Renal injury and hypocalcaemia in newborns with birth asphyxia at a tertiary care centre: a prospective observational study.

BACKGROUND: Birth asphyxia, also known as perinatal hypoxia-ischaemia, is a significant cause of neonatal morbidity and mortality. It occurs when a newborn experiences insufficient oxygen during birth, potentially leading to multisystem injury including renal dysfunction and disruption of calcium homeostasis. Understanding the impact of birth asphyxia on renal function and serum calcium levels is critical for early detection and timely intervention. MATERIALS AND METHODS: Of 185 term neonates assessed for eligibility (27 excluded-congenital anomalies n=10; septicemia n=9; respiratory distress

Cognitive development at late infancy and school age in children cooled for neonatal encephalopathy.

BACKGROUND: We investigated the association and individual changes in cognitive scores between late infancy and early school age in children cooled for neonatal encephalopathy secondary to perinatal asphyxia (NE) who did not develop cerebral palsy. METHODS: We included 50 children born ≥35 weeks gestation cooled for NE who did not develop cerebral palsy. We assessed cognition using an average of cognitive and language composite scores (CLC) from Bayley Scales of Infant and Toddler Development (Bayley-III) at 18-21 months and full-scale IQ (FSIQ) on Wechsler Intelligence Scale for Children (WI

Enzymatic and Non-enzymatic Antioxidants as Related to Immediate Hospital Outcomes in Term Nigerian Babies with Perinatal Asphyxia.

BACKGROUND AND OBJECTIVES: Antioxidants are often released to neutralise free radicals and oxidants generated in babies with perinatal asphyxia (PA). This study aims to determine the enzymatic and non-enzymatic antioxidants in babies with PA and the relationship between these antioxidants and hospital outcome at the Wesley Guild Hospital (WGH), Ilesa, Nigeria. METHODS: Consecutive term babies with or without asphyxia (defined as 5th minute Apgar score <7) were consecutively recruited into a comparative cross-sectional study at the labour ward of the WGH over a nine-month period. Babies were a

Effects of perinatal asphyxia on cortical activity in two-year-old children.

Perinatal asphyxia can lead to clinical hypoxic-ischemic encephalopathy (HIE) associated with high morbidity and mortality, but less is known about long-lasting effects of perinatal asphyxia alone (PA). Here, we investigate how PA with versus without clinical HIE affects cortical activity networks at two years of age. Electroencephalographic (EEG) recordings were acquired during sleep from three cohorts of children (PA only (n = 10), PA with mild to moderate HIE (n = 8), and healthy controls (n = 37)), and we assessed the group differences in local cortical function and cortico-cortical networ

Evaluation of Educational Activity with Guatemalan Birth Attendants About Helping Babies Breathe.

Introduction: Guatemala's Infant Mortality Rate was 20th highest in the world at 18 deaths per 1,000 live births in 2022. Part of the problem is lay midwives (LMs) who attend most births at home lack training in helping babies breathe (HBB). LMs transfer babies struggling to breathe to firefighters (FF). The aims of this mixed-methods pilot study were to evaluate a HBB educational activity, explore LM and FF's lived experience HBB, and to assess if the HBB educational activity changed participants' knowledge. Methods: Twelve LMs and six FFs (N = 18) participated in focus group discussions, com

Temporal development of seizure threshold and spontaneous seizures after neonatal asphyxia and the effect of prophylactic treatment with midazolam in rats.

Birth asphyxia (BA) and subsequent hypoxic-ischemic encephalopathy (HIE) is one of the most serious birth complications affecting full-term infants and can result in severe disabilities including mental retardation, cerebral palsy, and epilepsy. Animal models of BA and HIE are important to characterize the functional and behavioral correlates of injury, explore cellular and molecular mechanisms, and assess the potential of novel therapeutic strategies. Here we used a non-invasive, physiologically validated rat model of BA and acute neonatal seizures that mimics many features of BA and HIE in h

Rapid oxygen titration following cardiopulmonary resuscitation mitigates cerebral overperfusion and striatal mitochondrial dysfunction in asphyxiated newborn lambs.

