Infective endocarditis after body art: a review of the literature and concerns.

PURPOSE: Infective endocarditis (IE) is a rare but dangerous complication of tattooing and body piercing in adolescents and young adults 15- … des more evidence of IE and body art concerns, and should stimulate further discussion regarding IE antibiotic prophylaxis. It is believed that IE is triggered by normal flora at the body art site, microorganism colonization around the jewelry, or by a localized site infection that stimulates episodes of transient bacteremia (commonly caused by staphylococi) and then se …

Do anterior cruciate ligament allograft culture results correlate with clinical infections?

PURPOSE: In 1998, four cases of contaminated allografts for anterior cruciate ligament (ACL) reconstruction resulted in Clostridium infection, and a patient with Clostridium infection from a femoral condylar allograft died. It was subsequently published that implanting surgeons should culture ACL allografts so that action could be taken should highly pathogenic bacteria be encountered. The purpose of this study is to test the hypothesis that ACL allograft cultures correlate with clinical infections. METHODS: Since October 2003, a single surgeon performing ACL reconstruction prospectively cult

Penicillins for the prophylaxis of bacterial endocarditis in dentistry.

BACKGROUND: Many dental procedures cause bacteraemia and it is believed that this may lead to bacterial endocarditis (BE) in a few people. G … any beneficial effect. Ethically practitioners need to discuss the potential benefits and harms of antibiotic prophylaxis with their patients before a decision is made about administration.

Neisseria meningitidis: presentation, treatment, and prevention.

Neisseria meningitidis is a leading cause of meningitis and septicemia in children and young adults in the United States. Highly publicized outbreaks of disease caused by this organism in communities and on college campuses have resulted in a heightened public awareness of its potentially devastating effects. The rapid progression of signs and symptoms of meningococcemia necessitate early recognition and institution of appropriate therapeutic measures. Identifying contacts of index cases who are at high risk of acquiring the disease allows health care providers to institute appropriate chemopr

Decrease in acute periprosthetic joint infections incidence with vancomycin-loaded calcium sulfate beads in patients with non-modifiable risk factors. A randomized clinical trial.

OBJECTIVES: The influence of local antibiotic therapy in orthopedic surgery remains unclear. In this trial, we evaluated the incidence of periprosthetic joint infections (PJI), after local or intravenous (IV) antibiotic prophylaxis. The aim of this intervention was to compare the PJI incidence in a population with non-modifiable risk factors after local prophylaxis with vancomycin-loaded calcium sulfate beads versus a control group. METHODS: A total of 83 subjects were evaluated, inclusion criteria included participants over 60 years of age, with at least one main risk factor for PJI who unde…

Microbiological profile of nosocomial infections following cardiac arrest: Insights from the targeted temperature management (TTM) trial.

AIMS: Infectious complications frequently occur in intensive care unit patients admitted after out-of-hospital cardiac arrest. There is debate on the effects of temperature management on the incidence of infections, as well as on the efficacy and choice of antibiotic prophylaxis. In this substudy of the targeted temperature management (TTM) trial, we describe the microbiological profile of infectious complications in patients with cardiac arrest and examined the impact of TTM at 33 °C compared to TTM at 36 °C. Furthermore we aimed to determine the association between antibiotic prophylaxis and…

Post-operative prophylactic antibiotics in aseptic revision hip and knee arthroplasty: a propensity score matching analysis.

The use of extended antibiotic (EA) prophylaxis (> 24 h) remains controversial in aseptic revision arthroplasty. We sought to determine whether EA prophylaxis reduces the risk of periprosthetic joint infection (PJI) in aseptic revision hip and knee arthroplasty. A total of 2800 patients undergoing aseptic revision hip and knee arthroplasty at five institutional databases from 2008 to 2017 were evaluated. One to two nearest-neighbor propensity score matching analysis was conducted between patients who did and did not receive extended antibiotic prophylaxis. The matching elements included age, s…

A Prolonged Post-Operative Antibiotic Regimen Reduced the Rate of Prosthetic Joint Infection after Aseptic Revision Knee Arthroplasty.

