trends in the pattern of injuries related to workplace violence over the period 1997-2007. It tracks occupations and industries at elevated risk of workplace violence with a special focus on the persistently high claims rates among healthcare and social assistance workers. METHODS: Industry and occupational incidence rates were calculated using workers' compensation and employment security data from Washington State. RESULTS: Violence-related claims rates among certain Healthcare and Social Assistance industries remained particularly high. Incidents where worke…

Work-related carpal tunnel syndrome in Washington State workers' compensation: temporal trends, clinical practices, and disability.

BACKGROUND: Work-related carpal tunnel syndrome (CTS) is a leading cause of disability. There is a need for information about temporal trends, clinical practices, and treatment outcomes. METHODS: A population based, retrospective cohort study of Washington State workers' compensation claims for CTS was initiated focusing on claims filed during 1990-1994, followed through 2000 (n = 16,710). RESULTS: Half of the claims were filed for conditions other than CTS, but were eventually identified to be or include CTS. The first CTS diagnosis occurred more than 3 months after claim filing in 20% of c…

Impact of case manager training on worksite accommodations in workers' compensation claimants with upper extremity disorders.

Management of the return-to-work process in claimants with work-related upper extremity disorders often poses challenges to the health care provider, claimant, and employer. Modifying workplace ergonomic risk factors as a component of the workplace accommodation process may improve return-to-work outcomes by reducing recurrent pain and discomfort. The present study is a case-control evaluation of the effects of a 2-day training program for nurse case managers that was designed to facilitate the implementation of workplace accommodations within a workers' compensation health care delivery syste…

Improving the quality of workers' compensation health care delivery: the Washington State Occupational Health Services Project.

This article has summarized research and policy activities undertaken in Washington State over the past several years to identify the key problems that result in poor quality and excessive disability among injured workers, and the types of system and delivery changes that could best address these problems in order to improve the quality of occupational health care provided through the workers' compensation system. Our investigations have consistently pointed to the lack of coordination and integration of occupational health services as having major adverse effects on quality and health outcome…

Injuries in home health care workers: an analysis of occupational morbidity from a state compensation database.

BACKGROUND: Home health services represent one of the fastest-growing segments of the US economy. Home health care workers (HHCWs) might be expected to have a high incidence and increased severity of injury because of inherent difficulty in control over their work environment, and the limited amount of research on injuries in home health care appears to support this hypothesis. METHODS: Using data on workers' compensation claims for 1995-1996 from a large state database, we calculated the incidence, frequency, and types of injuries occurring in this working population. Comparison data were dr…

Occurrence Of Long COVID In Healthcare Workers At A University Hospital And Challenges In Insurance And Compensation Evaluation.

INTRODUCTION: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), developed in 2019, led to the outbreak of the COVID-19 pandemic state, which have significantly impacted on healthcare systems worldwide. During the pandemic state, in addition to all the problems related to high mortality and morbidity during the active phase of the disease, it had to deal with a condition known as "Long-COVID". Long-COVID includes both persistent symptomatic forms, where signs and symptoms last between 4 and 12 weeks, and post-COVID syndrome, where symptoms persist beyond 12 weeks. The evaluation sys

Primary care involvement and outcomes of care in patients with a workers' compensation claim for back pain.

STUDY DESIGN: A retrospective case series of primary care patients receiving Workers' Compensation (WC) for back pain. OBJECTIVE: To describe the extent of primary care provider (PCP) involvement in work-related low back pain (WRLBP), and whether PCP involvement is a significant determinant of patterns of care or outcomes. SUMMARY OF BACKGROUND DATA: In general, WRLBP has inferior outcomes compared with nonoccupational LBP. Although it has been suggested that better outcomes are achieved when care for WRLBP involves specialists such as occupational medicine physicians rather than a PCP, limi…

Asbestos-related lung cancer: Clinical characteristics and survival outcomes in an Australian cohort seeking workers compensation.

BACKGROUND AND OBJECTIVES: Due to difficulties in identifying sufficient-sized cohorts there remains uncertainty about prognostic and clinical differences that may be unique to asbestos-related lung cancer (ARLC). In this study, we use the Helsinki Criteria to define a group of ex-workers with lung cancer attributable to asbestos exposure and investigate differences that may exist. METHODS: A total of 529 patients seeking workers' compensation for their lung cancer were assigned to either ARLC or the non-ARLC based on parameters defined in the Helsinki Criteria. Clinical and survival details …

Investigating Physical Violence Against Classroom and Other School Personnel Using Ohio Workers' Compensation Data: 2001-2012.

This study uses workers' compensation data to explore the extent, severity, and context of violence-related injuries sustained by classroom (teachers and aides) and other personnel (e.g., administrators, education support specialists, security, custodial and maintenance workers, food workers) in Ohio's K-12 urban public schools. The Ohio Bureau of Workers' Compensation provided access to claims filed by workers from the state's nine urban school districts from January 01, 2001 to December 31, 2012 (N = 19,508). Injury trends were explored with descriptive statistics and logistic regression ana…

Ergonomic and psychosocial factors affect daily function in workers' compensation claimants with persistent upper extremity disorders.

