Combined Effects of Exercise and Denosumab Treatment on Local Failure in Post-menopausal Osteoporosis-Insights from Bone Remodelling Simulations Accounting for Mineralisation and Damage.

Denosumab has been shown to increase bone mineral density (BMD) and reduce the fracture risk in patients with post-menopausal osteoporosis (PMO). Increase in BMD is linked with an increase in bone matrix mineralisation due to suppression of bone remodelling. However, denosumab anti-resorptive action also leads to an increase in fatigue microdamage, which may ultimately lead to an increased fracture risk. A novel mechanobiological model of bone remodelling was developed to investigate how these counter-acting mechanisms are affected both by exercise and long-term denosumab treatment. This model

Are ISPD Guidelines on Peritonitis Diagnosis Too Narrow? A 15-Year Retrospective Single-Center Cohort Study on PD-Associated Peritonitis Accounting for Untrained Patients.

Background:Peritoneal dialysis (PD)-associated peritonitis remains by far the most important complication requiring patients to transfer to hemodialysis and has a major impact on patient morbidity and mortality. Current International Society for Peritoneal Dialysis (ISPD) guidelines on peritonitis recommend analysis of peritonitis episodes only in trained patients. In a large tertiary care center, we analyzed peritonitis episodes accounting for different groups of untrained patients and compared these with episodes in the trained patient population. Methods:We analyzed data collected prospectiv…

A multi-dimensional measure of population ageing accounting for Quantum and Quality in life years: An application of selected countries in Europe and Asia.

Population ageing measured through a fixed old-age threshold like 60+ or 65+ ignores the other important dimensions of ageing. There has bee … looking approach to measure ageing and specifies an old-age threshold for different countries after accounting for different dimensions of life expectancy, health and human capital. This method is more suitable for comparison across countries with distinct demographic and health achievements. The empirical application of our method using selected countries f …

Editorial Commentary: Graft Removal Versus Retention for Septic Arthritis After Anterior Cruciate Ligament Reconstruction: Accounting for Patient Preference Flips the Script on Current Practice.

Septic arthritis is a rare but potentially devastating complication after anterior cruciate ligament (ACL) reconstruction. Studies examining surgical treatment options including graft retention and removal are largely inconclusive. Although the literature indicated that 43.8% of patients who have the graft removed later go on to have a revision ACL reconstruction compared with only 6.5% among those whose graft is retained, this expected-value decision analysis found removal to be the optimal treatment for patients with septic arthritis after ACL reconstruction. This is largely based on patient

The emotion regulation and recognition account of empathy-related processes in autism: a multimodal, multilevel case-control study.

BACKGROUND: Recent evidence suggests that many autistic individuals exhibit heightened emotional responsiveness to others' affect despite greater difficulty recognizing others' emotions and accurately inferring their perspectives. This study introduces and preliminarily evaluates the Emotion Regulation and Recognition Account (ERRA), a model proposing that susceptibility to emotion dysregulation and challenges with emotion cue interpretation jointly shape empathy-related processing in autism. METHODS: Children aged 6-13 years (Nautistic = 103; Nnon-autistic = 40) participated; groups were mat

Empowered yet accountable? Digitalisation and the shifting boundaries of responsibility in healthcare a qualitative investigation with patients and healthcare professionals.

Responsibility for illness management is being reconfigured changing professional practices and increasing expectations of patient self-management, e.g. through digitalisation. This article examines how responsibility is produced, distributed and enacted between patients, healthcare professionals and healthcare institutions. This study draws upon 54 qualitative interviews with 38 patients and 16 healthcare professionals in Danish hospital settings. The work utilizes abductive thematic analysis informed by perspectives on governmentality and responsibilisation (Foucault, Lupton) and Bourdieu's

A Mixture of Distributed Lag Non-Linear Models to Account for Spatially Heterogeneous Exposure-Lag-Response Associations.

Environmental exposures, such as air pollution and extreme temperatures, have complex effects on human health. These effects are often characterized by non-linear exposure-lag-response relationships and delayed impacts over time. Accurately capturing these dynamics is crucial for informing public health interventions. The Distributed Lag Non-Linear Model (DLNM) is a flexible statistical framework for estimating such effects in epidemiological research. However, standard DLNM implementations typically assume a homogeneous exposure-lag-response association across the study region, overlooking po

Designing for generalism, courage, and systems: a curriculum scholarship account of an integrated family medicine and primary care block.

