As a group, calcium antagonists cause vascular dilation and do not cause bronchial constriction, in contrast to the beta-adrenoceptor blocking agents. In many patients, these diverse properties allow safe combination of …

Radiofrequency catheter ablation has established itself as a first line therapy for the curative treatment of many patients with supraventricular or atrioventricular tachycardias and also for selected types of ventricular tachycardia. The success rates of catheter ablation of various types of cardiac arrhythmias are impressively high. Procedure related complications can be attributed to the invasive nature of the technique (e.g., bleeding or other vascular complications, radiation exposure) but may also occur as a specific complication related to the type of intervention performed (e.g., compl

INTRODUCTION: Atrioventricular nodal re-entry tachycardia (AVNRT) is the most common, regular narrow-complex tachycardia. The established treatment is catheter ablation of the AV nodal slow pathway (SP). However, in a select group of patients with long PR intervals in sinus rhythm, SP ablation can lead to AV block due to the absence of robust anterograde conduction through the fast pathway (FP). This report aims to demonstrate that AV nodal FP ablation is a reasonable approach in patients with AVNRT and poor or absent anterograde FP conduction.
METHODS AND RESULTS: Standard electrophysiology

Atrial premature contractions (PACs) that block at the atrio-ventricular (AV) nodal level and occurring in a bigeminal frequency are recognized as a cause of symptomatic bradycardia. Appropriate suppression of the PACs often results in restoration of a regular rhythm with resolution of bradycardia-related symptoms. We report a series of three patients with non-conducted bigeminal PACs arising from the mitral annulus that resulted in symptomatic bradycardia and who were referred for consideration of cardiac pacing. Focal ablation suppressed PACs restoring a normal heart rate and resolution of s

Cardiac pacemakers are an extremely effective treatment for bradycardia but can, however, cause desynchronization of ventricular contraction leading to cardiomyopathy. Pacing of the conduction system can prevent and even reverse desynchronization, which is impressively visible in echocardiography with speckle tracing. His' bundle and left bundle branch pacing requires a specific implantation technique, sheaths and leads which can achieve successful stimulation of the conduction system in up to 98% of cases. Data on conduction system pacing have been acquired in numerous studies but only a few

A 68-year-old man with sick sinus syndrome (SSS) was referred to our department for pacemaker implantation. After implantation of a pacemaker with rate-responsive dual chamber (DDDR) mode and minimized ventricular pacing (MVP) functions, paroxysmal atrial fibrillation (PAF) repeatedly developed. Pacemaker memory showed that the intrinsic atrioventricular (AV) (atrial pacing-ventricular sensing [Ap-Vs]) interval was paradoxically prolonged during rate-responsive atrial single-chamber (AAIR) mode rapid pacing because of MVP. Accordingly, to eliminate the paradoxical prolongation of the AV interv

The absence of an inferior vena cava is a rare congenital condition often without clinical significance. Alternative venous approaches are often needed to treat these patients. We report a case of successful ablation of both isthmus dependent flutter and the AV junction using the superior vena cava in a patient with an inferior vena cava anomaly.

A 30-year-old man presented with narrow QRS tachycardia. The intracardiac electrocardiogram showed an atrial-HIS (AH) interval of 75 msec an … s most likely mediated by a left fasciculoventricular fiber which connects the HIS bundle below the atrioventricular node to the myocardial tissue of the left ventricle. The HIS-Purkinje system is used as an antegrade conduction limb and the fasciculoventricular fiber as a retrograde limb in the tachycardia circuit.

In patients with Wolff-Parkinson-White syndrome, right anteroseptal accessory pathways are uncommon and run from the atrium to the ventricle in close anatomic proximity to the normal AV conduction system. Radiofrequency catheter ablation is the first-line therapy for elimination of these accessory pathways. Although the initial success rate is high, there is a potential risk of inadvertent development of complete heart block, and the recurrence rate is relatively high. The capability of cryothermal energy to create reversible lesions (ice mapping) at less severe temperatures provides a potenti

Three months after orthotopic cardiac transplantation, a 46-year-old man developed paroxysmal supraventricular tachycardia. Electrophysiological investigation of the arrhythmia led to the diagnosis of an atrioventricular reentrant tachycardia involving a left lateral concealed accessory pathway. When antiarrhythmic drugs failed to suppress the arrhythmia, radiofrequency catheter ablation of the accessory pathway was performed without complication.

Automatic junctional tachycardia is frequently refractory to medical management and difficult to treat with nonpharmacological methods. A 12-year-old female with symptomatic, refractory automatic junctional tachycardia is reported. Earliest atrial activation during supraventricular tachycardia was in the posterior portion of the intraatrial septum. The patient underwent electrophysiological study and successful radiofrequency current ablation of the ectopic automatic focus within the atrioventricular junction. Normal atrioventricular junctional conduction was maintained, and at 7-month follow-

Prevention of recurrences of atrial fibrillation, slowing the ventricular rate during atrial fibrillation, and the acute management of atrioventricular junctional reentrant supraventricular tachycardia (paroxysmal supraventricular tachycardia) often require treatment with antiarrhythmic drugs. These drugs comprise a pharmacodynamically and pharmacokinetically heterogeneous group of agents whose individual properties determine correct use, contraindications and side effects. Stabilisation of sinus rhythm can be achieved with class IA and class IC sodium channel blocking drugs as well as with th

Percutaneous catheter ablation using radiofrequency energy can be used to interrupt atrioventricular (AV) conduction in patients with supraventricular tachycardia refractory to drugs. Results of radiofrequency ablation of the AV junction using a custom-designed catheter with a large, 3-mm-long distal electrode, 2-mm interelectrode spacing, and a shaft with increased torsional rigidity were compared with those using a standard quadripolar electrode catheter (Bard EP). An electrocoagulator (Microvasive Bicap 4005) supplied unmodulated radiofrequency current at 550 kHz, which was applied between

The purpose of this study was to ablate atrioventricular (AV) conduction in dogs with an argon laser using a transvascular approach. Six dogs were anesthetized and underwent a right thoracotomy and atriotomy. A bipolar #7 French lumen catheter containing a 400 u silica fiber was used to map the region of the AV junction. When a stable His deflection was obtained, the silica fiber was extruded from the end of the catheter and argon laser radiation was delivered for up to 20 s at 3.0-4.5 watts. In five of six dogs, complete heart block was successfully created after 3 to 15 lasings. A continuous

The value of a transcardiac lead system (coronary sinus to right ventricular apex) to record atrial and ventricular electrical activity and its pacing capabilities was assessed in 20 patients with a variety of tachycardias (atrial tachycardia in 3 patients, atrial flutter in 4, intranodal tachycardia in 6, circus movement tachycardia using an accessory pathway in 1 patient, and ventricular tachycardia in 9). The transcardiac lead invariably showed both atrial and ventricular electrical activity during sinus rhythm and tachycardias, allowing application of the same criteria as used when analyzi