Pain and other symptoms associated with work-related upper extremity disorders (WRUEDs) can lead to significant distress, lost function, and disability. Identifying factors associated with decreased upper extremity function may lead to the development of more effective interventions. In this study, participants were 165 government employees (127 female, 38 male) with an accepted workers' compensation claim (< 90 days from claim filing) for a WRUED who were unable to perform their normal work. Participants completed baseline measures of upper extremity functional limitation, symptoms, general h…

Incidence and risk of work-related fracture injuries: experience of a state-managed workers' compensation system.

Incidence rates of occupational fractures at various anatomic sites and associated risk factors have not been well documented. We identified 3490 work-related fractures from a West Virginia Workers Compensation database that occurred between July 1, 1994, and June 30, 1995. The annual incidence rate was 55 per 10,000 workers, which is substantially higher than the work-related fracture rates reported previously. The incidence rate of fracture was highest in the agricultural sector, followed by the mining, construction, and manufacturing sectors (202.0, 165.2, 116.7, and 88.0 per 10,000 workers

Computed tomography, electrodiagnostic and clinical findings in chronic workers' compensation patients with back and leg pain.

One hundred patients with complaints of low-back pain and leg pain, consistent with a diagnosis of sciatica, were evaluated. All patients had complaints for longer than 6 months and had recently undergone electrodiagnostic testing and computed tomography (CT). Correlation was made between symptoms, straight leg raising, clinical neurological deficits, electrodiagnostic and CT findings. The radiation of pain above or below the knee and pain on straight leg raising did not show a high correlation with each other or with neurological deficits or CT findings. Electrodiagnostic studies often define

Time to Service and Its Relationship with Outcomes in Workers with Compensated Musculoskeletal Conditions: A Scoping Review.

PURPOSE: A comprehensive review of the literature on the time between the onset of symptoms and the first episode of care and its effects on important worker outcomes in compensated musculoskeletal conditions is currently lacking. This scoping review aimed to summarize the factors associated with time to service and describe outcomes in workers with workers' compensation accepted claims for musculoskeletal conditions. METHODS: We used the JBI guidelines for scoping reviews and reported following the PRISMA-ScR protocol. We included peer-reviewed articles published in English that measured the…

Signs and symptoms after chemonucleolysis. A detailed evaluation of 214 worker's compensation and noncompensation patients.

The effectiveness of chemonucleolysis was examined in a retrospective study of 214 consecutive patients with herniated lumbar discs. All patients were candidates for surgery. All underwent myelography, and most had computed tomography (CT) scans. The clinical status of patients was assessed at 6 weeks and at 6 months, and evaluation was made from responses of the patients to a questionnaire at 1 year. Evaluation at 1 year demonstrated that the results of chemonucleolysis were successful for 181 patients (84.6%). Of the 172 patients who received no worker's compensation, the procedure was succe

Workers' compensation status is associated with increased complications following distal radius fracture open reduction and internal fixation.

BackgroundWhile workers' compensation (WC) patients often experience worse orthopedic outcomes, complication patterns following distal radius fracture (DRF) open reduction and internal fixation (ORIF) remain poorly defined. Prior DRF-specific studies include only small, regional cohorts. This study leverages Epic Cosmos to conduct the first nationwide analysis comparing complications between WC and non-WC populations.MethodsThis retrospective study queried the Epic Cosmos database for DRF ORIF procedures (2017-2024). WC patients (N = 4,101) were matched 1:5 with non-WC patients (N = 20,505) ba…

Patterns of antidepressant use in people with low back pain: A retrospective study using workers' compensation data.

BACKGROUND: Antidepressants are commonly used to treat low back pain (LBP), but little is known about patterns of antidepressant use in this population. This study aimed to identify patterns of antidepressant dispensing and switching in Australian workers with an accepted workers' compensation claim for LBP, and to investigate factors associated with dispensing and switching. METHODS: This retrospective study included data from accepted workers' compensation time loss claims for LBP between 2010 and 2018 with a 2-year follow-up. We described the type of antidepressant dispensed, the time for …

The Impact of Morphine Equivalent Daily Dose Threshold Guidelines on Prescribed Dose in a Workers' Compensation Population.

BACKGROUND: Prescription opioid overdose has increased markedly and is of great concern among injured workers receiving workers' compensation insurance. Given the association between high daily dose of prescription opioids and negative health outcomes, state workers' compensation boards have disseminated Morphine Equivalent Daily Dose (MEDD) guidelines to discourage high-dose opioid prescribing. OBJECTIVE: To evaluate the impact of MEDD guidelines among workers' compensation claimants on prescribed opioid dose. METHODS: Workers' compensation claims data, 2010-2013 from 2 guideline states and…