BACKGROUND: Pre-clerkship Family Medicine teaching often covers content well but does too little with experiential immersion, structured reflection, and the open engagement with clinical uncertainty that defines generalist work in practice. We describe the partnership-based redesign of the COM353 Family Medicine and Primary Care block at Alfaisal University, Riyadh, with student-reported outcomes from the first delivered cohort. The paper is offered as a worked account of how academic-academic and academic-service partnerships can themselves become curriculum. PEDAGOGICAL FRAMEWORK: The redes

Reclaiming the writing process using AI: a scaffolded writing assignment that promotes critical thinking, reflection, and accountability.

As the educational landscape changes with the widespread use of generative artificial intelligence (AI), we have made the deliberate choice to adapt with intention through the constructive integration of this tool within the curriculum. Writing assignments are often vulnerable to AI shortcuts, and therefore, we reimagined the project in our online microbiology lab to incorporate AI in a structured and pedagogically sound way that prioritizes both the writing process and the final product. Students must work with AI to create a script for a 5-minute video which they then illustrate and record o

A first account of fossil centrohelid heliozoans from an ancient Eocene locality near the Arctic Circle in Canada.

Centrohelid heliozoans are non-motile, single-celled, heterotrophic protists with radiating filose pseudopodia used to capture food prey, that are common components of soil, marine, and freshwater environments. Centrohelids are characterized by a cell covering consisting of overlapping siliceous components referred to as scales that serve as the primary means to distinguish between genera and species. Given the widespread occurrence of heliozoans in aquatic habitats, and the fact that the siliceous scales can fossilize, it is surprising that these organisms have not been previously reported fr

Pregnancy Anxiety as a Social Symptom: Personal Accounts of Pregnant Women During the COVID-19 Pandemic in Quebec.

This article presents the results of a study conducted in Quebec (Canada), in the Montreal and Abitibi-Temiscamingue regions, with forty pregnant women interviewed between 2020 and 2021 during the COVID-19 pandemic. Based on interpretive phenomenological analysis, the study highlights pregnancy-related anxiety exacerbated by reduced medical care, the exclusion of partners, social isolation, and the disruption of pregnancy-related rituals. While pregnancy is generally a significant transition in terms of identity and social life, women experienced this period in a context marked by loneliness,

Recalibrating occupational stress and well-being: Junior high school teachers' retrospective accounts of mindfulness training.

Quantitative research has reported associations between mindfulness-based interventions (MBIs) and teacher stress and well-being, but less is known about how teachers themselves retrospectively experience and make sense of learning mindfulness. This interpretative phenomenological study examined eight Chinese junior high school teachers' retrospective accounts following a four-week standardized mindfulness program. Participants described an initial struggle with a "busy mind" and physical discomfort, followed in some accounts by a more noticeable capacity to pause before responding to classroo

Sustainable high performance computing in the UK: a position paper on public investment, accountability, and the path to net zero.

BACKGROUND: Factors associated with early re-collapse after posterior fixation of thoracolumbar burst fractures have been sparsely examined in previous studies. Early re-collapse is challenging to predict due to multifactorial risk. Therefore, we aimed to investigate factors associated with vertebral re-collapse within the first 3 months following posterior fixation. METHODS: This retrospective study used data from the Swedish Fracture Register (2015-2019) and included patients aged 18 to 66 years with a single-level thoracolumbar burst fracture between T11 and L3 treated with posterior fixat

Feeling ill, not harmed: a phenomenological account of the long-term experience of false-positive breast cancer screening.

Scholarship has highlighted how the identification of breast cancer risk and false-positive screening results operate as modes of governance that shape subjects and extend the reach of medicine. Building upon this work, this article offers a phenomenologically-informed analysis of women's experiences of false-positive results in breast cancer screening, examining how such encounters with risk and the possibility of disease are lived and carried into everyday life. Based on ethnographic interviews with women who received false-positive results approximately 18 to 20 years previously, this study

Physiological Evidence Supporting the Emotional Motivation Account of the Ending Effect: Pupil Diameters Increase Toward the End.

The phenomenon of increased risk-taking in the last round of a set of risky decision-making tasks is called the ending effect. Recent empirical studies proposed an emotional motivation account to explain the ending effect. That is, the pursuit of an emotionally satisfying ending leads to the increase of risk-taking. However, previous studies have mostly examined the ending effect at the behavioral level, there is yet no physiological evidence to examine the emotional motivation account. To fill in this gap of knowledge, the current study examined the emotional motivation account at the physiol
