Anatomy and injuries of the pediatric wrist: beyond the basics.

Ligamentous injuries of the pediatric wrist, once thought to be relatively uncommon, are increasingly recognized in the context of acute high-energy mechanism trauma and chronic axial loading, including those encountered in both recreational and high-performance competitive sports. Recent advances in MR-based techniques for imaging the pediatric wrist allow for sensitive identification of these often radiographically occult injuries. Detailed knowledge of the intrinsic and supportive extrinsic ligamentous complexes, as well as normal developmental anatomy and congenital variation, are essentia

Hand and Wrist Injuries in Boxing and the Martial Arts.

Hand and wrist injuries in martial arts are typically a reflection of the combat nature of this discipline. In striking sports, the axial load mechanism of injury is common and causes fractures and dislocations; in grappling sports, sprain injuries and degenerative changes predominate. There is clear evidence to support that hand protection reduces the risk of hand injury. Traditional training in martial arts on proper technique and target selection in striking sports reduces the risk of hand injury, and is an important component of hand and wrist injury prevention.

Particularities of hand and wrist complex injuries in polytrauma management.

Hand and wrist lesions are relatively common in polytraumatised patients. These subjects sustain a wide range of potential life-threatening conditions and hand and wrist injuries incurred are often not diagnosed or are insufficiently treated. Closed lesions are the most frequently missed diagnosis, but even severe open lesions may be incorrectly treated. Most of these hand and wrist injuries can have a strong negative impact on long-term quality of life, particularly when treatment of these injuries is poor or delayed. Orthopaedic and hand surgeons should be vigilant in their assessment and tr…

Triage nurse X-ray protocols for hand and wrist injuries.

Enabling triage nurses to request X-rays has a number of benefits for patients and staff. This article describes the development of a protocol and in-house training programme that allows triage nurses to assess patients with hand and wrist injuries, make a differential diagnosis of potential fracture and request X-rays. The protocol not only improves patients' journeys, but also empowers members of the emergency department's nursing team to develop new skills.

Acute hand and wrist injuries in athletes: evaluation and management.

Acute hand and wrist injuries in the athlete constitute a unique orthopaedic challenge. Because of the particular demands on the athlete (e.g., financial implications, coaching and administration pressures, self-esteem issues), a specialized management approach is often necessary. Common sites of injury include the ulnar collateral ligament of the thumb metacarpophalangeal joint, proximal interphalangeal joint, metacarpals and phalanges, scaphoid, hamate, and distal radius. Treatment of these injuries varies depending on the patient's age, sport, position played, and level of competition, but …

Closed tendon injuries of the hand and wrist in athletes.

Closed-tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, if seen acutely, can be treated successfully with nonoperative management, although some do require operative treatment. It is those injuries, however, that go undiagnosed until the end of the season, which may result in permanent disability.

[Recent injuries of the nerves of the wrist and hand].

The advent of microsurgery has in the last few decades considerably improved the results of treatment of lesions of nerves in the wrist and the hand, both with regard to functional restoration of sensation and movement and to reduction of local irritant factors or those linked to cold injury. Peripheral nerves are frequently injured at the wrist and the hand by contusion, compression, straining or total or partial section. In the case of wrist, hand or finger wound, only well-directed, precise and systematic clinical examination can determine the neurological deficit. Surgery is always indicat

Helicopter skiing wrist injuries. A case report of "bugaboo forearm".

Of the 42 skiers, 5 (11.9%) experienced symptoms of intersection syndrome early in their week of powder skiing. According to the mountain guides employed by the helicopter service, this overuse injury is quite common and does not represent an unusual incidence for this group of skiers. The mechanism of injury is likely repetitive dorsiflexion and radial deviation of the wrist against the resistance of deep snow on withdrawal of the planted ski pole. Interestingly, the more advanced competitive alpine skiers appeared to be most at risk, possibly because of their more aggressive pole plants. Pre

Office management of athletic injuries of the hand and wrist.

Most common hand injuries occurring in athletics can be simply treated and the athlete returned to participation at an early date. Delayed or inadequate treatment may result in an unacceptable outcome, including permanent deformity. The injured athlete must be carefully examined, treated, and monitored until fully rehabilitated.

Rehabilitation of common athletic injuries of the hand and wrist.

Rehabilitation of athletic injuries of the hand and wrist should begin at the time of injury. Early recognition and prompt medical attention can serve to minimize the initial effects of injury. A systematic evaluation should include assessment of deformity, joint instability, active and passive motion, edema, and nerve and tendon function. In spite of initial negative findings, painful joints should be protected. Joints of the hand and wrist are particularly vulnerable to the effects of aggressive rehabilitation. Signs of reaction to exercises should be closely observed. Upgrading of activity

CT of the wrist for the evaluation of traumatic injuries.

Computed tomography (CT) of the wrist can help evaluate many types of posttraumatic injuries, particularly bony trauma. Most wrist fractures can be diagnosed with routine radiography, but problematic cases and complicated healing fractures can be difficult to evaluate with conventional radiography. CT provides improved contrast resolution and multiplanar imaging. The results of complex healing fractures and fusion procedures can be facilitated with CT. Even in the face of advanced osteoporosis CT can delineate bone edges and detect small areas of bony fusion. The degree of bone graft assimilat

Diagnosing and managing hand and wrist injuries in the emergency department.

Injuries of the hand and wrist are commonly encountered in the emergency department. Though they are rarely life-threatening, there is potential for significant patient pain, morbidity, and long-term functional loss if they are not treated appropriately. A thorough and focused examination and plain radiographs are sufficient to diagnose most hand injuries, with outpatient referral to a hand specialist when indicated. Clinicians must be able to identify the life- or limb-threatening hand injuries that can present, both in treating and facilitating urgent hand consultation. This review provides

Imaging of Sports-related Hand and Wrist Injuries: Sports Imaging Series.

Hand and wrist injuries are common occurrences in amateur and professional sports and many of them are sport-specific. These can be divided into two categories: traumatic injuries and overuse injuries. The aim of this article is to review the most common hand and wrist sports-related lesions. Acute wrist injuries are predominantly bone fractures, such as those of the scaphoid, hamate hook, and ulnar styloid. Ligament lesions are more challenging for radiologists and may lead to carpal instability if undiagnosed. Overuse wrist injuries are mainly represented by tendinous disorders, with De Quer…

Arthroscopic management of partial scapholunate and lunotriquetral injuries of the wrist.

Outcome was studied in 14 patients with chronic mechanical (crepitant) wrist pain (symptomatic for more than 6 months) who had undergone arthroscopic debridement and early mobilization for the treatment of isolated partial scapholunate and lunotriquetral ligament injuries. Fourteen patients were available for a minimum 2-year follow-up interview, physical examination, and x-ray film evaluation. Eleven patients had complete relief of symptoms and had returned to work within 7 weeks of surgery. The other three (all workers' compensation cases) reported occasional mild wrist pain; two had returne

Arthroscopic management of wrist triangular fibrocartilage complex injuries in the athlete.

Wrist arthroscopy can facilitate early definitive diagnosis of debilitating soft tissue injuries in athletes. Many such injuries can be treated successfully with minimally invasive arthroscopic techniques, reducing the morbidity associated with surgical exposure and permitting earlier return to competition. The triangular fibrocartilage complex (TFCC) is vulnerable to injury from rotational forces or axial load applied to the hand. Under arthroscopic control, injuries to the central articular disk can be treated by excision of unstable tissue fragments; peripheral separation of the disk from t