BACKGROUND: The aim of this study was to compare the prosthetic joint infection (PJI) rate after knee revision arthroplasty in two consecutive periods with different antibiotic prophylaxis: short (one day) versus long (five days). METHODS: From January 2007 to September 2010 antibiotic prophylaxis consisted of 800 mg of teicoplanin and 2 g of ceftazidime intravenously and 1 g of ceftazidime two hours after the first dose. From October 2010, it was prolonged post-operatively using vancomycin and ceftazidime intravenously until the fifth day. RESULTS: During the study period, 341 revision surg…

Reduction of prophylactic antibiotics for elective pediatric cholecystectomy: A QI initiative to promote antimicrobial stewardship.

PURPOSE: Antibiotic prophylaxis minimizes the risk of infection, but overtreatment leads to antimicrobial resistance. Adult studies suggest antibiotic prophylaxis for elective cholecystectomy may not be needed. Our quality improvement project aimed to decrease prophylactic antibiotic use for elective cholecystectomies from 100 % to ≤50 % and maintain that rate for one year. METHODS: Prior to initiation, the pre-operative orderset was modified, education was provided, and surgeon consensus was achieved. All patients undergoing elective minimally invasive (laparoscopic and robotic) cholecystect…

Current antibiotic and prophylactic antifungal drug policies in UK neonatal intensive care units: a national survey.

OBJECTIVES: To survey the current antibiotic and antifungal drug practices of UK neonatal intensive care units (NICUs), and to identify antibiotic preferences and policies for treatment of early- and late-onset sepsis (EoS and LoS), meningitis, and antifungal prophylaxis. METHODS: Between January and May 2024, we contacted all 53 tertiary-level UK NICUs via telephone and/or e-mail. We requested a copy of each unit's guidelines for antibiotic treatment of EoS and LoS, and antifungal prophylaxis. RESULTS: We obtained guidelines from 53/53 (100%) units. Onabotulinum Toxin A-Led Urinary Tract Infections-Should we Safeguard? A Randomized Controlled Trial.

INTRODUCTION AND HYPOTHESIS: The objective of our study was to evaluate the need for antibiotic prophylaxis for urinary tract infection (UTI) prevention before Onabotulinum toxin A injection for overactive bladder (OAB). We hypothesize that the lack of antibiotic prophylaxis might not be inferior to administering prophylaxis. METHODS: This was a multi-centered, nonblinded, randomized controlled trial conducted between August 2022 and September 2024. Participants were randomized to either receive oral antibiotics or no treatment. Our primary outcome was to measure the rate of UTI at 2 weeks. O…

[Verification of antibiotic treatment after animal bites among patients at the antirabies center in Rome].

The study aimed at searching whether antibiotic prophylaxis was practised after animal bites, and, in this case, what were the criteria adopted. It was performed by considering 162 outpatients requiring assistance after animal bites at the Antirabies Centre of Rome, Italy. In this circumstances, prevention of bacterial and viral infections is performed by wound detersion, by administering anti-tetanus and anti-rabid prophylaxis where required by italian policy, according to the patient's history. Infectious complications are described in literature as a common consequence of animal bites and i…

Effects of intrapartum penicillin prophylaxis on intestinal bacterial colonization in infants.

Early-onset group B streptococcal (GBS) infections remain a leading cause of morbidity and mortality in infants. To prevent the vertical transmission of GBS and neonatal GBS infection, guidelines recommend intrapartum penicillin or amoxicillin prophylaxis. This intrapartum antibiotic prophylaxis (IAP) is suspected to favor colonization by antibiotic-resistant bacteria. However, the effects of this prophylaxis on the patterns of acquisition of gastrointestinal bacterial flora in infants have never been studied. We collected stool samples from 3-day-old infants born to mothers who received intra…