Asphyxiated neonates must have oxygenation rapidly restored to limit ongoing hypoxic-ischemic injury. However, the effects of transient hyperoxia after return of spontaneous circulation (ROSC) are poorly understood. We randomly allocated acutely asphyxiated, near-term lambs to cardiopulmonary resuscitation in 100% oxygen ("standard oxygen", n = 8) or air (n = 7) until 5 minutes after ROSC, or to resuscitation in 100% oxygen immediately weaned to air upon ROSC ("rapid-wean", n = 7). From 5 minutes post-ROSC, oxygen was titrated to target preductal oxygen saturation between 90-95%. Cerebral tiss

Increase in Newborns Ventilated Within the First Minute of Life and Reduced Mortality After Clinical Data-Guided Simulation Training.

INTRODUCTION: Birth asphyxia-related deaths is a major global concern. Rapid initiation of ventilation within the "Golden Minute" is important for intact survival but reported to be challenging, especially in low-/middle-income countries. Helping Babies Breathe (HBB) is a simulation-based training program for newborn resuscitation. The aim of this HBB quality improvement (QI) intervention was to decrease time from birth to ventilation and document potential changes in perinatal outcomes. METHOD: Prospective observational QI study in a rural Tanzanian hospital, October 1, 2017, to August 31, 2

The Impact of Phenobarbital on the Ability of Electroencephalogram to Predict Adverse Outcome in Asphyxiated Neonates during Therapeutic Hypothermia.

OBJECTIVE: Classification of electroencephalogram (EEG) background has been established to predict outcome in neonates with hypoxic ischemic encephalopathy (HIE). However, the impact of phenobarbital therapy on the predictability of EEG background has not been studied. Our objective is to determine if EEG background after treatment with phenobarbital during therapeutic hypothermia (TH) remains a good predictor for brain injury in neonates with HIE. STUDY DESIGN: This is a single-center, retrospective study of consecutive neonates with HIE who underwent TH and EEG monitoring from October 2017

Aberrant expression of miR-199a in newborns with hypoxic-ischemic encephalopathy and its diagnostic and prognostic significance when combined with S100B and NSE.

Neonatal hypoxic-ischemic encephalopathy (HIE) is a disorder mainly due to asphyxia during the perinatal period, and late diagnosis leads to high mortality. In this study, the expression of microRNA-199a (miR-199a) in HIE newborns was investigated, as well as its clinical significance in HIE diagnosis and prognosis. Circulating levels of S100B and NSE in HIE newborns were measured using enzyme-linked immunosorbent assay, and the expression of miR-199a was analyzed using quantitative real-time PCR. The diagnostic value of miR-199a, S100B and NSE was evaluated using the receiver operating charac

Feasibility of oscillating and pulsed gradient diffusion MRI to assess neonatal hypoxia-ischemia on clinical systems.

Diffusion-time- (td) dependent diffusion MRI (dMRI) extends our ability to characterize brain microstructure by measuring dMRI signals at varying td. The use of oscillating gradient (OG) is essential for accessing short td but is technically challenging on clinical MRI systems. This study aims to investigate the clinical feasibility and value of td-dependent dMRI in neonatal hypoxic-ischemic encephalopathy (HIE). Eighteen HIE neonates and six normal term-born neonates were scanned on a 3 T scanner, with OG-dMRI at an oscillating frequency of 33 Hz (equivalent td ≈ 7.5 ms) and pulsed gradient (

A randomized controlled trial on the use of magnesium sulfate and melatonin in neonatal hypoxic ischemic encephalopathy.

BACKGROUND: Birth asphyxia is a leading case of neonatal morbidity and mortality especially in developing countries. Hypoxic-ischemic encephalopathy (HIE) attributed to asphyxia can be ameliorated with several remedies, although full recovery is currently not feasible. The aim of this trial on infants with HIE who are receiving melatonin therapy, is to assess the added effect of magnesium sulfate (MgSO4) on the expression of S100-B, a marker of brain injury. METHODS: This study is a randomized controlled trial on neonates with moderate HIE (Sarnat grade II). Infants were randomized into 2 gro
