A preclinical study on online monitoring of exhaled ciprofol concentration by the ultraviolet time-of-flight spectrometer and prediction of anesthesia depth in beagles.

BACKGROUND: Exhaled air has been demonstrated as a reliable medium for monitoring propofol concentration. However, online monitoring of exhaled ciprofol have not been reported. METHODS: Thirty-six beagles undergoing mechanical ventilation were divided into 6 groups, including bolus injection of low (Group BL, n = 6), medium (Group BM, n = 6), and high dose of ciprofol (Group BH, n = 6) groups; as well as 1 h continuous infusion of low (Group IL, n = 6), medium (Group IM, n = 6), and high dose of ciprofol (Group IH, n = 6) groups. The ciprofol concentration in exhaled air (CE) was determined b

Emergence Agitation with Earthquake-Related Traumatic Stress Symptoms After Intravenous Sedation.

Sedation with intravenous anesthetics is a sedation method that is often preferred during minor surgical procedures for anxious patients. Among the anesthetic agents used are drugs such as midazolam and ketamine, which can cause psychiatric symptoms such as loss of control over the behavior of the person (disinhibition) or dissociation. In people with high anxiety levels, a paradoxical rise of anxiety may rarely occur with midazolam, and emergence agitation or delirium may occur after anesthesia with ketamine. Post-traumatic stress disorder is a known risk factor for emergence agitation. Accom

Pharmacokinetics of remimazolam after intravenous infusion in anaesthetised children.

BACKGROUND: The pharmacokinetic properties of the new benzodiazepine remimazolam have been studied only in adults. We investigated the pharmacokinetics of remimazolam after i.v. infusion in anaesthetised paediatric patients. METHODS: Twenty-four children (2-6 yr, ASA physical status 1-2, BMI 15-18 kg m-2) undergoing general anaesthesia with sevoflurane were enrolled. During surgery, remimazolam was administered as an i.v. infusion over 1 h at 5 mg kg-1 h-1 for 5 min, followed by 1.5 mg kg-1 h-1 for 55 min. Plasma concentrations of remimazolam and its metabolite CNS7054 were determined from ar

Relationship Between Anesthesia Depth and Quality of Seizures in Patients Undergoing Electroconvulsive Therapy: A Prospective Observational Study.

OBJECTIVES: Electroconvulsive therapy under general anesthesia is an established treatment for mood disorders, such as therapy-resistant depression. As most anesthetic drugs used for induction of anesthesia increase the seizure threshold, adequate depth of anesthesia without diminishing the therapeutic efficacy of interventions is crucial. The aim of this study was to investigate whether anesthesia depth as assessed by Narcotrend (NCT) monitoring correlates with maximum seizure quality. METHODS: An observational study was performed in psychiatric patients undergoing multiple interventions of

Anesthetic induction trismus, more than a closed-mouth problem.

Trismus, or masseter hypertonia, that results from the use of succinylcholine during induction of anesthesia is a rare and dangerous phenomenon. It presents to the anesthesiologist the immediate problem of airway management but it also must be recognized by the physician as a harbinger of malignant hyperthermia. We report a case of induction trismus and discuss its association with malignant hyperthermia. The pathophysiology, diagnosis and treatment of malignant hyperthermia are reviewed.

Intravenous anaesthesia and the need for new agents.

Ideally one would like a water-soluble, non-irritant, rapidly-acting, smooth induction agent, with no antanalgesic action. Cardiovascular and respiratory depression should be minimal with normal dosage. Since these are unlikely to be achieved with a single drug, the combination of opioid and induction agent offers possibilities that should be explored. A slight delay in onset would not be a major obstacle, provided this is predictable, but the prime need is for a rapidly acting less toxic alternative to thiopentone.

[Clinical transcutaneous measurement of oxygen pressure during the induction of anesthesia with barbiturates].

Intravenous injection of barbiturates is followed by a depression of the respiratory centre depending on the dose. Blood carbon dioxide increases. This can be measured continuously in the expired air. In the same time blood oxygen modifies. The hypoxia produced by this depressant action was determined by a non-invasive transcutaneous blood gas measurement technique during the induction of anesthesia.

[Anesthetic management of patients with muscular disease?].

The past two decades has been characterized by important advances in symptomatic care to the muscular patient. In particular, the development of home mechanical ventilation was associated by a substantial improvement in life expectancy. The presence of a muscular disease is less frequently a contra-indication of surgery and subsequently the issue of anesthetizing these patients has become a routine question. A growing number of muscular disorders have been subjected to specific guidelines for anesthetic procedures.

Effect of progressive mandibular advancement on pharyngeal airway size in anesthetized adults.

BACKGROUND: General anesthesia in adult humans is associated with narrowing or complete closure of the pharyngeal airway. The purpose of this study was to determine the effect of progressive mandibular advancement on pharyngeal airway size in normal adults during intravenous infusion of propofol for anesthesia. METHODS: Magnetic resonance imaging was performed in nine normal adults during wakefulness and during propofol anesthesia. A commercially available intraoral appliance was used to manually advance the mandible. Images were obtained during wakefulness without the appliance and during an

Anesthetic Considerations for Patients With Hypokalemic Periodic Paralysis Undergoing Ambulatory Surgery: A Case Report.

The perioperative management of patients with hypokalemic periodic paralysis is challenging due to the risk of postoperative muscle weakness. We describe a patient with hypokalemic periodic paralysis who presented for wisdom teeth extraction. Our perioperative goals included minimizing exposure to weakness triggers such as hypokalemia, pain, anxiety, hypothermia, and neuromuscular blockade. The patient tolerated the procedure under deep sedation with intravenous agents, local anesthesia, and a natural airway with spontaneous ventilation. He experienced mild upper extremity weakness postoperati

Anesthetic Management for Inclusion Body Myositis in Coronary Artery Bypass Graft Surgery.

Anesthetic management for patients with certain neuromuscular disorders may be challenging due to contraindications to triggering agents secondary to increased susceptibility for malignant hyperthermia (MH). Inclusion body myositis (IBM) is an inflammatory muscle disease that causes concern for the anesthesiologist due to potential respiratory muscle weakness and hyperkalemia with succinylcholine. Elevated serum creatinine kinase levels found in IBM also raise the possibility of increased susceptibility to MH. This case report describes a successful anesthetic course with special consideration

Surgery as a Recurrent Trigger for Immune Thrombocytopenia (ITP) in a Patient Cured of M7 Acute Myelogenous Leukemia.

We report the case of a 10-year-old boy, 8 years post-M7 acute myeloid leukemia with a history of significant thrombocytopenia and bleeding, requiring treatment, after 2 surgical procedures performed under general anesthesia. In both instances, the thrombocytopenia and bleeding responded to intravenous immunoglobulin treatment. Between surgeries, the platelet counts were normal. Before a third surgical procedure, he was successfully pretreated with dexamethasone and experienced no bleeding or thrombocytopenia after the operation. This case highlights the potential utility of corticosteroid pre

[Prolonged respiratory depression after fentanyl administration in a patient with mitochondrial encephalomyopathy].

A 21-year-old man with mitochondrial encephalomyopathy underwent surgery for removal of a maxillary cyst. During the induction of anesthesia, the patient fell into the state of apnea after intravenous administration of fentanyl 100 microgram. Trachea was successfully intubated followed by propofol 50 mg without any muscle relaxants. Anesthesia was maintained uneventfully under sevoflurane (1-2%), nitrous oxide (30%), and oxygen (70%). The respiratory depression lasted for about 120 minutes after administration of fentanyl, and was antagonized by naloxone 40 microgram. This case suggests that c
